[Monteggia injuries in childhood].

The Monteggia-fracture dislocation is a characteristic combined injury of the forearm with fracture of the ulna and dislocation of the head of the radius. Monteggia- and equivalent injuries are rare. In the present study, we attempt to clarify the concept of therapy and observe the influence of this injury on the growth of the forearm bones. Between 1977 and 1996, 27 patients (three to thirteen years) with a Monteggia or a Monteggia-equivalent injury were treated. We present clinical and radiological long-term results of 20 patients, two to 21 years following the injury. Group I includes 12 pa

Bilateral lower limb ischaemia following acetabular reconstruction and arthroplasty.

The incidence of acetabular fractures has increased markedly in patients over 60 in the last quarter of a century, with open reduction and internal fixation being regarded to be the treatment of choice in most patients with displaced acetabular fractures. Lower limb ischaemia following acetabular fixation or arthroplasty is rare. However, such complications can be limb or life threatening, and therefore, it is important to recognise them as early as possible. We present the case of a 70-year-old man with no significant medical history who underwent acetabular fixation with simultaneous arthrop

Early Tibiotalocalcaneal Arthrodesis Intramedullary Nail for Treatment of a Complex Tibial Pilon Fracture (AO/OTA 43-C).

UNLABELLED: Management of severely comminuted, complete articular tibial pilon fractures (AO/OTA 43-C) remains a challenge, with few treatment options providing good clinical outcomes. Open reduction and internal fixation of the tibial plafond, tibiotalar arthrodesis, and salvage hindfoot reconstruction procedures are all associated with surgical complications and functional limitations. In this report, we present a case of a complex pilon fracture in a patient with multiple medical comorbidities and socioeconomic disadvantages that was successfully and acutely treated with a retrograde tibiot

Simultaneous bilateral tibial tubercle avulsion fracture in an adolescent: a case report.

Bilateral simultaneous avulsion fractures of the anterior tibial tubercle (ATT) are extremely rare. Since the first description in 1954, 15 similar cases have been reported. We report a further case in a 16-year-old boy who sustained simultaneous bilateral tibial tubercle avulsion fractures (Watson-Jones Type III) from jumping during a gymnastics session. The right knee presented an associated partial avulsion of the patellar tendon. Both knees were treated successfully by open reduction and internal fixation with two cannulated screws. The recovery of the patient was complete; the screws were

Infection in diabetic patients with ankle fractures.

Between January 1992 to June 1993, 93 ankle fractures underwent surgical treatment, of which, 10 patients were diabetic and 83 were non-diabetic. Infection occurred in 5 patients and all belonged to the diabetic group. The average follow-up period of the infected cases was 16.2 months. Wound infection occurred in 4 patients treated with open reduction and internal fixation. An infected pressure sore developed in the other patient treated initially in a below-knee cast for which arthrodesis of the ankle was performed. Infection was resistant to treatment in 2 patients and they ended up with bel

Irish rugby injury survey: Dungannon Football Club (1986-87).

The injuries sustained during one season by players at an Ulster Senior Rugby Club were documented with reference to time of injury, phase of play, team position, and nature of injury. The overall injury pattern in Irish rugby as found in this study is broadly comparable with that in similar studies in England and Scotland. Eighty-four players were injured in total. Three of these had fractures of which one required hospital admission for open reduction of a fractured wrist. The remaining eighty-one players had various types and grades of soft-tissue damage. The tackle accounted for approximat

Long-term prognosis of tarsal dislocations.

Eighty tarsal dislocations treated between 1954 and 1979 are presented. Our of 48 cases reviewed after a mean follow-up time of 7 years, satisfactory results were achieved in 30 (62%). Osteonecrosis of the talus was not recorded whereas necrosis of the navicular developed in 5 patients. Failures are to be related to significant osteoarthritic changes which occurred after a long period particularly in compound injuries with associated fractures of the tarsal bones. Early closed reduction and immobilization in a below-knee cast for 4 weeks is recommended in pure luxation; associated fractures of

Treatment of Perilunate Injuries With Radial-sided Fixation.

BACKGROUND: Perilunate dislocations and fracture-dislocations are high-energy wrist injuries. Standard treatment involves urgent closed reduction followed by open reduction and fixation of both radial (scapholunate) and ulnar (lunotriquetral) intervals. In our experience, fixation of the radial-sided injury alone (scaphoid fracture or scapholunate ligament tear) restores carpal alignment without separate lunotriquetral stabilization. We present the largest series to date using this approach. METHODS: Between 2004 and 2025, 36 patients with acute perilunate injuries were treated by a single su

A technical note on percutaneous fixation of trapezium fracture under gravitational traction.

Trapezium fractures rarely present as isolated fractures and warrant anatomical fixation to minimize post-traumatic arthritis. Fixation techniques reported include open approach to trapezium fracture and fixation, percutaneous fixation under image intensifier guidance or percutaneous arthroscopic fixation under gravitational traction. We have reported a novel method of percutaneous fixation of a trapezium Walker type IV coronal split fractures under gravitational traction and per-operative image intensifier guidance. Two percutaneous headless differential pitch screws were used for fixation, w

Results of treatment of displaced supracondylar humeral fractures in children by K-wiring.

The supracondylar fracture of the humerus in children remains the most challenging injury for the orthopedic surgeon. It is important to consider the options of treatment very carefully and tailor the treatment according to the characteristics of each fracture. In this study we observed outcomes of surgical procedures using the Kirschner-wire for the dislocated (displaced) supracondylar fractures in 135 children (mean age 6.7 years). In 96 patients closed reposition (reduction) and fixation with crossed K-wire was done. A total of 41 children were operated by the means of open reposition and c

The use of freeze-dried cancelous allograft in the management of impacted tibial plateau fractures.

The purpose of this study is to assess the behaviour of freeze-dried cancellous allograft used for supporting the reconstructed articular surface in impacted tibial plateau fractures. Freeze-dried cancellous allograft was used as metaphyseal defect filling agent in twenty-three patients (fifteen men and eight women, mean age 53 years), who suffered impacted tibial plateau fractures. According to Schatzker classification, ten fractures were classified as type II, six as type III, three as type V and four as type VI. All fractures were treated with open reduction-internal fixation after restorat

Treatment of nonunion of humeral shaft fracture with dynamic compression plate and cancellous bone graft.

BACKGROUND: This study was conducted to evaluate the treatment of aseptic nonunion of the humeral shaft with a dynamic compression plate (DCP) and cancellous bone graft. METHODS: One hundred and five cases of nonunion of a humeral shaft fracture between 1982 and 2001 were analyzed retrospectively. The study population comprised 66 males and 39 females with an average age of 46.2 years (range, 17-81 years). Sixty-seven fractures were defined as atrophic nonunion, and 20 as hypertrophic nonunion, whereas 18 could not be defined clearly. All the fractures were managed by open reduction and inter

[Simple bone cysts of the calcaneus. Differential diagnosis and therapy].

Fifty-two calcaneal simple bone cysts from our clinic were evaluated. The lesions had a pathognomonic radiologic appearance and diagnosis was histologically confirmed in all operatively treated cases. Four cases presented with pathological fractures, three of which were treated by open reduction internal fixation and bone grafting, while one was treated nonoperatively. In addition, six patients with large cysts without apparent fracture but spontaneous pain were treated by curettage and subsequent autogenous bone grafting or calcium phosphate cement filling, and there were no recurrences. Fracture of the intercondylar eminence of the tibia: a review of 35 patients.

Thirty-five patients (28 children and seven adults) were reviewed six months to 22 years after sustaining the common childhood fracture of the intercondylar eminence of the tibia. The aim was to assess both short-term and long-term results and prognosis by clinical and radiological examination and to discover whether conservative treatment was adequate for those severe fractures where a fragment of the tibial crest had been totally displaced. All the less severe fractures and 14 of the 17 severe fractures were treated conservatively. Early improvement occurred in more patients when the knee wa

Surgical treatment of three and four-part proximal humeral fractures.

BACKGROUND: Optimal surgical management of three and four-part proximal humeral fractures in osteoporotic patients is controversial, with many advocating prosthetic replacement of the humeral head. Fixed-angle locked plates that maintain angular stability under load have been proposed as an alternative to hemiarthroplasty for the treatment of some osteoporotic fracture types. METHODS: The records of 122 consecutive patients who were fifty-five years of age or older and in whom a Neer three or four-part proximal humeral fracture had been treated surgically between January 2002 and November 200

Two-stage strategy for neglected Monteggia fracture in children: A retrospective study of 51 patients.

A neglected Monteggia fracture is defined as the fracture of the proximal ulna associated with radial head dislocation (RHD) without undergoing any treatment for 4 weeks or more after injury. One-stage operation of ulnar corrective osteotomy and open reduction of RHD might result in many complications. Therefore, a two-stage strategy, including ulnar osteotomy (UO) with or without annular ligament reconstruction (ALR), was adopted at our institute since 2010.We performed a retrospective review of 51 patients with neglected Monteggia fracture between January 2010 and January 2018. Patients with

Comparison of supraintercondylar and supracondylar femur fractures treated with condylar buttress plates.

BACKGROUND: Treatment of supraintercondylar (AO/OTA 33-C) and supracondylar (AO/OTA 33-A) femur fractures is generally challenging. Standard treatments include open reduction and internal fixation. However, optimal implants are now being well-defined. This study focus on the comparison between clinical and functional outcomes of fractures treated with condylar buttress plates (CBPs). METHODS: We treated 87 patients with supraintercondylar or supracondylar femur fracture from 2004 to 2008, including 30 supraintercondylar and 24 supracondylar fractures treated with CBPs. [Superelastic constructions in surgical treatment of mandible condylar process fractures with full medial head of mandible dislocation].

Between 2007 and 2009 15 patients with medially dislocated head fractures of condylar process were surgically treated in Sechenov Moscow Medical Academy. All patients were surgically treated with open reduction--mandibular rami osteotomy, replantation of condylar head and osteosynthesis with nikelide titanium gages. Modern surgical technologies implementing nikelide titanium gages for osteosynthesis simplifies operation technique, decreased surgical trauma and increased overall treatment quality of such patients.
