Type I atlanto-occipital dislocation complicated by non-communicating hydrocephalus - A case report.

Hydrocephalus, an extremely rare complication of craniocervical junction injuries, is postulated to result from compression of the fourth ventricular cerebrospinal fluid (CSF) outlets by fractured and displaced bone fragments, a swollen upper spinal cord or adhesions formed after a traumatic subarachnoid haemorrhage. We present the case of a 21-year-old woman for whom an injury to the cervical spine complicated by a type I atlanto-occipital dislocation contributed to the development of non-communicating hydrocephalus. The hydrocephalus was probably a consequence of impaired CSF circulation at

Imaging of sports ligamentous injuries of the elbow.

Ligamentous injuries of the elbow are common in sports. The diagnosis is usually based on a combination of the assessment of the mechanism of injury during the sporting activity and physical examination. Imaging with radiographs and MR imaging has an important diagnostic role, particularly when the clinical presentation is unclear and to evaluate for other important potential associated injuries such as myotendinous injuries, fractures, dislocations, and osteochondral lesions. Treatment includes conservative and surgical measures depending on the severity of the instability and associated inju

Pigmented villonodular synovitis causing painless chronic knee swelling in an adolescent.

It is uncommon for an adolescent to present with a painless chronic knee effusion without a history of trauma. Acute knee effusions are usually caused by internal derangement, patellar dislocations, or fractures. The differential diagnosis of a chronic knee effusion must include the more uncommon diagnoses of juvenile rheumatoid arthritis, septic arthritis, reactive arthritis, synovial osteochondromatosis, synovial hemangioma, synovial sarcoma, or pigmented villonodular synovitis. Radiographic imaging, plain radiographs, and magnetic resonance imaging in addition to a biopsy specimen are most

Injuries to the hip joint in frontal motor-vehicle crashes: biomechanical and real-world perspectives.

Hip fractures and dislocations in frontal crashes are of substantial concern to clinicians and automotive safety engineers because of the frequency at which hip injuries occur and the associated potential for long-term disability. Impacts to the flexed knees of unembalmed cadavers under loading conditions similar to those that occur in frontal crashes of newer-model vehicles indicate that the hip is the weakest part of the knee-thigh-hip complex and that hip injury tolerance is reduced by hip flexion and adduction from a typical driving posture. These results are being used to develop new knee

Cardiac reoperation in a patient with osteogenesis imperfecta: a case report.

A case of a 40-year-old man with dehiscence of the prosthetic aortic valve and recurrence of mycotic aneurysm of the left ventricular outflow tract with osteogenesis imperfecta is presented. He had an operation of aortic valve replacement and direct closure of the mycotic aneurysm for infective endocarditis twenty-one months ago. We performed reoperation of prosthetic aortic valve, patch closure of the mycotic aneurysm and graft replacement of the ascending aorta. He was complicated with multiple fractures of bilateral scapla and dislocation of left shoulder one postoperative day. Fortunately,

[The therapy of zygomatic fractures. Clinical and experimental studies].

The analysis of 120 patients with zygoma fractures showed disturbances of sensibility in the field of the N. infraorbitalis in 84% and pathological statements of eyes in 20%. The re-examinations of 76 of these patients 0.5-4.5 years postoperative showed that more than the half of these patients, who were treated by only the bone-retractor have a redislocation of the zygoma, and 49% of all cases have permanent damages of the N. infraorbitalis. The reason for the damage of the nerve is not only the compression of the nerve for a short time by the dislocated zygoma but also regular present additi

Radiography after orthognathic surgery. Part II. Surgical complications.

In a retrospective review of the radiographs from 100 patients who had undergone orthognathic surgery, the authors found that 10% of radiographs had evidence of surgical complications. These complications were fractures, temporomandibular joint dislocations, transected tooth roots, retained surgical devices, avascular necrosis, malunion or nonunion of osteotomy fragments, and osteomyelitis. Some of these complications are of little significance, while others require immediate intervention. Opacified maxillary sinuses and lucencies around tooth roots are frequent findings that may be confused w

[Pediatric femoral shaft fractures. Evaluation of 504 follow-up studies].

From 657 children who suffered from fractures of the femoral shaft during the years 1965-1980 504 could be followed-up in 13 hospitals of the GDR clinically and roentgenologically. 313 had undergone conservative treatment, 191 surgery with different methods. In all surgically treated cases there was overgrowth of the broken femoral shaft there was a shortening only in a few cases. In most cases the rotational deformity of the distal fragment persisted. Dislocation ad latus always had disappeared, whereas varus and expecially valgus deformities as well as antecurvation had improved partially. C

Distal Radioulnar Joint Dislocation Following Radius Bridging Plate for DRF.

BACKGROUND: This case report highlights two cases of progressive distal radioulnar joint dislocation following dorsal bridging plate (DBP) fixation for complex distal radius fractures (DRFs), a complication not previously documented in this situation. CASE DESCRIPTION: Both cases involved young patients with associated non-displaced basal ulnar styloid fractures, treated using DBP without addressing ulnar-sided lesions. Postoperative CT scan analysis revealed anterior translation of the distal radius metaphysis. LITERATURE REVIEW/CLINICAL RELEVANCE: The non-anatomic straight design of the pl

Arthroplasty for post-traumatic carpometacarpal joint (CMC) V osteoarthritis with minced cartilage: a case report.

OBJECTIVES: Fractures and dislocations of the bases of the fourth and fifth metacarpal bones are rare, accounting for <1 % of all hand injuries. If left untreated or inadequately fixed, there is a risk of persistent pain, functional impairment, and post-traumatic osteoarthritis. While common surgical approaches such as arthrodesis or suspension arthroplasty have been described, data on alternative treatment options remain limited. CASE PRESENTATION: This case report presents the first documented use of minced cartilage for treating post-traumatic CMC V osteoarthritis. A 17-year-old patient ex

Revision of ceramic fracture with ceramic-on-polyethylene in total hip arthroplasty: Medium-term results.

There is no consensus on the bearing of choice in revision for ceramic fracture after total hip arthroplasty (THA). The aim of this study was to evaluate the outcomes using ceramic-on-polyethylene (CoP) articulation in revision for ceramic breakage. Twelve patients who underwent revision hip surgery between 2002 and 2013 were followed-up. Appropriate surgical technique, including accurate synoviectomy, was used. The cup and the head were changed in four patients and only the liner and the head were replaced in the remaining eight patients. At the final follow-up there were no cases of re-revis

[Mitral regurgitation and atrial septum aneurysm complicated with osteogenesis imperfecta; report of a case].

A 43-year-old female with a history of osteogenesis imperfecta was admitted to our hospital for congestive heart failure due to mitral valve regurgitation. The patient had suffered from bone fractures 5 times, dislocation of elbow and fingers 3 times since childhood, and subarachnoid hemorrhage 2 years before. She and her son have blue scleras. She was diagnosed with osteogenesis imperfecta type IA clinically. Echocardiography revealed severe mitral regurgitation and aneurysmal formation in the interatrial septum bulging to the right atrium. Mitral valve replacement and repair of the atrial se

[Fixation of the tuberculum in multifragment fractures of the proximal humerus treated with hemiarthroplasty].

Hemiarthroplasty is an effective treatment for multifragmentary fractures of the proximal humerus. Purpose of this study was to compare 2 different methods of tuberosity dislocations, which is one of the primary reasons for successful posttraumatic arthroplasty. Between 2006 and 2008, 29 patients with multifragmentary fractures of the proximal humerus underwent hemiarthroplasty of the shoulder. Mean gae was 69y., while mean postoperative follow up was 27 months. We evaluated two different techiques of tuberosity fixation after anatomic reattachment on humeri: (A) intertuberosity suture stabiliz

Long-term results of total hip arthroplasty in congenital dislocation and dysplasia of the hip. A follow-up note.

Twenty-three of twenty-nine hips that were previously reported on were studied at an average of fourteen years (range, eight to 16.5 years) after total hip arthroplasty with cement. The original arthroplasties had been performed between November 1971 and January 1976. In all hips, there was dislocation or severe dysplasia. In six hips, superolateral bone grafts were used to increase acetabular coverage. At the latest follow-up examination, seven hips were rated excellent; nine, good; and one, fair. There were six failures (26 per cent) that were revised: four hips (17 per cent) had a fractured

Total hip arthroplasty after failed treatment of proximal femur fracture.

INTRODUCTION: Total hip arthroplasty (THA) is a good option as a salvage procedure after failed treatment of proximal femur fracture. The anatomy of the proximal femur, however, makes this surgery complicated and challenging. The purpose of this study was to evaluate the radiographic and clinical outcomes of THA after failed treatment of proximal femur fractures. MATERIALS AND METHODS: We retrospectively analysed 50 consecutive THAs (42 women, 8 men; mean age 77 years) after failed treatment of a proximal femur fracture. Mean postoperative follow-up was 58.1 months. Preoperative diagnoses wer

Pediatric Patellar Injuries.

BACKGROUND: Pediatric patellar injuries are common causes of anterior knee pain in children and adolescents and are frequently encountered in emergency and acute care settings. These conditions range from load-related overuse syndromes to acute traumatic injuries requiring timely recognition and referral. Unique features of the skeletally immature knee, including evolving ossification centers, open physes, and cartilaginous structures, may complicate diagnosis and contribute to missed or delayed recognition. OBJECTIVE: This review provides an evidence-informed, emergency medicine-oriented app

Is routine implant removal necessary after open reduction internal fixation of Lisfranc injuries? Comparing functional outcomes of routine and on-demand removal: A multicenter study.

INTRODUCTION: Lisfranc joint injuries are a severe cause of disruption of foot stability and function, often requiring surgical intervention such as open reduction and internal fixation (ORIF). The necessity of routine implant removal after healing remains controversial. This study aimed to compare functional recovery and postoperative complications among patients undergoing routine, on-demand, or no implant removal following ORIF for Lisfranc fracture-dislocations. MATERIALS AND METHODS: This multicenter retrospective study analyzed 188 patients treated with ORIF for Lisfranc fracture-disloc
