[New developments in joint surgery].

The treatment of joint injuries has seen a marked development in the last years. The surgical trauma of articular reconstruction was reduced and our deeper understanding of the mechanism of connective tissue healing influenced the rehabilitation program after joint injuries significantly. Indirect soft-tissue sparing reduction techniques can be applied to most joint fractures and improved fixation techniques using intraoperative fluoroscopy or arthroscopy allow to avoid vast exposures of the joint. The process of ligament healing requires joint movement and a certain stress to the healing tiss

Supracondylar fractures of the humerus in childhood: range of movement following the posterior approach to open reduction.

The posterior approach for open reduction of supracondylar fractures of the humerus has been condemned for causing decreased elbow movement. This study investigates this by comparing the range of movement in children treated by posterior open reduction and Kirschner wiring with those treated by closed reduction and immobilization. A total of 65 children with severely displaced supracondylar humeral fractures have been reviewed. There was some loss of movement in 66 per cent of the open reduction group and 42 per cent of the closed reduction group. The proportion losing more than 10 degrees of

Irreducible distal radioulnar joint occurring in a Galeazzi fracture - case report.

This is a report of an irreducible distal radioulnar joint associated with a Galeazzi fracture. The irreducible dislocation of the radioulnar joint was not appreciated until 2 weeks following compression plating of the radius fracture. The extensor carpi ulnaris was found to be wrapped anteriorly about the ulnar border of the distal ulna and was trapped in this position by the displaced ulnar styloid. The patient was successfully treated with open reduction of the styloid fracture and temporary wire fixation.

[Method of extrafocal osteosynthesis of compound fractures of the lower end of the radius].

The Ilizarov device was used for extrafocal osteosynthesis in complex fractures of the distal end of radius. With the aid of the device the bone fragments can be joined and kept in a correct position till a callus is formed. The mean time of fixation in the apparatus is equal to 21 days. In 114 patients 117 operations were performed. Good remote results were obtained in 73 of 99 patients (73,7%), satisfactory results in 25 (25,3%).

Non-Operative Management of a Failed Olecranon Fracture Fixation Leading to Functional Recovery: A Case Report.

An 85-year-old female walker dependent presented with a displaced olecranon fracture following a fall. Open reduction and internal fixation (ORIF) using an anatomic olecranon locking plate was performed. Two-week postoperative X-rays revealed fixation failure with persistent fragment displacement and a wide fracture gap. To manage this condition, non-operative management, including sling immobilization for 4 weeks and progressive physical therapy, was pursued with the elbow at 90 degrees flexion. At the 6-month follow-up, the elbow was pain-free with functional range of motion (ROM) being achi

Fixation with multiple K-wires and PRP injection as an effective way to manage heel pad avulsion.

The treatment of acute heel pad avulsion differs significantly from managing other soft tissue injuries due to its unique soft tissue structure. This case report outlines a scenario involving a male in his 20s who suffered heel pad avulsion without a calcaneal fracture but with an ipsilateral medial malleolus fracture after a twisting injury to the ankle caused by a road traffic accident. Immediate action was taken within 24 hours of the injury, involving thorough debridement of the wound, fixation to the calcaneum using multiple K-wires, primary suturing and internal fixation of the medial ma

Radiocarpal fracture-dislocations.

Radiocarpal fracture-dislocations most often are caused by high-energy trauma. These difficult, uncommon injuries involve significant soft-tissue and osseous trauma, requiring meticulous reduction and fixation. The mechanism of injury is generally a severe shear or rotational insult. Anatomically, the dislocation results in disruption of the radiocarpal ligaments and, usually, both the radial and the ulnar styloid. Understanding the anatomy of the radiocarpal joint is central to understanding the osseous and soft-tissue constraints that are disrupted with a radiocarpal dislocation. Diagnosis c

[Stress fracture of the internal sesamoid bone of the first metatarsophalangeal joint. Case report].

We present the case of a fourty-two year old truck driver without a major trauma history who was diagnosed in our unit as having a stress fracture of the first metatarsophalangeal joint medial sesamoid bone. Conservative treatment consisting of 30 day immobilisation was our treatment choice. The result was very good and at six month follow-up the patient had no complains. We have searched for other opinions but there is a paucity of published literature on the injuries of the lesser toe sesamoids of the foot. The literature review revealed different types of treatment such as: conservative tre

Gauze Fixation Technique for Stabilization of Depressed Nasal Bone Fracture: A Novel Approach.

BACKGROUND: Reduction and stabilization of depressed nasal bone fractures often rely on nasal packing, which may become dislodged, resulting in recurrent displacement. Open reduction with internal fixation is effective but more invasive, costly, and associated with visible scars. This study introduced and evaluated a novel percutaneous gauze fixation technique for stabilizing nasal bone fractures. METHODS: This retrospective study was conducted between 2023 and 2024 and included 10 patients with nasal bone fractures treated by a single plastic surgeon. All underwent closed reduction followed

Intercondylar eminence fracture treated by resorbable magnesium screws osteosynthesis: A case series.

INTRODUCTION: Tibial spine avulsion fractures are mostly a paediatric injury which appropriate treatment is currently debated in literature. The choice between conservative and surgical treatment is based on the radiographic classification of Meyers-McKeever. The most diffused surgical techniques involve either internal fixation devices (screws) or bone tunnels fixation with resorbable sutures. Today, a third option is represented by resorbable magnesium screws which could combine the best features of the two classical systems. Objective of this study is to investigate the efficacy of these ne

Obturator dislocation of the hip with ipsilateral femoral neck fracture: A case report.

Obturator dislocation of the hip associated with ipsilateral femoral neck fracture is an unusual injury. We report a case of a 40-year-old man with such a combination of injuries which was treated with an open reduction and internal fixation. He has a good follow-up result. There was no evidence of avascular necrosis on radiographs after 3 years.

Non-union of Weber B distal fibula fractures: a case series.

Non-union of distal fractures of the fibula type Weber B is rare. Undisplaced and minimally displaced isolated lateral malleolar fractures (Weber B) usually unite without operative intervention. We present three cases of lateral malleolus non-union in healthy individuals. Two were treated with internal fixation plus bone grafting. The third patient remained asymptomatic and therefore did not undergo surgery for the fracture.

Comparison of Risk Factors and Postoperative Outcomes in Syndesmosis Injuries with Various Ankle Fracture Types.

There is a lack of understanding of risk factors and postoperative outcomes of syndesmotic injuries with singular versus multipart fractures. A retrospective analysis was done between 2008 and 2016, utilizing the American College of Surgeons National Surgical Improvement Program database. Patients with open reduction and internal fixation of isolated lateral malleolus fractures, bimalleolar fractures, or trimalleolar fractures were identified. A total of 2045 patients underwent syndesmotic fixation. Subjects with bimalleolar or trimalleolar fractures were more likely than those with unimalleol

Simultaneous Open Reduction and Corrective Osteotomy for a Child With Lateral Condylar Fracture and Cubitus Varus Deformity Following Supracondylar Fracture: A Case Report.

Posttraumatic cubitus varus deformity in pediatric patients may cause second fractures of the distal humerus. Corrective osteotomy is used to obtain good alignment and is generally performed for patients with prolonged deformity or bony union after fracture. We report the case of a 10-year-old boy who presented with elbow pain after falling. Plain radiography showed lateral condylar fracture and cubitus varus deformity. This injury was the fourth fracture of the same distal humerus. Open reduction and internal fixation for lateral condylar fracture and lateral closing wedge osteotomy for cubit

Incidence and risk factors of knee injuries associated with ipsilateral femoral shaft fractures: A multicentre retrospective analysis of 429 femoral shaft injuries.

A femoral shaft fracture is usually a high-energy injury and, thus, is likely to be accompanied by an injury of adjacent joints such as a knee ligament injury. However, these associated injuries are often neglected because of severe pain and deformity. The purpose of the current study is to evaluate the incidence, type and risk factors of ipsilateral knee injuries associated with femoral shaft fractures. A total of 429 femoral shaft fractures were included in this study from January 2010 to September 2015. There were 320 males and 109 females, with mean age of 40.7 years (range, 15-88). Exclus
