Treatment Satisfaction with Nonoperative Management of Suspected Hip Fractures in Nursing Home Patients with a Do-Not-Hospitalize Directive: A Prospective Case Series (NONU-HIP).

ObjectivesSome nursing home residents opt to forgo hospital admission in case of a suspected hip fracture due to the poor prognosis. However, outcomes in these patients are unknown and hamper advance care planning and expectation management. This study assesses treatment satisfaction and quality of life in nursing home residents with a suspected hip fracture and a do-not-hospitalize directive.MethodsA prospective case series study was conducted in three nursing home organizations in The Netherlands. The primary outcome was the treatment satisfaction according to proxies and caregivers. Seconda…

Delayed Primary Hip Arthroplasty for Geriatric Low-Energy Femoral Neck Fracture Is Not Associated With Worse Outcomes: A Study of the Arthroplasty for Hip Fracture Consortium.

BACKGROUND: There is no consensus on the association between mortality and surgical delay in hemiarthroplasty (HA) or total hip arthroplasty (THA) for elderly, low-energy, displaced femoral neck fractures (FNFs). This study assessed whether delayed surgery is associated with worse outcomes. METHODS: This retrospective study reviewed FNF in patients > 60 years among nine academic tertiary-care institutions from January 2010 through December 2019. A total of 1,420 FNF patients underwent HA (n = 939) or THA (n = 481) < 48 hours (86.5%; n = 1,229) or ≥ 48 hours (13.5%; n = 191) from admission. Co…

Intraoperative Findings in Total Hip Arthroplasty Leading to a Diagnosis of Alkaptonuria in a Patient With Severe Hip Osteoarthritis Initially Attributed to Rheumatoid Arthritis: A Case Report.

Alkaptonuria is a rare inherited condition caused by elevated levels of homogentisic acid, which confers a characteristic dark color on tissues like cartilage and bones over time, a process known as ochronosis. We present the case of a 62-year-old male patient with a presumed diagnosis of rheumatoid arthritis and severe pain and stiffness in his right hip. Plain radiographs revealed severe right hip osteoarthritis. The patient underwent total hip arthroplasty (THA) without any perioperative complications. The intraoperative findings (black ring-like spot at the femoral head and also at the car…

The Lateral Acetabular Registration Technique of Robot-Assisted Total Hip Arthroplasty for Arthrodesed Hips: A Retrospective Cohort Study With a One-Year Follow-Up.

BACKGROUND: The hip joint is one of the peripheral joints most commonly affected by ankylosing spondylitis (AS). Previous research showed that compared to manual total hip arthroplasty (THA), robot-assisted THA significantly improves the likelihood of achieving cup positioning within the target zone. METHODS: In this study, we investigated robot-assisted THA for arthrodesed hips, including fused hip joints and fibrously fused hip joints. We retrospectively analyzed patients who had AS who underwent THA in our hospital from January 2021 to October 2023. There were 60 patients who underwent rob…

Optimal retractor insertion point for nerve safety during total hip arthroplasty: an anatomical study on the femoral and sciatic nerves in relation to hip motion.

BACKGROUND: Nerve injury is one of the most serious complications of total hip arthroplasty (THA). It is suspected to be a result from nerve compression or direct injury caused by an acetabular retractor. The anatomical relationship between the acetabular rim and the femoral and sciatic nerves, including hip motion, has not been investigated. This study aimed to identify the optimal position for retractor insertion during THA to prevent nerve damage. METHODS: A total of 28 hip joints from 14 freshly frozen cadavers were used. Using an anterolateral approach, each cadaver was immobilised in th…

Effect of Acetabular Labral Tear Orientation on Hip Joint Kinematics: A Comparison of Radial Tears, Chondrolabral Junction Tears and Complex Tears in Cadaveric Hips.

BACKGROUND: Acetabular labral tear morphology or orientation may influence hip stability. HYPOTHESIS: A radial tear of the acetabular labrum would result in greater rotational and translational motion compared with a chondrolabral separation. STUDY DESIGN: Controlled laboratory study. METHODS: Included were 12 unpaired nonarthritic hip specimens, none of which had capsular laxity (8 male; mean age, 34.5 years). The specimens were stripped of all soft tissue except the hip capsule and labrum and, then potted using a custom jig. In 6 specimens, a 1-cm anterosuperior separation of the labrum f…

Effect of lumbosacral transitional vertebra on developmental alterations of the hip: a quantitative investigation of the lumbo-pelvic-hip complex via whole-body computed tomography.

BACKGROUND: Lumbosacral transitional vertebra (LSTV) is a common spinal variant, with the reported prevalence varying from 8.1% to 36%. LSTV has been shown to alter the lumbo-pelvic parameters and reduce the benefits of total hip arthroplasty, but the specific effects of LSTV on hip development remain unclear. The aim of this study was thus to investigate the impact of LSTV on developmental alterations of the hip. METHODS: A total of 310 individuals were categorized into three groups according to whole-body computed tomography (CT) imaging: a group with sacralization of 23 presacral vertebrae…

No increased mortality after total hip arthroplasty in patients with a history of pediatric hip disease: a matched, population-based cohort study on 4,043 patients.

Background and purpose - Patients with pediatric hip diseases are more comorbid than the general population and at risk of premature, secondary osteoarthritis, often leading to total hip arthroplasty (THA). We investigated whether THA confers an increased mortality in this cohort.Patients and methods - We identified 4,043 patients with a history of Legg-Calvé-Perthes disease (LCPD), slipped capital femoral epiphysis (SCFE), or developmental dysplasia of the hip (DDH) in the Swedish Hip Arthroplasty Register (SHAR) between 1992 and 2012. For each patient, we matched 5 controls from the general …

Innervation of the hip joint capsular complex: A systematic review of histological and immunohistochemical studies and their clinical implications for contemporary treatment strategies in total hip arthroplasty.

The hip joint capsule contributes to the stability of the hip joint and lower extremity, yet this structure is incised and often removed during total hip arthroplasty (THA). Increasing incidence of osteoarthritis is accompanied by a dramatic rise in THAs over the last few decades. Consequently, to improve this treatment, THA with capsular repair has evolved. This partial restoration of physiological hip stability has resulted in a substantial reduction in post-operative dislocation rates compared to conventional THA without capsular repair. A further reason for the success of this procedure is…

Higher prevalence of periprosthetic fractures with ceramic on polyethylene hip bearing compared with ceramic on ceramic on the contralateral side: a forty year experience with hip osteonecrosis.

PURPOSE: It is unclear whether late THA periprosthetic femoral fractures are related to a mechanical mechanism that decreases strength of the femur (for example, loosening) or to a biological problem as osteolysis. It is also unknown if ceramic on ceramic bearing couples decrease the risk of late periprosthetic fractures as a result of the absence of wear and osteolysis. MATERIAL AND METHODS: We therefore asked whether the cumulative long-term fractures were different according to the couple of friction ceramic on ceramic or ceramic on polyethylene in 327 patients (654 hips) with bilateral TH…

Untreated congenital and posttraumatic high dislocation of the hip treated by replacement in adult age: 22 hips in 16 patients followed for 1-8 years.

Between 1993 and 2001, we replaced 22 hips in 16 patients with high dislocation of the hip. All but 1 case was due to congenital dislocation. In all cases the femur was osteotomized below the lesser trochanter and a straight uncemented stem inserted in the medullary canal. The acetabular prostheses were inserted at the site of the original acetabulum, usually after augmenting the acetabular rim by using the medial half of the resected proximal femur. The lateral part of the proximal femur with the intact attachment of the gluteus medius muscle was transposed and fixed to the femoral diaphysis …

[Radiological status of hip joints of children at the beginning of walking, when treatment of congenital hip dysplasia and dislocation was begun in the first two weeks of life].

This paper describes the radiological status on the start of walking in 31 hip joints in 24 children, in which the treatment of the developmental dysplasia with dislocation of the hip began in the first 2 weeks. Before the beginning of the treatment, ultrasonographic assessment revealed type D in 22% hips, type III in 55% and type IV in 23%. The radiographic evaluation was based on 6 parameters, which values were compared to the normal ones for the age group. It was estimated that only 32% hips had a correct radiological status, the other hips revealed smaller or greater dysplasia.
