Expanding access to BRCA1/2 genetic counseling with telephone delivery: a cluster randomized trial.

BACKGROUND: The growing demand for cancer genetic services underscores the need to consider approaches that enhance access and efficiency of genetic counseling. Telephone delivery of cancer genetic services may improve access to these services for individuals experiencing geographic (rural areas) and structural (travel time, transportation, childcare) barriers to access. METHODS: This cluster-randomized clinical trial used population-based sampling of women at risk for BRCA1/2 mutations to compare telephone and in-person counseling for: 1) equivalency of testing uptake and 2) noninferiority o

Exploring the Major Barriers to Physical Activity in Persons With Multiple Sclerosis: Observational Longitudinal Study.

BACKGROUND: Physical activity (PA) represents a low-cost and readily available means of mitigating multiple sclerosis (MS) symptoms and alleviating the disease course. Nevertheless, persons with MS engage in lower levels of PA than the general population. OBJECTIVE: This study aims to enhance the understanding of the barriers to PA engagement in persons with MS and to evaluate the applicability of the Barriers to Health Promoting Activities for Disabled Persons (BHADP) scale for assessing barriers to PA in persons with MS, by comparing the BHADP score with self-reported outcomes of fatigue, d

Optimal hand surgery fellowship interview format.

CONTEXT: The ideal format for residency and fellowship interviews has been consistently debated. Secondary to the COVID-19 pandemic, many institutions, including all hand surgery fellowship programs, transitioned interviews to an all-virtual format. In the past year, with ease of travel restrictions, some programs have transitioned back to in-person interviews, while others remain solely virtual. Hand surgery fellowship programs are continually assessing what are the best means for conducting these interviews, with little perspective regarding the applicant's preferences. OBJECTIVES: The purp

Comparing large language models and human experts in interpreting MRI reports for personalized patient education.

RATIONALE AND OBJECTIVES: Knee osteoarthritis (OA) is a prevalent global condition. While MRI guides clinical decisions, its technical complexity hinders patient understanding and engagement. Translating these findings into comprehensible, personalized patient education remains challenging. Large language models (LLMs) show promise in automating this process. To evaluate and compare the effectiveness of advanced large language models against experienced clinicians in generating comprehensible, personalized patient education materials derived from knee MRI reports. Associations between respiratory function, balance, postural control, and fatigue in persons with multiple sclerosis: an observational study.

INTRODUCTION: Fatigue, postural control impairments, and reduced respiratory capacities are common symptoms in persons diagnosed with Multiple Sclerosis (MS). However, there is a paucity of evidence establishing correlations among these factors. The aim of this study is to analyze respiratory function in persons with MS compared to the control group as well as to analyze the relationship between fatigue, respiratory function and postural control in persons with MS. MATERIALS AND METHODS: A total of 17 persons with MS and 17 healthy individuals were enrolled for this cross-sectional study. The

Normative data for PID-5 domains, facets, and personality disorder composites from a representative sample and comparison to community and clinical samples.

The introduction of the alternative model of personality disorders in the fifth edition of the Diagnostic and Statistical Model of Mental Disorders (American Psychiatric Association, 2013) represented a substantive change in how personality disorders (PDs) are diagnosed. One barrier to its adoption (among several) in clinical practice, however, is a lack of information as to what constitutes an elevated score on the 25 domains and facets that comprise Criterion B. Unique sets of facets can be configured to assess any 1 of 6 PDs retained in the alternative model of personality disorders; each o

An agenda for establishing the DSM-5 alternative model of personality disorder (AMPD) trait model in adolescence: Comment on Clark and Watson (2022).

In the years since the publication of the Diagnostic and Statistical Manual for Mental Disorders, Fifth Edition (DSM-5; American Psychiatric Association, 2013), considerable progress has been made in understanding the structure of the traits included in the Alternative Model of Personality Disorders (AMPD) in adulthood. In the next phase of research, the structure of the pathological trait model needs to be established more firmly in adolescence as well. There is promising evidence from studies of the Personality Inventory for DSM-5 (PID-5; Krueger et al., 2012) that the structure of pathologi

Survey of 24-h personal formaldehyde exposures in geographically distributed urban office workers in the USA.

An air sampling study was conducted to evaluate personal formaldehyde exposures in a group of office workers spread across five geographical locations in the USA. Passive badge samples for formaldehyde were collected on three participants in each location, as well as in the office and home indoor microenvironments of each participant over 3 individual days. Median personal 24-h formaldehyde concentrations ranged from 9.9 to 18 μg/m3. Personal 24-h formaldehyde concentrations in one location were significantly higher than concentrations measured in the other four locations; no significant diffe

Long-Term Supportive Housing is Associated with Decreased Risk for New HIV Diagnoses Among a Large Cohort of Homeless Persons in New York City.

It is unknown whether providing housing to persons experiencing homelessness decreases HIV risk. Housing, including access to preventive services and counseling, might provide a period of transition for persons with HIV risk factors. We assessed whether the new HIV diagnosis rate was associated with duration of supportive housing. We linked data from a cohort of 21,689 persons without a previous HIV diagnosis who applied to a supportive housing program in New York City (NYC) during 2007-2013 to the NYC HIV surveillance registry. We used time-dependent Cox modeling to compare new HIV diagnoses

The mediator effect of personality traits on the relationship between childhood abuse and depressive symptoms in schizophrenia.

Previous studies indicated that personality traits have a mediator effect on the relationship between childhood abuse and depressive symptoms in major depressive disorder and nonclinical general adult subjects. In the present study, we aimed to test the hypothesis that personality traits mediate the relationship between childhood abuse and depressive symptoms in schizophrenia. We used the following questionnaires to evaluate 255 outpatients with schizophrenia: the Child Abuse and Trauma Scale, temperament and character inventory, and Patients Health Questionnire-9. A Type A and Type D Combined Personality Typology in Essential Hypertension and Acute Coronary Syndrome Patients: Associations with Demographic, Psychological, Clinical, and Lifestyle Indicators.

Many studies have focused on Type A and Type D personality types in the context of cardiovascular diseases (CVDs), but nothing is known about how these personality types combine to create new profiles. The present study aimed to develop a typology of Type A and Type D personality in two groups of patients affected by and at risk for coronary disease. The study involved 711 patients: 51.6% with acute coronary syndrome, 48.4% with essential hypertension (mean age = 56.4 years; SD = 9.7 years; 70.7% men). Cluster analysis was applied. External variables, such as socio-demographic, psychological,

Suicide in autism spectrum disorders.

This review focuses on suicide in patients with Autism Spectrum Disorders (ASD) as well as risk factors and comorbidities of persons with ASD who have attempted suicide. Research was conducted by searching PubMed and Psychinfo for articles. Suicide in ASD is largely understudied. Although suicide is common in clinical samples, we have little knowledge of suicide in persons with ASD in the general population. Comorbidity, particularly with depression and other affective disorders or schizoid disorders and psychotic symptoms, is often reported, so it is difficult to determine if suicidality is a

[Gait impairment in the oldest old].

To evaluate senile gait patterns in octogenarians and nonagenarians, we provided a standardized questionnaire on gait disabilities to 153 elderly subjects over 88 years of age. Subjects represented a relatively healthy subgroup of non-institutionalized residents who participated in a gerontological survey of all inhabitants of the city of Leiden who were 85 years or older. Of the 142 subjects who responded to this questionnaire, 87 persons (61%) claimed distinct diseases as a cause of gait impairment. Of the remaining 55 persons, 42 received a standardized gait assessment. Gait was classified

Prevalence and severity of periodontal conditions among adults in urban and rural Morogoro, Tanzania.

A study to assess the prevalence and severity of periodontal conditions among 809 adults in Tanzania was carried out in 1982 in Morogoro District. The prevalence of plaque (99.6%), calculus (95.7%) and gingivitis (93.8%) among them was high. The mean number of teeth per person with pockets between 3.5 and 5.5 mm was 3.5 for the urban and 4.2 for the rural population. Pockets of more than 5.5 mm were found on average in 0.1 (urban) and 0.2 (rural) teeth per person. In the 45+ age group on average 3.0 teeth per person were missing in urban areas and 6.9 teeth in rural areas. There were no statis

Personality Traits and Psychological Well-Being: Association Within the "Seguimiento Universidad de Navarra" (SUN) Cohort.

Psychological well-being (PWB) is a core component of mental health influenced by personality; however, most prior research focused on subjective well-being and normal-range personality traits. Less is known about the longitudinal role of maladaptive personality traits in shaping eudaimonic well-being. This study examined prospective associations between maladaptive personality domains and subsequent PWB in the SUN cohort. Personality traits were assessed at the 16-year follow-up using the Personality Inventory for DSM-5 Abbreviated Form (PID-5-BF). PWB was evaluated 2 years later using Ryff's

Connectome-Guided Personalization of Optimal TDCS Intervention Selection in Alzheimer's Disease: A Modeling Study.

Transcranial direct current stimulation (tDCS) could reduce the neurophysiological effects in Alzheimer's disease (AD), but progress is hampered by variable outcomes across studies, likely related to both methodological and individual differences. We recently described a virtual brain network simulation method for optimizing tDCS interventions and now propose a method for further personalizing this approach. We now personalized the model for six female and four male biomarker-confirmed AD patients based on their brain structure and functional connectivity by using individual structural magneti

Criteria A and B of the alternative model for personality disorders as prospective predictors of psychosocial functioning in community-based adolescents: A 1-year follow-up study.

Research on the dimensional approach to personality disorders in relation to psychosocial functioning is limited, particularly among adolescents. Since adolescence is a critical developmental period from the emergence of personality disorders, it is crucial to understand how both Criterion A (the level of personality functioning) and Criterion B (maladaptive personality traits) contribute to various domains of psychosocial functioning in this age group. To address this research gap, the current study evaluated the extent to which Criterion A and Criterion B of the alternative model for persona

HIV self-testing knowledge, attitudes, and acceptance in New York City.

BackgroundHIV testing is a critical strategy for prevention. During the COVID-19 pandemic, many community-based organizations (CBOs) offering free HIV testing were unable to provide in-person services. Innovations like free HIV Self-Testing (HIVST), provided a solution and can be done outside clinical settings, help address barriers such as cost, availability, and stigma. However, HIVST uptake remains low in resource-rich settings like the US, and best practices for integrating it into existing HIV services are still undetermined.MethodsGay Men's Health Crisis (GMHC) partnered with the NYC Hea
