Validation of ultrasound methods to monitor gastric volume changes in preterm infants.

OBJECTIVES: The aim of this study was to validate 4 sonographic methods of estimating stomach volume for the purpose of monitoring changes in gastric volume over time and to describe the echogenic characteristics of milk feeds. METHODS: Twenty-four infants were monitored during a single intragastric tube feed, with 2 ultrasound images of the entire stomach and an image of the antral cross-sectional area (ACSA) before, during, and after the feed. Raw measurements, 3 stomach volume calculations, and ACSA were tested for intra- and interrater agreement. Calculated stomach volumes and ACSA were c

Behavioral measures and EEG monitoring using the Brain Symmetry Index during the Wada test in children.

EEG monitoring is used routinely during the Wada test in children. We quantified EEG asymmetry using the Brain Symmetry Index (BSI) to reduce subjectivity of EEG interpretation. Clinical and procedural variables were obtained and EEG data were retrieved from 46 patients with a total of 89 injections. The BSI, the absolute value of the relative difference of the average spectral density of the right and left hemisphere, was calculated over time for all EEGs. Lateralized slowing was correctly identified in all procedures. Asymmetry was minimal at baseline (BSI 0.16) and increased with injection

Lack of correlation of overnight monitoring data and lumbar infusion data in iNPH patients.

OBJECTIVE: Hydrodynamic theories of idiopathic normal pressure hydrocephalus (iNPH) favor a decrease in compliance as an important underlying principle of the disease's immanent pathophysiology. ICP overnight monitoring (ONM) and lumbar infusion study (LIS) are both established methods that aid as supplemental tests in the selection of shunt-responsive patients. Pulse wave amplitude (AMP) and RAP index are measures derived from ONM and are related to intracranial compliance. Elastance (E) and pressure volume index (PVI), parameters derived from LIS, also describe the compliance of the system.

A retrospective exploration of patient-ventilator monitoring intensity, therapeutic intervention intensity, and compliance with lung protective guidelines in a cohort of patients with adult respiratory distress syndrome.

BACKGROUND: The current approach to mechanical ventilation for adult respiratory distress syndrome (ARDS) and acute lung injury (ALI) involves maintaining key patient-ventilator parameters within established lung protective targets. Monitoring is part of the processes of nursing care believed to guide therapeutic intervention and facilitate compliance with these targets. Empirical relationships between monitoring, therapeutic intervention, and compliance with these practice guidelines have not been adequately explored. METHODS: A retrospective observational design was used to explore relation

Management and monitoring of haemodynamic complications in acute heart failure.

The pathophysiology of acute heart failure syndromes (AHFS), defined as a change or worsening in heart failure symptoms and signs, is complex. The variety of adverse neurohormonal adaptations includes increased levels of plasma renin, aldosterone and angiotensin II, all responsible for cardio-renal dysfunction. In fact, such alterations result in an array of clinical changes that include abnormal haemodynamics, altered ventricular filling pressures, pathological neurohormonal responses, leading to fluid overload, congestion and ultimately heart failure symptoms. Clinical pictures can be variou

Cerebral near-infrared spectroscopy during cardiopulmonary bypass predicts superior vena cava oxygen saturation.

OBJECTIVE: Cerebral and flank near-infrared spectroscopy are used to monitor tissue oxygenation during cardiopulmonary bypass in pediatric patients. We sought to validate these noninvasive measurements as predictors of oxygen saturation in the superior and inferior venae cavae during cardiopulmonary bypass. METHODS: Eight patients underwent elective repair of congenital heart defects with bicaval cannulation. Ultrasonic flow probes and oximetric catheters were placed in the superior and inferior venae cavae limbs of the perfusion circuit. Cerebral and flank near-infrared spectroscopy and 12 a

Feeding, growth, nutrition, and optimal interstage surveillance for infants with hypoplastic left heart syndrome.

Improvement in operative survival of patients with hypoplastic left heart syndrome has led to increasing emphasis on prevention of interstage mortality. Many centres have improved interstage results through programmes of home monitoring following discharge after the Norwood (Stage 1) operation. Experience with heightened interstage surveillance has identified failure to thrive during infancy as a modifiable risk factor for this population, one that has been linked to concerning outcomes at subsequent palliative surgeries. Ensuring normal growth as an infant has thus become a priority of manage

Direct vascular control results in less physiologic derangement than proximal aortic clamping in a porcine model of noncompressible extrathoracic torso hemorrhage.

BACKGROUND: The optimal method of vascular control and resuscitation in patients with life-threatening, extrathoracic torso hemorrhage remains debated. Guidelines recommend emergency department thoracotomy (EDT) with aortic clamping, although transabdominal aortic clamping followed by vascular control and direct vascular control (DVC) without aortic clamping are alternatives. The objective of this study is to compare the effectiveness of three approaches to extrathoracic torso hemorrhage in a large animal model. METHODS: Adolescent swine (Sus Scrofa) (mean weight = 80.9 kg) were randomized in

Development of a smart garment to reduce kyphosis during daily living.

Many of the aches and pains of adults are the result of the long-term effects of bad posture or body misalignment. Postural kyphosis in adolescence, which is an excessive rounding of the upper spine, may be one of the effects of poor standing and sitting habits. A smart garment, consisting of a harness and two data-sensor loggers, was developed to monitor and provide vibration feedback to wearers to improve their posture during daily activities. Laboratory tests verified that the garment could provide an accuracy of 2 ± 2° during static measurement and 3 ± 2° during stable or slowly changing p

Using amplitude-integrated EEG in neonatal intensive care.

The implementation of amplitude-integrated electroencephalography (aEEG) has enhanced the neurological monitoring of critically ill infants. Limited channel leads are applied to the patient and data are displayed in a semilogarithmic, time-compressed scale. Several classifications are currently in use to describe patient tracings, incorporating voltage criteria, pattern recognition, cyclicity, and the presence or absence of seizures. In term neonates, aEEG has been used to determine the prognosis and treatment for those affected by hypoxic-ischemic encephalopathy, seizures, meningitis and even

Prolonged recording of duodenal acid exposure in patients with functional dyspepsia and controls using a radiotelemetry pH monitoring system.

INTRODUCTION: Increased duodenal acid exposure (DAE) is associated with ulcer and dyspeptic symptoms but our understanding is limited by cumbersome recording methods. We have recently shown that prolonged radiotelemetry pH monitoring (RpHM) in the duodenum is feasible. This study compared DAE in controls and patients with functional dyspepsia (FD) using RpHM. METHODS: FD and controls underwent 48-hour recording of DAE using endoclip-secured RpHM capsules in the distal duodenal bulb. Subjects were nil per os except for meals eaten twice daily. FD completed a 14-item symptom checklist. Sleep pe

First example of hepatocyte transplantation to alleviate ornithine transcarbamylase deficiency, monitored by NMR-based metabonomics.

We demonstrate the effective use of NMR spectroscopic profiles of urine and plasma from the first successful use of hepatocyte transplantation as a bridge to auxiliary partial orthotopic liver transplantation in a child antenatally diagnosed with severe ornithine transcarbamylase deficiency. In this single-patient study, NMR profiles indicated that the disrupted urea cycle could be normalized by hepatocyte cell infusion and this was confirmed using orthogonal partial least-squares-based chemometrics. However, despite dietary manipulations and adminstration of ammonia scavengers, the desired re

Effects of spinal anesthesia on resting metabolic rate and quadriceps mechanomyography.

Previous work by our group has shown by mechanomyography (MMG) that resting muscle is mechanically active. Ten patients having spinal anesthesia for surgery, which paralyses muscle below the waist, were studied to help determine whether resting-muscle mechanical activity plays a significant role in resting metabolism, and to further determine if the phenomenon is neurally mediated. Resting metabolic rate (RMR) by indirect calorimetry, and mid-anterior thigh MMG by accelerometer, were recorded before and during spinal anesthesia. Spinal anesthesia produced a 25% decrease in oxygen uptake (mean

Simulated central apnea detection using the pressure variance.

This paper presents use of an unobtrusive pressure sensor array for simulated central apnea detection. Data was collected from seven volunteers who performed a series of regular breathing and breath holding exercises to simulate central apneas. Results of the feature extraction from the breathing signals show that breathing events may be differentiated with epoch based variance calculations. Two approaches were considered: the single sensor approach and the multisensor vote approach. The multisensor vote approach can decrease false positives and increase the value of Matthew's Correlation Coef
