Craniocervical intradural neurenteric cysts.

Neurenteric cysts are rare lesions of the central nervous system that are lined by epithelium of intestinal nature. They result from abnormal separation of germ cell layers in the 3rd week of embryonic life, leading to persistence of entodermal elements in the spinal canal. The common location is cervical and ventral to the spinal cord. The lesions have been recognized as being entodermal in origin by such markers as vertebral anomalies, gut cysts, bowel reduplication and the presence of keratin markers. Three unusual cases of childhood craniocervical intradural neurenteric cysts in patients a

[Personalized surgical management of otogenic cerebrospinal fluid leak].

INTRODUCTION: Otogenic cerebrospinal fluid leak is a condition that requires surgery due to the risk of developing potentially life-threatening meningitis. The surgical approach should consider the location of the defect, the status of the middle ear cavity, the hearing threshold, and the etiology. OBJECTIVE: To summarize our experience with personalized closure techniques for leak. METHOD: Retrospective analysis of data from patients who underwent closure in our department between April 2022 and June 2024, regarding surgical success and hearing outcomes. RESULTS: During the aforementioned

The different routes of parallel evolution in epiarenic growth in a hyperarid desert environment.

The Atacama Desert, one of the driest and oldest regions on Earth, represents an extreme environment that has driven remarkable adaptive evolution over millions of years. Within this setting, the genus Tillandsia comprises specialists capable of surviving at the dry limit of plant life. These species exhibit crassulacean acid metabolism (CAM), lack functional roots, and possess trichomes adapted for water and nutrient absorption. Of the more c. 750 known Tillandsia species, nine occur in the Chilean-Peruvian Atacama Desert, where they colonize bare sand surfaces (epiarenic growth) under hypera

A Complex Case of Hemophagocytic Lymphohistiocytosis Secondary to Untreated Hepatitis C: Diagnostic Challenges.

Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyperinflammatory syndrome caused by uncontrolled activation of cytotoxic T cells and macrophages, leading to cytokine storm and organ failure. Secondary HLH is the acquired form that is commonly triggered by underlying malignancies, autoimmune diseases, or infections. While Epstein-Barr virus (EBV) is a common infectious trigger for HLH, hepatitis C virus (HCV) is rarely reported in the literature. Herein, we present the case of a patient with HLH with multiple possible infectious triggers that was ultimately attributed to HCV. A

Comparison of Sino-Nasal Outcome Test 22 Symptom Scores in Rhinogenic and Odontogenic Sinusitis.

BACKGROUND: Odontogenic maxillary sinusitis (OMS) and rhinogenic sinusitis (RS) are the main types of chronic rhinosinusitis (CRS) and have a significant impact on health-related quality of life (HRQL), but the difference in HRQL and symptom presentation between them has not been specifically evaluated to date. Obejctive: Our aim was to compare patterns of symptoms and HRQL disease-specific domains in patients affected with these 2 types of CRS. METHODS: A group of 201 patients with CRS (99 with rhinogenic and 102 with odontogenic origin) completed the Sino-Nasal Outcome Test 22 (SNOT-22) que

Secondary Infections in Febrile Neutropenia in Hematological Malignancies: More Than Another Febrile Neutropenic Episode.

INTRODUCTION: Febrile neutropenic episodes (FNEs) are among major causes of mortality in patients with hematological malignancies. Secondary infections develop either during the empirical antibiotic therapy or one week after cessation of therapy for a FNE. The aim of the study was to investigate the risk factors associated with secondary infections in febrile neutropenic patients. METHODS: We retrospectively analyzed 750 FNEs in 473 patients between January 2000 and December 2006. RESULTS: Secondary infections were diagnosed in 152 (20%) of 750 FNEs. The median time to develop secondary infe

Percutaneous repair or surgery for mitral regurgitation.

BACKGROUND: Mitral-valve repair can be accomplished with an investigational procedure that involves the percutaneous implantation of a clip that grasps and approximates the edges of the mitral leaflets at the origin of the regurgitant jet. METHODS: We randomly assigned 279 patients with moderately severe or severe (grade 3+ or 4+) mitral regurgitation in a 2:1 ratio to undergo either percutaneous repair or conventional surgery for repair or replacement of the mitral valve. The primary composite end point for efficacy was freedom from death, from surgery for mitral-valve dysfunction, and from

Trends of oral and pharyngeal cancer in Israel, by gender, age, ethnic group, and country of origin: 1970-2006.

BACKGROUND: Oral and pharyngeal cancer is one of the few life-threatening oral diseases. Israel is recognized as a developed country, characterized by a heterogeneous population, a high level of sun exposure, and a large proportion of faired skinned inhabitants. The purpose of this study was to describe trends in incidence and survival rates of oral and pharyngeal cancer between 1970 and 2006. METHODS: The incidence and survival data were derived from the Israel National Cancer Registry between 1970 and 2006. Oral and pharyngeal cancer included cancers of the lips, tongue, oral mucosa, floor

[Hydrops fetalis--a retrospective study].

In a retrospective study, the authors examined the occurrence of hydrops in foetal and neonatal cases in a five-year period. During this time, the clinical and pathologic diagnosis of hydrops was established in 28 cases. In three cases, the hydrops was caused by Rh incompatibility, and in 25 cases non-immune hydrops was discovered by clinical and pathologic examination. The cause of hydrops was recognised in 25 cases and no underlying disease was discernible in three. In 12 cases, the pregnancy was spontaneously terminated. Artificial abortion was performed in four cases. In this series, four

Delineating a putative phobic-anxious temperament in 126 panic-agoraphobic patients: toward a rapprochement of European and US views.

BACKGROUND: The current US official position, since DSM-III, is that panic attacks represent the hallmark of panic disorder and play a major role in the development of the agoraphobic syndrome. The more favoured view in the European tradition is that neurotic personality and/or prodromal features such as mild depression and excessive worries precede the illness. METHOD: We studied 126 consecutive cases of panic disorder with or without agoraphobia by DSM-III-R criteria, evaluated by relevant structured and semi-structured interviews. RESULTS: We provide evidence that characterological and pr

Rationale and results of in utero transplants of stem cells in humans.

Following 18 years experience in postnatal fetal liver transplantation (FLT), we have developed a new therapeutical method, namely the in utero transplantation of stem cells from the human fetal liver. This early transplant takes advantage of the immunological tolerance that exists in young fetal recipients. The four fetuses that we treated were 28, 26, 17 and 12 weeks of age (weeks after fecundation). The first two patients had immunodeficiencies, the two other had thalassemia major. Donor cells were obtained from 7- to 12-week-old fetuses, with conditions approved by the National Committee f

Cardiac Syncope: An Underestimated Cause of Unexplained Syncope in the Elderly-Data from a Single High-Volume Syncope Unit.

Background/Objectives: Syncope remains a common problem in the elderly, adversely affecting quality of life, morbidity and mortality. Diagnosis is challenging due to the atypical presentation, multifactorial aetiology, overlap with non-syncoptic falls and increased prevalence of cardiac disease. This study aims to investigate the impact of cardiac syncope in this high-risk population. Methods: A retrospective single-centre observational cohort study, including 171 patients ≥65 years old with syncope of unknown origin or other falls, was conducted. Different diagnostic tests and strategies were

Evolution of Extraintestinal Manifestations in Patients With Ulcerative Colitis Post-Restorative Proctocolectomy and Ileal Pouch-Anal Anastomosis: A Single-Center Retrospective Study.

BACKGROUND AND AIMS: Extraintestinal manifestations (EIMs) are common in patients with ulcerative colitis (UC). However, the prevalence and associated factors of EIMs in UC patients post-restorative proctocolectomy with ileal pouch-anal anastomosis (RPC + IPAA) are not well established. METHODS: We extracted clinical, demographic, and laboratory data of all UC patients who underwent IPAA surgery and followed up in our comprehensive pouch clinic between 2003 and 2021. EIMs were classified as musculoskeletal, mucocutaneous, ophthalmologic, and hepatic, and their frequency before and after the I

Phytoestrogens as potential anti-osteoporosis nutraceuticals: Major sources and mechanism(s) of action.

By 2050, the global aging population is predicted to reach 1.5 billion, highlighting the need to enhance the quality of life of the elderly population. Osteoporotic fractures are projected to affect one in three women and one in five men over age 50. Initial treatments for osteoporosis in postmenopausal women include antiresorptive agents such as bisphosphonates, strontium ranelate, estrogen replacement therapy (ERT) and selective estrogen receptor modulators (SERMs). However, these do not rebuild bone, limiting their effectiveness. Denosumab, an FDA-approved antiresorptive monoclonal antibody

Characterization of Fabry disease-associated lyso-Gb3 on mouse colonic ion transport and motility.

Fabry disease (FD) is a rare X-linked lysosomal storage disorder caused by a deficiency in α-galactosidase A leading to the accumulation of globotriaosylceramide (Gb3) and subsequent increase in globotriaosylsphingosine (lyso-Gb3) in different cells and organs, including the gastrointestinal (GI) tract. GI symptoms represent some of the earliest manifestations of FD and significantly impact quality of life. The origin of these symptoms is complex, and the exact mechanisms remain poorly understood. Here, we sought to determine whether lyso-Gb3 contributes to the pathophysiology of GI symptoms a

Understanding the migration experience of unaccompanied youth: A review of the literature.

Migration of unaccompanied immigrant and refugee youth has increased around the world in the last decade, peaking in 2014 in the United States and in 2015 in Europe. Youth are pushed to migrate without parents or guardians to escape war, persecution, gang violence, and extreme poverty in their country of origin. Youth are vulnerable to violence and abuse at the hands of smugglers during their journeys but show great courage and strength as they embark on their journey to a better life. The goal of this literature review is to provide a synthesis of empirical literature concerning this populati
