Avoiding compression of extracardiac valved conduits.

Extracardiac valved conduits are routinely employed in the correction of complex congenital heart lesions. Compression of a conduit and/or of the heart may present serious operative and postoperative complications. We believe its occurrence can be minimized and hemodynamic results improved by adhering to simple surgical techniques. It is important to select a conduit which will produce minimal resistance to flow. Partial or subtotal thymectomy may increase the mediastinal space to accommodate the conduit. The conduit should be cut into the appropriate shape and should be placed in such a way t

2025 ESC/EACTS valvular heart disease guidelines: key updates and surgical implications for cardiac surgeons.

PURPOSE OF REVIEW: The 2025 ESC/EACTS Guidelines for valvular heart disease introduce substantial changes in timing and modality of intervention, making a focused appraisal of their surgical implications highly relevant for contemporary cardiac practice. RECENT FINDINGS: The guidelines formalize imaging-based staging of cardiac damage, lower thresholds for intervention in aortic stenosis and primary mitral regurgitation, and refine age- and risk-stratified allocation of surgery vs. transcatheter therapies across aortic, mitral, and tricuspid disease. They expand indications for transcatheter

Maternal and Fetal Outcomes in Pregnant Patients With Mechanical and Bioprosthetic Heart Valves.

Background Guidelines for choice of prosthetic heart valve in people of reproductive age are not well established. Although biologic heart valves (BHVs) have risk of deterioration, mechanical heart valves (MHVs) require lifelong anticoagulation. This study aimed to characterize the association of prosthetic valve type with maternal and fetal outcomes in pregnant patients. Methods and Results Using the 2008 to 2019 National Inpatient Sample, we identified all adult patients hospitalized for delivery with prior heart valve implantation. Multivariable regressions were used to analyze the primary

Cardiohepatic Syndrome Is Associated With Poor Prognosis in Patients Undergoing Tricuspid Transcatheter Edge-to-Edge Valve Repair.

OBJECTIVES: The aim of this study was to evaluate the prevalence and prognostic implications of cardiohepatic syndrome (CHS) in patients with tricuspid regurgitation (TR) treated with tricuspid transcatheter edge-to-edge valve repair (T-TEER). BACKGROUND: The role of CHS in patients undergoing T-TEER for severe TR has not been studied. METHODS: This study included patients who underwent T-TEER for TR between 2016 and 2020 at 2 high-volume academic centers. CHS was defined as elevation of at least 2 of 3 cholestatic hepatic enzymes. The impact of CHS on 1-year all-cause mortality and clinical

Tricuspid Regurgitation in Congestive Heart Failure: Management Strategies and Analysis of Outcomes.

Tricuspid regurgitation is a notable aspect of congestive heart failure and is linked with worse outcomes if untreated. Functional tricuspid regurgitation commonly is seen in patients with heart failure, particularly in patients presenting for surgical management, such as those for mechanical cardiac assist device implantation. This review aims to study the published data related to the surgical management of tricuspid regurgitation in the cardiac surgical population comprehensively.

Therapeutic options for functional mitral regurgitation in chronic heart failure.

INTRODUCTION: Mitral regurgitation is a common finding in patients with chronic heart failure and is associated with a progressive worsening of symptoms, reduced survival and increased cost of care. However, the use of mitral valve surgery for these patients remains controversial and has not been shown to improve survival. Consequently, research has been increasingly directed towards the nonsurgical management of this important co-morbidity of heart failure. AREAS COVERED: The present review will describe the relevance of mitral regurgitation in people with chronic heart failure, the current

[Analysis of Cases of Thrombolytic Therapy Administration for Acute Thrombosis of Mechanical Prosthetic Valves].

Mechanical prosthetic valve thrombosis is a life-threatening complication that in most cases requires urgent re-intervention. A significant risk of intraoperative complications, death and presence of a small fresh thrombus suggest thrombolytic therapy to be an alternative option of treatment because of a relatively low risk of the procedure. This article presents successful experience of systemic thrombolysis in patients with acute thrombosis of mechanical prosthetic valves.

[ESC guidelines on the management of valvular heart disease. What has changed and what is new?].

In 2012 the new and collaborative "Guidelines on the management of valvular heart disease (version 2012)" were published by the European Society of Cardiology (ESC) and the European Association for Cardio-Thoracic Surgery (EACTS). These guidelines emphasize that decision-making in patients with valvular heart disease should ideally be carried out by a"heart team" with particular expertise in valvular heart disease. In aortic regurgitation pathologies of the aortic root are frequent and in patients with Marfan syndrome, surgery is indicated when the maximal ascending aortic diameter is ≥50 mm,

Aortic stenosis and the failing heart.

The combination of aortic stenosis and left-ventricular dysfunction is a challenging situation for the physician. Diagnosis of this condition requires a detailed evaluation to understand the etiology and reversibility of the ventricular dysfunction and to accurately determine the real severity of the stenosis. Whether the aortic stenosis the cause of the left ventricular failure or is an independent disease has significant diagnostic, prognostic and therapeutic implications. Dobutamine echocardiography provides critical information to determine the real severity and the left ventricle's potent

Advances in percutaneous valvular intervention.

We are entering a new era in the percutaneous treatment of valvular heart disease. Novel techniques and devices have given rise to the possible treatment of a range of valvular heart diseases that previously necessitated surgical therapies, including aortic stenosis, pulmonary regurgitation and mitral regurgitation. Despite the enormous potential of these percutaneous therapies, enthusiasm needs to be balanced by an understanding of the challenges that need to be overcome before such therapies can be widely embraced. This review provides a critical assessment of the status of the major develop

Nonsurgical pulmonary valve replacement: why, when, and how?

Percutaneous transcatheter interventions for valve replacement or implantation is one of the most exciting developments in the field of interventional cardiology. Valvular stenosis has been treated by balloon dilatation with early and late results; however, treatment for valvular regurgitation has remained surgical until now. Most new designs have been investigated for implantation of valves in the left or right ventricular outflow tracts. Patients with surgery on the right ventricular outflow tract for congenital heart disease constitute the most common group for reoperations during late foll

["Open" surgery of mitral heart diseases complicated by pulmonary hypertension].

Under analysis were the results of 386 operations on the "open" heart made for mitral diseases complicated by pulmonary hypertension of different degrees. Prosthetics of the mitral valve was performed in 251 patients, in 135 patients the so-called "organ-saving" correction of the defect was fulfilled. The decision on the method of the defect correction depends on the anatomical particularities, morphological alterations of the valvular apparatus. The main place among the causes of postoperative lethality (9-11%) is occupied by cardiac insufficiency and renohepatic failure which are noted mainl
