Novel therapeutics and targets in myelofibrosis.

Myelofibrosis is a myeloproliferative neoplasm characterized by clonal proliferation of myeloid cells, bone marrow fibrosis and cytopenias, extramedullary hematopoiesis and hepatosplenomegaly, increased pro-inflammatory cytokine production, and systemic symptoms. Patients with MF also have a propensity toward leukemic transformation. Allogeneic hematopoietic stem cell transplantation (aHCT) is the only curative therapy for patients with MF; however, transplant-related morbidity and mortality precludes this option for the majority of patients. In the last decade, two targeted therapies have bee

[DIAGNOSTIC, CLINIC AND TREATMENT OF DISEASES OF SUBMANDIBULAR SALIVARY GLAND].

The purpose of the research was to study the structure of diseases of the submandibular salivary gland, to improve the diagnosis and effectiveness of the surgical treatment of patients with diseases of the submandibular salivary glands. A survey and treatment of 1094 patients with diseases of the submandibular salivary glands was performed. It was revealed that the most common disease of the submandibular salivary gland is calculous submaxillitis in the exacerbated and chronic stages of the inflammatory process (82.1%). Tumors and tumor-like formations of the submandibular glands account for 1

Adenoid Cystic Carcinoma of Salivary Gland: A Ten-Year Single Institute Experience.

Adenoid cystic carcinoma is a rare tumor, accounting for about 7.5% of all salivary gland neoplasms. More frequent developing in minor salivary gland, this is a slow-growing tumor with a long-lasting natural evolution, quite aggressive locally, but which has a tendency toward local recurrence and even for distant metastasis. We conducted a retrospective study limited to a period of 10 years in a single medical institution to investigate the morphoclinical profile of this tumor. Thus, we have established that about 60% of the tumors developed in men, with near 40% of the cases in patients in th

Unclassified Mixed Germ Cell-Sex Cord-Stromal Tumor of the Ovary: An Unusual Case Report.

Unclassified mixed germ cell-sex cord-stromal tumor (UMGC-SCST) is a rare ovarian neoplasm composed of germ cells and sex cord elements, which occurs in genetically and phenotypically normal women without the usual histological features seen in gonadoblastoma. Few cases have been reported in the literature so far. The age of presentation is more frequent in girls younger than 10 years of age, although it can also occur in adult women. It can be associated with isosexual pseudoprecocity. The preferred management is the resection of the gonad that contains the tumor and the conservation of the o

Hypoglossal Nerve Schwannoma: Case Report and Literature Review.

BACKGROUND: Hypoglossal schwannomas are rare, benign intracranial neoplasms; they represent 5% of all nonvestibular schwannomas. CASE DESCRIPTION: A 22-year-old male patient had presented 4 months before admission with left-sided hearing loss, an increase in the base of support with lateralization to the left, and dysphagia to solids. Physical examination on admission showed hypotrophy of the tongue and deviation to the left. Magnetic resonance imaging showed evidence of an extraaxial lesion compressing the medulla oblongata and pons, which protruded through the hypoglossal canal. The patient

Combinational therapy of crizotinib and afatinib for malignant pleural mesothelioma.

Malignant pleural mesothelioma (MPM) is a relative rare but highly aggressive neoplasm which is associated with asbestos exposure in most patients. The majority of patients are diagnosed in advanced stages so patients neither benefit from chemotherapy (e.g. pemetrexed-platinum combination) nor from surgery. It has been reported that cellular-mesenchymal to epithelial transition factor (MET) and epidermal growth factor receptor (EGFR) were critical for MPM cell proliferation. Moreover, targeting MET and EGFR drugs have gained promising results on anti-tumor therapy. Here, a striking difference

Pleuropulmonary Blastoma: A Single-center Case Series of 6 Patients.

Pleuropulmonary blastoma (PPB) is a rare malignancy of childhood which when left untreated often shows pathologic progression resulting in a more aggressive neoplasm with an increasingly poor prognosis. Because of this it is important to diagnose and initiate treatment early. However, early stage PPB can appear as a cystic lung lesion on imaging and can be easily misdiagnosed given the rarity of the malignancy. Moreover, current therapeutic guidelines for these lesions are not well established, making treatment decisions and management difficult for clinicians. DICER1 mutations are known to be

Primary esophageal melanoma: report of a case.

INTRODUCTION: Primary malignant melanoma of the esophagus is a rare tumor representing only 0.1-0.2% of esophageal malignancies. The goal of the study was to report on the management of a new case diagnosed and treated in our site. CASE REPORT: A 67-year-old patient presented with dysphagia to solids with no other remarkable history or associated skin lesions. He underwent gastroscopy, which revealed a polypoid mass suggestive of neoplasm in the distal third of the esophagus. Biopsy indicated melanoma with positive immunohistochemical markers S100 and HMB45, and negative cytokeratins and CEA.

Renal Medullary Carcinoma and Sickle Cell Trait: A Push for Early Diagnosis and Intervention Report of Two Cases.

Renal medullary carcinoma (RMC) is a rare but highly aggressive neoplasm that primarily affects young African Americans with sickle cell trait. Most patients present with macroscopic hematuria and have metastases at diagnosis. Chemotherapy, biologics directed against the more common renal cell carcinomas and radiation have all shown limited efficacy in treating patients with advanced RMC. We report two patients with RMC. Both had Stage IV disease. One underwent radical nephrectomy followed by radiation and biologic drug therapy but died five months later; the other underwent multiple cycles of

Polypoid anal melanoma. A case report and review of the literature.

INTRODUCTION: Ano-rectal melanoma is an uncommon finding in patients complaining of rectal bleeding and/or anal mass often misinterpreted as a haemorroidal pile. CASE REPORT: A 55-years-old woman, complaining of rectal bleeding, frequent anal pain and anal mass suspected for haemorroidal thrombosis was referred for evaluation and possible treatment. A brown polypoid mass arising from the anal canal/lower rectum with a maximum diameter of 6 cm was diagnosed. The hystological examination of the neoplasm, transanally removed, revealed the presence of a polypoid melanoma partially involving the r

Mutation Analysis for TP53 in Chronic-Type Adult T-Cell Leukemia/Lymphoma.

Adult T-cell leukemia/lymphoma (ATLL) is a T-cell neoplasm caused by human T-cell leukemia virus type I (HTLV-I). ATLL is classified into four clinical subtypes based on the clinical manifestation: acute, lymphoma, chronic and smoldering. Approximately half of chronic type ATLL cases progressed to the acute type. We previously demonstrated that genomic alterations related to the cell cycle de-regulation such as CDKN2A and immune escape such as CD58 alteration can serve as predictive biomarkers for acute transformation of the chronic type. Although alteration of TP53, which is known to be a maj

Advanced Mesodermal (Müllerian) Adenosarcoma of the Ovary: Metastases to the Lungs, Mouth, and Brain.

Background. A malignant mixed Müllerian tumor (MMMT) is a malignant neoplasm found in the uterus, the ovaries, the fallopian tubes, and other parts of the body that contains both carcinomatous (epithelial tissue) and sarcomatous (connective tissue) components. Outcome of MMMTs is determined primarily by depth of invasion and stage. The metastatic background of these lesions is controversial and unknown. Case Report. A 75-year-old woman was admitted to the hospital with anorexia, weakness, and persistent coughing. The imaging exams revealed a solid, promiscuous lesion of 16 × 14 cm in dimension

Bilateral choroidal metastasis from non-small cell lung cancer.

Breast and lung cancers are the most common primary neoplasms to manifest with choroidal metastases. The incidence of choroidal metastases from metastatic lung cancer was reported to be 2-6.7%. We report a case of bilateral choroidal metastasis from non-small cell lung cancer. A 59-year-old Caucasian female patient, never a smoker, was diagnosed with stage IV lung adenocarcinoma metastatic to the pleura, bones, and the brain. Her initial scan of the chest showed innumerable soft tissue nodules and mediastinal adenopathy compatible with metastatic disease. Her initial brain MRI showed numerous

[Age-related changes in the rat lacrimal gland: specific morphology and unknown nature].

The rat lacrimal apparatus includes several glands; among them, the exorbital gland plays the central role. Its parenchyma and stroma undergo prominent morphologic changes with age. The parenchymal transformation includes metaplasia of some of its acini and their turning into Harderian gland-like structures (harderization), accumulation of gland ducts ("ductularization"), and morphologic dysplasia-cytomegaly, karyomegaly, and'cell and nuclearpolymorphism in the other part of acini. All these transformations are hormone-dependent andsex-specific: theyoften appear in males. On the final stages o

JAK2 inhibitors and their impact in myeloproliferative neoplasms.

The BCR-ABL-negative myeloproliferative neoplasms (MPNs) include essential thrombocythemia, polycythemia vera, and primary myelofibrosis. Historically, complex biochemical alterations defining these heterogeneously distinct malignancies have remained elusive and constrained available therapy options. The discovery of Janus kinase (JAK) mutations collectively present in BCR-ABL-negative MPNs has led to a resurgence of medical interest in JAK-STAT targeted treatment modalities, as well as provided a unique platform for inhibiting symptom-directing proinflammatory cytokines. Cutaneous T-cell-rich B-cell lymphoma in a horse.

Cutaneous malignant lymphomas are rare in horses and comprise predominantly T-cell-rich B-cell lymphomas. They are characterized by multiple tumour nodules affecting predominantly female horses with a survival rate of months to years. At the final stage, metastases to regional lymph nodes occur, whereas widespread organ involvement is rarely reported. In this case report, a cutaneous T-cell-rich B-cell lymphoma in a 7-year-old standardbred gelding with metastases is described. Clinically, multiple cutaneous and subcutaneous nodules, enlarged superficial lymph nodes, rapid weight loss, and vent

Small cell neuroendocrine cervical carcinoma with 1-year follow-up: case report and review.

BACKGROUND: Small cell neuroendocrine cervical carcinoma (SCNCC) is a rare tumor that comprises 1-3% of cervical tumors. SCNCC exhibits clinical and biological characteristics of both cervical neoplasm (such as local aggressiveness and involvement of papillomavirus) and neuroendocrine small cell cancer of any site (such as early dissemination of the disease and loss of heterozygozity at different loci) making it an original nosologic entity. There is no unanimous opinion regarding the optimal management strategy. CASE REPORT: A 48-year-old Caucasian woman, para 3, referred postmenopausal blee

Primary laryngeal T/NK-cell lymphoma, nasal-type: an unusual location for an aggressive subtype of extranodal lymphoma.

Primary haematopoietic neoplasms of the larynx constitute less than 1% of all laryngeal tumours. Most of them are extramedullary plasmocytomas, diffuse large B-cell lymphomas, or MALT-type marginal zone B-cell lymphomas. T- or NK-cell lymphomas have rarely been reported in this location. The diagnosis of laryngeal lymphomas is a challenge, due to the absence of clinical and gross differential criteria. A pathological study is required in all cases. We present hereby a primary laryngeal T/NK-cell lymphoma, nasal-type. The patient, of Mexican origin, presented with involvement of the epiglottis,

Role of the primary care physician in Hodgkin lymphoma.

Approximately 8,200 new cases of Hodgkin lymphoma are diagnosed annually in the United States. Common presenting features include painless lymphadenopathy (usually above the diaphragm), cough, fever, night sweats, and weight loss. To decrease late complications, treatment has gradually evolved toward shorter-duration chemotherapy with use of lower-dose, shorter-duration radiation therapy. ABVD (a chemotherapy regimen consisting of doxorubicin, bleomycin, vinblastine, and dacarbazine) is now more commonly used than MOPP (a regimen consisting of mechlorethamine, vincristine, procarbazine, and pr

Minireview: PRKAR1A: normal and abnormal functions.

The type 1alpha regulatory subunit (RIalpha) of cAMP-dependent protein kinase (PKA) (coded by the PRKAR1A gene) is the main component of type I PKA, which regulates most of the serine-threonine kinase activity catalyzed by the PKA holoenzyme in response to cAMP. Carney complex (CNC), or the complex of spotty skin pigmentation, myxomas, and endocrine overactivity, is a multiple endocrine (and not only) neoplasia syndrome that is due to PRKAR1A-inactivating mutations. The R1alpha protein and PRKAR1A mRNA have been found to be up-regulated in a series of cell lines and human and rodent neoplasms,
