Rethinking guidelines for VTE risk among nursing home residents: a population-based study merging medical record detail with standardized nursing home assessments.

BACKGROUND: Nursing home (NH) residents are at increased risk for both VTE and bleeding from pharmacologic prophylaxis. Construction of prophylaxis guidelines is hampered by NH-specific limitations with VTE case identification and characterization of risk. We addressed these limitations by merging detailed provider-linked Rochester Epidemiology Project (REP) medical records with Centers for Medicare and Medicaid Services Minimum Data Set (MDS) NH assessments. METHODS: This population-based nested case-control study identified all Olmsted County, Minnesota, residents with first-lifetime VTE Oc

Hospital readmissions after surgical treatment of proximal humerus fractures: is arthroplasty safer than open reduction internal fixation?

BACKGROUND: With technologic advances such as locked periarticular plating, hemiarthroplasty of the humeral head, and more recently reverse total shoulder replacement, surgical treatment of proximal humerus fractures has become more commonplace. However, there is insufficient information regarding patient outcomes after surgery, such as the frequency of unplanned hospital readmissions and factors contributing to readmission. QUESTIONS/PURPOSES: We measured (1) the frequency of unplanned hospital readmissions after surgical treatment of proximal humerus fractures, (2) the medical and surgical

Association between global biomarkers of oxidative stress and hip fracture in postmenopausal women: a prospective study.

Human studies suggest that oxidative stress is a risk factor for osteoporosis, but its relationship with fracture risk is poorly understood. The purpose of the present study was to investigate the association between biomarkers of oxidative stress and hip fracture in postmenopausal women. We conducted a prospective study in the Nurses' Health Study among 996 women aged 60 years or older at baseline blood collection in 1989-1990. Plasma fluorescent oxidation products (FlOPs) were measured at three excitation/emission wavelengths (360/420 nm named as FlOP_360; 320/420 nm named as FlOP_320; and 4

Cost effectiveness of transcatheter aortic valve replacement compared to medical management in inoperable patients with severe aortic stenosis: Canadian analysis based on the PARTNER Trial Cohort B findings.

OBJECTIVE: The only effective treatment for severe aortic stenosis (AS) is valve replacement. However, many patients with co-existing conditions are ineligible for surgical valve replacement, historically leaving medical management (MM) as the only option which has a poor prognosis. Transcatheter Aortic Valve Replacement (TAVR) is a less invasive replacement method. The objective was to estimate cost-effectiveness of TAVR via transfemoral access vs MM in surgically inoperable patients with severe AS from the Canadian public healthcare system perspective. METHODS: A cost-effectiveness analysis

Overt and hidden forms of chronic malnutrition in the rat and their relevance to man.

We have examined the physiological weight changes seen in rat dams and their offspring as sequelae of either an overt or a hidden form of chronic protein malnutrition. In the overt model, which was produced by feeding dams a very low protein diet (6% casein) starting 5 weeks prior to conception and continued through lactation, the females showed significant weight losses at all ages compared to dams maintained on a normal diet (25% casein). This caused the malnourished 6% dams to have offspring that were categorized as small-for-date at birth in terms of their weight indices and peripheral met

"My mom chose my brother over me": The influence of the parent-child bond on coping with a sibling's cancer.

UNLABELLED: Childhood cancer has a profound impact within families, affecting all members, including siblings. They may experience psycho-emotional difficulties due to changes in family dynamics resulting from the prioritization of care for the sick sibling, which can compromise the parent-child bond. OBJECTIVE: To understand how the parent-child bond influences the psycho-emotional coping of healthy adolescents and young adults in response to a sibling's cancer diagnosis. METHODS: A descriptive qualitative design based on Krippendorff's content analysis. Narratives from 12 siblings and 6 ac

Knowing Pain Without Certainty: Recognition When Self-Report is Unavailable.

Pain assessment often follows a hierarchy: self-report is treated as the gold standard, while behavioural and physiological evidence are used when the report is unavailable. This protects patients against professional disbelief but can make those unable to self-report appear unable to contribute to knowledge about their pain. I argue that verbal report and nonverbal expression belong to one interpretive practice while retaining different epistemic roles. Self-report carries presumptive first-person authority because it conveys experience, meaning, and preferences. Behavioural evidence is indir

Adoption, scale-up, and sustainability of a neuropalliative care model: qualitative exploration of pre-implementation barriers and enablers.

BACKGROUND: People with long-term neurological conditions such as Alzheimer's dementia and Parkinson's disease live with these increasingly debilitating conditions for years. However, palliative care is provided only towards the end of life. To address the gap between patient needs and services provided, we piloted an early neuro-palliative model of care - Neurosupport. In this study, we explored pre-implementation barriers and enablers for the adoption, scale-up, and sustainability of Neurosupport. METHODS: We conducted semi-structured interviews using an interview guide based on the Theoret

Frailty Screening in the Emergency Department Enables Personalized Multidisciplinary Care for Geriatric Trauma Patients.

Frailty is a multidomain reduction in physiologic reserve that impacts recovery and can contribute to poor outcomes following trauma beyond what chronological age, comorbidities, or injury severity predicts. In geriatric trauma patients, a large proportion are frail or prefrail on initial encounter in the emergency department, and because there are opportunities for actionable management plans, major trauma guidelines endorse systematic screening integrated into coordinated geriatric trauma care. We reviewed the literature and identified practical instruments used in the acute trauma setting f

Bearing the Weight of End-of-Life Work: A Qualitative Study of Emotional Experience and Coping Among Palliative Care Professionals in Northern Italy.

Background: Palliative care is a clinically, relationally and emotionally demanding field. Continuous exposure to suffering and death exposes professionals to compassion fatigue, vicarious trauma and grief, and burnout, while also offering opportunities for compassion satisfaction and meaning-making. How professionals cope is central to sustaining the supporting role in diseases with a poor prognosis. Objectives: To explore the emotional and experiential dimensions of palliative care work and the coping strategies, individual and team-based, that professionals deploy when accompanying patients
