Literature Review of Postoperative Distal Radius Fracture Immobilization Recommendations.

Distal radius fractures (DRFs) are a commonly treated injury in orthopedics. DRFs have a high incidence across patient demographics, including pediatrics, young patients in high-energy trauma, and geriatric patients in low-energy trauma. While DRFs occur across a large range of age groups, they are especially consequential in geriatric patients with osteoporosis. Management of DRFs has extensive variability, ranging from conservative casting to surgical interventions, such as open reduction and internal fixation surgical procedures. The diversity of treatment options for DRFs is due to a consi

GDF-15 plasma levels are elevated in mobility-limited older adults with frailty and sarcopenia-results from the BIOFRAIL study.

Growth differentiation factor 15 (GDF-15) has been proposed as a potential biomarker for the geriatric syndromes of frailty and sarcopenia. This study investigated plasma GDF-15 as a biomarker associated with sarcopenia and frailty in a cohort of mobility-limited older adults. Patients (≥ 65 years) were recruited from a geriatric outpatient clinic for fall assessment. Frailty was identified using the Clinical Frailty Scale with a cut-off ≥ 5. Sarcopenia was defined according to the European Working Group on Sarcopenia in Older People 2. Plasma GDF-15 was analyzed using electrochemiluminescence

Type 2 Odontoid Fractures: Atlantodental Arthrosis as a Novel Risk Factor for Failure of Conservative Management.

STUDY DESIGN: Retrospective case-control study. OBJECTIVE: The purpose of this study is to assess how the presence and grade of atlantodental arthrosis impact outcomes of nonoperatively managed type 2 odontoid fractures. SUMMARY OF BACKGROUND DATA: The nonoperative management of geriatric type 2 odontoid fractures requires consideration of all potential variables that may influence outcomes. The presence of underlying atlantodental arthrosis can create a more rigid lever arm adjacent to the fracture site inducing greater biomechanical strain on an already tenuous healing environment. METHOD

Age and orthostatic hypotension are associated with baroreflex sensitivity and cerebral oxygenation after postural change.

Baroreflex sensitivity (BRS), maintaining blood pressure (BP), and cerebral autoregulation, maintaining cerebral blood flow (CBF), are regulatory mechanisms to counteract posture-related BP changes and their effect on CBF. These mechanisms may fail in geriatric conditions such as orthostatic hypotension (OH) and cause symptoms and falls. This study aimed to determine the association of age, sex, antihypertensive use, comorbidity, and OH with BRS and cerebral oxygenation after postural change. Thirty-four younger adults (median age 25 years), 30 older adults (median age 77 years), and 41 geriat

Strategies to Improve Emergency Transitions From Long-Term Care Facilities: A Scoping Review.

BACKGROUND AND OBJECTIVES: Older adults residing in residential aged care facilities (RACFs) often experience substandard transitions to emergency departments (EDs) through rationed and delayed ED care. We aimed to identify research describing interventions to improve transitions from RACFs to EDs. RESEARCH DESIGN AND METHODS: In our scoping review, we included English language articles that (a) examined an intervention to improve transitions from RACF to EDs; and (b) focused on older adults (≥65 years). We employed content analysis. Dy et al.'s Care Transitions Framework was used to assess t

Cholesterol Lowering in Older Adults: Should We Wait for Further Evidence?

PURPOSE OF REVIEW: Current guidelines for primary and secondary prevention of cardiovascular events in adults up to age 75 years are well-established. However, recommendations for lipid-lowering therapies (LLT), particularly for primary prevention, are inconclusive after age 75. In this review, we focus on adults ≥ 75 years to assess low-density lipoprotein-cholesterol (LDL-C) as a marker for predicting atherosclerotic cardiovascular disease (ASCVD) risk, review risk assessment tools, highlight guidelines for LLT, and discuss benefits, risks, and deprescribing strategies. RECENT FINDINGS: The

Effects of resistance training and nutritional support on osteosarcopenia in older, community-dwelling postmenopausal Korean females (ERTO-K study): a study protocol.

BACKGROUND: Osteosarcopenia is geriatric syndrome defined as the concomitant occurrence of osteopenia/osteoporosis, and sarcopenia. Osteosarcopenia is a relatively new concept in geriatric medicine; however, it may increase the risk of fragility fractures, several morbidities and mortalities, and socioeconomic costs. Although resistance exercises and nutritional support-including protein, calcium, and vitamin D-are potential non-pharmacological management procedures, evidence is still lacking. The objective of this study was therefore to evaluate the effect of combined resistance exercise and

Association of Vulnerability Screening on Hospital Admission with Discharge to Rehabilitation-Oriented Care after Acute Hospital Stay.

BACKGROUND: We assessed the vulnerability of patients aged ≥70 years during hospital admission based on the Short Dutch Safety Management Screening (DSMS). Screening of four geriatric domains aims to prevent adverse outcomes and may support targeted discharge planning for post-acute care. We explored whether the DSMS criteria for acutely admitted patients were associated with rehabilitation-oriented care needs. METHODS: This retrospective cohort study included community-dwelling patients aged ≥70 years acutely admitted to a tertiary hospital. We recorded patient demographics, morbidity, funct
