Adaptive deep brain stimulation (aDBS).

Deep brain stimulation is an established technique for the treatment of movement disorders related to neurodegenerative diseases such as Parkinson's disease (PD) and essential tremor (ET). Its application seems also feasible for the treatment of neuropsychiatric disorders such as treatment resistant depression (TRD) and Tourette's syndrome (TS). In a typical deep brain stimulation system, the amount of current delivered to the patients is constant and regulated by the physician. Conversely, an adaptive deep brain stimulation system (aDBS) is a closed loop system that adjusts the stimulation pa…

Prevalence of anxiety disorders and anxiety subtypes in patients with Parkinson's disease.

Anxiety disorders are common in Parkinson's disease (PD), but are not well characterized. This study determined the prevalence and clinical correlates of all DSM-IV-TR anxiety disorder diagnoses in a sample of 127 subjects with idiopathic PD who underwent comprehensive assessments administered by a psychiatrist and neurologist. A panel of six psychiatrists with expertise in geriatric psychiatry and/or movement disorders established by consensus all psychiatric diagnoses. Current and lifetime prevalence of at least one anxiety disorder diagnosis was 43% (n = 55) and 49% (n = 63), respectively. …

Risperidone in the management of psychiatric and neurodegenerative disease in the elderly: an update.

Neuropsychiatric disorders are common among elderly patients in long-term care facilities. Although dementia is most common, schizophrenia a … ntington's disease. Risperidone was well tolerated in the elderly and associated with a low risk of movement disorders and anticholinergic effects. A recent change in the product information described an increased incidence of cerebrovascular events in some elderly and suggests the need for further investigations in this population. There is a substantial published …

Good sleep, bad sleep: a meta-analysis of polysomnographic measures in insomnia, depression, and narcolepsy.

Primary insomnia, major depression, and narcolepsy are usually considered to be separate disorders, distinguished by different polysomnographic profiles. But do polysomnographic data provide adequate evidence to segregate the three disorders, or might they display fundamentally the same sleep disturbance, differing only in degree? To test the viability of these two alternate hypotheses, the authors performed a meta-analysis of controlled polysomnographic studies of these disorders. A summary measure of degree of sleep disturbance was constructed from five variables: wakefulness after sleep ons

Is There a Relationship Between Movement and Sleep Disturbances in Essential Tremor?

BACKGROUND: Essential tremor (ET) is a clinically heterogeneous disorder involving motor and non-motor features. Voluntary movement abnormalities, including movement slowness (bradykinesia), have been frequently described in ET. Among non-motor symptoms, insomnia is also frequently reported, raising the possibility of shared underlying mechanisms with bradykinesia (e.g., brainstem noradrenergic dysfunction involving the locus coeruleus due to Lewy body pathology). We investigated the relationship between movement abnormalities, as objectively quantified using finger-tapping kinematic analysis,

Prodromal Markers of Upper Limb Deficits in FMR1 Premutation Carriers and Quantitative Outcome Measures for Future Clinical Trials in Fragile X-associated Tremor/Ataxia Syndrome.

BACKGROUND: Fragile X-associated Tremor/Ataxia Syndrome (FXTAS) is a rare, late-onset neurodegenerative disorder characterized by tremor and cerebellar gait ataxia, affecting premutation carriers (PMC) of CGG expansions (range, 55-200) in the fragile X mental retardation 1 (FMR1) gene. Discovery of early predictors for FXTAS and quantitative characterization of motor deficits are critical for identifying disease onset, monitoring disease progression, and determining efficacy of interventions. METHODS: A total of 39 PMC with FXTAS, 20 PMC without FXTAS, and 27 healthy controls performed a seri

Associations between single-nucleotide polymorphisms of human exonuclease 1 and the risk of hepatocellular carcinoma.

BACKGROUND: Single camera markerless motion capture has the potential to facilitate at home movement assessment due to the ease of setup, portability, and affordable cost of the technology. However, it is not clear what the current healthcare applications of single camera markerless motion capture are and what information is being collected that may be used to inform clinical decision making. This review aims to map the available literature to highlight potential use cases and identify the limitations of the technology for clinicians and researchers interested in the collection of movement dat

Alternating hemiplegia of childhood: insights into its pathophysiology.

Alternating hemiplegia of childhood is a rare disorder characterized by episodic paroxysmal attacks of neurologic dysfunction and has historically been thought to represent a migraine equivalent, an unusual form of epilepsy, or a movement disorder. Medical treatment with a variety of anticonvulsants and migraine agents is largely unsuccessful, and progressive mental deterioration is universally expected. Despite extensive laboratory studies, the pathophysiologic basis of this condition has thus far eluded identification. Recently, an international workshop was held to generate testable hypothe

MR imaging of Hallervorden-Spatz disease.

Hallervorden-Spatz disease (HSD) is a movement disorder with neuropathology including abnormal iron storage, disordered myelination, and loss of brain substance. Cranial magnetic resonance (MR) imaging of two patients with HSD provided good quality imaging of these patients' atrophic brain stems and cerebella, regions partially degraded by beam-hardening artifact on X-ray CT. Magnetic resonance also demonstrated signal aberrations in the lentiform nuclei and surrounding white matter consistent with iron storage or disordered myelination. However, quantification of relaxation times of these les

[Tourette syndrome--in the borderland between neurology and psychiatry].

Tourette syndrome is a hereditary tic disorder. The symptoms consist of compulsory movements and vocalizations. Stress has an aggravating effect on tics. Unfortunately tics are easily mistaken for oppositional and defiant behavior. Tourette syndrome is most frequently seen with psychiatric comorbidity. Tics are best treated with low arousal techniques, which may be supplemented with pharmacotherapy.

Associations of Adherence to the 24hour Movement Guidelines with Depressive Symptoms and Anxiety: the Mediating Role of Physical Fitness.

BACKGROUND: Mental health problems such as depression and anxiety were increasingly prevalent among students. Emerging evidence links adherence to 24-hour movement guidelines-including adequate physical activity, limited screen time, and sufficient sleep-with improved mental well-being. However, the mechanisms underlying these associations remained poorly understood. OBJECTIVES: This study aimed to examine the association of adherence to 24-hour movement guidelines with symptoms of depression and anxiety among Chinese students, and to explore the mediating role of perceived fitness in these r

Mirror movements and callosal dysgenesis in a family with a DCC mutation: Neuropsychological and neuroimaging outcomes.

Corpus callosum dysgenesis is a congenital abnormality whereby the corpus callosum fails to develop normally, and has been associated with a range of neuropsychological outcomes. One specific finding in some individuals with corpus callosum dysgenesis is "congenital mirror movement disorder", which is the presence of involuntary movements on one side of the body that mimic voluntary movements of the other side. Mirror movements have also been associated with mutations in the deleted in colorectal carcinoma (DCC) gene. The current study aims to comprehensively document the neuropsychological ou

Isolated rapid eye movement sleep without atonia in amyotrophic lateral sclerosis.

OBJECTIVE: The aim of this study was to quantitatively analyze, with the most recent and advanced tools, the presence of periodic leg movements during sleep (PLMS) and/or rapid eye movement (REM) sleep without atonia (RSWA), in a group of patients with amyotrophic lateral sclerosis (ALS), and to assess their eventual correlation with the clinical severity of the disease. METHODS: Twenty-nine ALS patients were enrolled (mean age 63.6 years) along with 28 age-matched "normal" controls (mean age 63.8 years). Functional impairment due to ALS was evaluated using the ALS-Functional Rating Scale-Rev

Long-latency afferent inhibition during phasic finger movement in focal hand dystonia.

In the motor system, one specific movement is generated, and, simultaneously, other possible movements are suppressed; a process called surround inhibition. Focal hand dystonia (FHD) is a movement disorder characterized by a loss of surround inhibition. In order to explain the deficit in surround inhibition induced by volitional movement in FHD patients, we examined the inhibitory circuit activated by afferent stimulation at "long latency". We studied 14 patients (age 48.9+/-13.2 years, 3 females, 11 males) with idiopathic task-related FHD. To measure long-latency afferent inhibition (LAI), tr

Apraxia differs in corticobasal degeneration and left-parietal stroke: A case study.

Corticobasal degeneration (CBD) is a progressive disorder characterized by both cortical and basal ganglia dysfunction such as asymmetrical apraxia, and akinetic rigidity, involuntary movements, and cortical sensory loss. Although apraxia is a key finding for the differential diagnosis of CBD, it has not been determined whether the features of apraxia seen in subjects with CBD are similar to those features exhibited by subjects with left-hemisphere damage from stroke. Therefore, for both clinical purposes and in order to better understand the brain mechanisms that lead to apraxia in CBD, we st
