Circulation of multiple enterovirus serotypes causing hand, foot and mouth disease in India.

Hand, foot and mouth disease (HFMD), a common contagious disease that usually affects children, can be caused by enteroviruses. Coxsackievirus A16 (CV-A16) and enterovirus 71(EV-71) are the major aetiological agents of HFMD. Other EV serotypes, CV-A4-7, CV-A9-10, CV-B1-3, CV-B5, E-4 and E-19, have also been found associated with both sporadic infections and outbreaks of HFMD. In India, outbreaks of HFMD have been documented; however, molecular characterization of the aetiological agents has rarely been reported. Cases of HFMD were identified during 2009-2010 on the basis of clinical features i…

Enterovirus co-infections and onychomadesis after hand, foot, and mouth disease, Spain, 2008.

Hand, foot, and mouth disease (HFMD), a common disease caused by enteroviruses (EVs), usually affects children. Clustered and sporadic HFMD cases, followed by onychomadesis (nail shedding), occurred during summer and fall 2008 in Valencia, Spain. Fecal samples from onychomadesis patients, who did or did not have previous HFMD, and from healthy children exposed to onychomadesis patients tested positive for EV. The complete viral protein 1 capsid gene sequence was obtained for typing and phylogenetic analysis. Two EV serotypes, coxsackievirus A10 and coxsackievirus B1 (CVB1), were mainly detecte…

Impact of a diabetic foot care education program on lower limb amputation rate.

BACKGROUND: Diabetic foot complications are a leading cause of lower extremity amputation. With the increasing incidence of diabetes mellitus in the Arab world, specifically in the Kingdom of Saudi Arabia, the rate of amputation will rise significantly. A diabetic foot care program was implemented at King Abdulaziz Medical City in Riyadh, Saudi Arabia, in 2002. The program was directed at health care staff and patients to increase their awareness about diabetic foot care and prevention of complications. The purpose of this study was to perform a primary evaluation of the program's impact on th…

Case report: correction of neglected club foot deformity by arthroscopic assisted triple arthrodesis.

Neglected club foot deformity at the adulthood is difficult to correct. It is usually a rigid deformity associated with arthritic change of the hindfoot joint. Combined bone and soft tissue procedure is necessary to correct the deformity. We present a case of neglected club foot in adulthood who was successfully corrected with arthroscopic triple arthrodesis.

Improving gait stability in stroke hemiplegic patients with a plastic ankle-foot orthosis.

Stroke is the leading cause of long-term disability, and many stroke patients have hemiparesis. Hemiparesis induces ankle-control disturbances and equinovarus deformity, leading to difficulty in walking and an increased risk for falling. Plastic ankle-foot orthosis (PAFO) is frequently prescribed to correct ankle joint alignment and increase walking speed and stride length during ambulation. While several studies have shown that PAFO improves gait parameters, such as stride length and walking speed, in hemiplegic patients, the effect of PAFO on gait stability remains unclear. We quantitatively…

Ankle peak systolic velocity: new parameter to predict nonhealing in diabetic foot lesions.

The objective of this study was to determine whether ankle peak systolic velocity (APSV) can predict nonhealing in diabetic foot lesions. Diabetic patients referred for duplex scanning of the lower extremity arteries were included if they had foot lesions such as ulcers, gangrene, or tissue necrosis and had no palpable pedal pulses. End points were healed or healing foot lesions, revascularization, major amputation, or death. One hundred consecutive limbs were included. Forty-three limbs with diabetic foot lesions reached the end point of adequate healing or complete healing, whereas 57 limbs …

Initial experience with visualizing hand and foot tendons by dual-energy computed tomography.

PURPOSE: To assess the feasibility of visualizing hand and foot tendons by dual-energy computed tomography (CT). METHODS AND MATERIALS: Twenty patients who suffered from hand or feet pains were scanned on dual-source CT (Definition, Forchheim, Germany) with dual-energy mode at tube voltages of 140 and 80 kV and a corresponding ratio of 1:4 between tube currents. The reconstructed images were postprocessed by volume rendering techniques (VRT) and multiplanar reconstruction (MPR). All of the suspected lesions were confirmed by surgery or follow-up studies. RESULTS: Twelve patients (total of 24…

Vacuum-assisted closure for bilateral severe ischemic foot after revascularization: a case report.

Vacuum-assisted closure (VAC) therapy is a unique system that helps promote wound healing. We report a case of severe ischemic foot in which VAC therapy markedly improved wound healing. A 73-year-old man underwent left axillopopliteal bypass and left 3rd, 4th , and 5th digital amputations for gangrene. Although his amputation stumps were complicated with methicillin-resistant Staphylococcus aureus (MRSA) infection, the stumps were successfully healed by VAC. He also had gangrene in his right 1st toe, which could not healed by VAC alone, and we performed right femoropopliteal bypass and right 1…

Negative pressure wound therapy for soft tissue injuries around the foot and ankle.

BACKGROUND: This study was performed to evaluate the results of negative pressure wound therapy (NPWT) in patients with open wounds in the foot and ankle region. MATERIALS AND METHODS: Using a NPWT device, 16 patients were prospectively treated for soft tissue injuries around the foot and ankle. Mean patient age was 32.8 years (range, 3-67 years). All patients had suffered an acute trauma, due to a traffic accident, a fall, or a crush injury, and all had wounds with underlying tendon or bone exposure. Necrotic tissues were debrided before applying NPWT. Dressings were changed every 3 or 4 day…

High-energy foot and ankle trauma: principles for formulating an individualized care plan.

Care of the patient with high-energy foot and ankle trauma requires an individualized care plan. Staged treatment respecting the traumatized soft tissue envelope is often advisable. Wound care is a priority, and the vacuum-assisted closure dressing serves an integral role. Before definitive reconstruction, the surgeon needs to develop a treatment plan designed to match the unique personality of the patient and injury. Amputation is considered a rational treatment option for the patient with severe injury and poor host biology. Despite the most appropriate management, many severe foot and ankle…

New tendon transfer for correction of drop-foot in common peroneal nerve palsy.

UNLABELLED: Common peroneal nerve palsy has been reported to be the most frequent lower extremity palsy characterized by a supinated equinovarus foot deformity and foot drop. Dynamic tendon transposition represents the gold standard for surgical restoration of dorsiflexion of a permanently paralyzed foot. Between 1998 and 2005, we operated on a selected series of 16 patients with traumatic complete common peroneal nerve palsy. In all cases, we performed a double tendon transfer through the interosseous membrane. The posterior tibialis tendon was transferred to the tibialis anterior rerouted th…
