How are automatic positive airway pressure and related devices prescribed by sleep physicians? A web-based survey.

STUDY OBJECTIVES: The main purpose of this study was to characterize the practices of sleep physicians prescribing positive airway pressure (PAP) therapy to patients with sleep-disordered breathing. The main outcome measures were to determine the indications for initiation of PAP therapy, the type of PAP devices prescribed, and the interfaces that were commonly used and to assess the physicians' familiarity with the newer PAP devices. DESIGN AND SETTING: An anonymous web-based survey with 20 questions aimed at determining point-prevalence estimates of physician practices and physician knowled

Systematic interindividual differences in neurobehavioral impairment from sleep loss: evidence of trait-like differential vulnerability.

OBJECTIVES: To investigate interindividual differences in neurobehavioral deficits during sleep deprivation, and to establish to what extent the neurobehavioral responses to sleep loss are a function of sleep history versus trait-like differential vulnerability. DESIGN: Individuals were exposed to sleep deprivation on 3 separate occasions in order to determine the stability of interindividual differences in neurobehavioral impairment. SETTING: The sleep-deprivation experiments were conducted under standardized laboratory conditions with continuous monitoring of wakefulness. Each subject unde

Determinants of altered quality of life in patients with sleep-related breathing disorders.

Recent reports have suggested that altered quality of life and well-being are reported by patients with sleep-related breathing disorders (SRBD). There seems to be no data available in the literature on factors underlying these behavioural consequences. In this study, health-related quality of life (HRQL) scores were examined in SRBD patients in order to establish which factors are implicated in these disturbances. The study group consisted of 130 patients: 49 snorers and 81 patients with obstructive sleep apnoea. The Medical Outcome Survey Short Form-36 questionnaire was administered the morn

Pelvic movements as rhythmic motor manifestation associated with restless legs syndrome.

Video-polysomnographic monitoring of a patient with a 4-year history of an unpleasant restless sensation originating in his lower abdomen showed stereotyped, repetitive, rhythmic pelvic body movements resembling coital behaviour at the wake-sleep transition. We discuss the association between restless legs syndrome and rhythmic movement disorder as a particular manifestation of a spectrum of rhythmic sleep-related movement disorders.

L-DOPA-induced excessive daytime sleepiness in PD: a placebo-controlled case with MSLT assessment.

A 74-year-old patient with idiopathic Parkinson's disease was evaluated for unintended sleep episodes that occurred after long-term treatment with 400 mg/day of L-dopa. Overnight sleep studies and multiple sleep latency testing were carried out under double-blind administration of either L-dopa or placebo. Mean sleep latency with L-dopa was 7 minutes, in contrast to a normal value of 19 minutes, 25 seconds with placebo. The authors' results suggest that L-dopa may cause daytime somnolence in some patients with Parkinson's disease.

Sleep but not hyperventilation increases the sensitivity of the EEG in patients with temporal lobe epilepsy.

PURPOSE: To evaluate the relative impact of 3 and 5 min of hyperventilation (HV) and different sleep stages on the sensitivity of the interictal EEG in focal epilepsy. METHODS: We examined 20 patients with temporal lobe epilepsy (TLE, 85%) or extratemporal epilepsy during EEG-monitoring. We compared 6 min EEG (12 epochs of 30s) during/after each: (a) waking; (b) 5 or 3 min of HV; (c) sleep stages 1, 2, 3/4 and REM regarding the frequency of epileptiform discharges (ED). The Wilcoxon matched pairs signed rank test was used. The main endpoint was the comparison of 5 min of HV with sleep stage 2

Apnoea characteristics across the night in severe obstructive sleep apnoea: influence of body posture.

Several studies have reported changes in apnoea characteristics across the night in patients with obstructive sleep apnoea (OSA). However, the effect of body posture on these changes has not been evaluated. The purpose of this study was to assess how body posture influences sleep apnoea characteristics across the night in severe OSA. Polysomnograms of 30 patients with severe OSA who had > or = 30 apnoeic episodes in the lateral position and 30 in the supine position during Stage 2 sleep, in the early, middle and late sleep periods were analysed. For each of the 30 events in each body position,

Prader-Willi syndrome with sleep disordered breathing: effect of two years nocturnal CPAP.

We report a case of a 3-year-old child with Prader-Willi syndrome who had severe sleep disordered breathing with daytime hypersomnolence. His daytime blood gases showed type II respiratory failure. He was effectively treated with continuous positive airway pressure (CPAP) and has used this form of therapy for 2 years now with improvement in daytime somnolence, improved mental acuity and normalisation of daytime blood gases.

Melatonin profiles and sleep characteristics in boys with fragile X syndrome: a preliminary study.

Sleep patterns and endogenous melatonin profiles in 13 fragile X boys between the age of 4.7 and 11.0 years were compared to those of 8 age-matched, normal control boys. Parents recorded sleep patterns on a Sleep Diary Chart for 14 consecutive days. Twelve saliva samples were obtained from 8 fragile X participants and all of the controls over 48 hours for the assessment of salivary melatonin profiles. The results showed greater variability in total sleep time and difficulty in sleep maintenance in fragile X boys compared with the control participants. Nocturnal melatonin production, expressed

The role of the expiratory phase in obstructive sleep apnoea.

The role of the expiratory phase in obstructive sleep apnoea (OSA) is not well known. The aim of our study was to verify the contribution of expiratory narrowing to apnoea in a group of OSA patients by evaluating the effects of short-term treatment with continuous positive airway pressure (CPAP), intermittent positive pressure ventilation (IPPV) and bi-level positive airway pressure (BIPAP). We studied a selected group of 10 OSA patients whose therapeutic pressure level of CPAP was at least 10 cm H2O. During CPAP therapy the mean apnoea/hypopnoea index (AHI) and oxyhaemoglobin desaturation ind

[Drivers with obstructive sleep apnea as potential authors of car accidents].

The aim of this work was to estimate excessive somnolence and frequency of episodes of dozing off during driving a car in patients with obstructive sleep apnea. Result of questionnaire and polysomnographic investigations were analysed in a group of 503 patients. Mean age for the whole group was 49.2 +/- 9.7 of years, BMI 32.1 +/- 1.4 kg/m2 and the value for apnea and hypopnea index 45.4 +/- 24.8. Excessive somnolence during driving was reported by 49.1% of patients. 31.2% of the group notified at least one episode of falling asleep while driving a car. Sleepy drivers (group A) were significant

A case of subwakefulness syndrome.

We report a patient, a 30-year-old male Japanese-Brazilian migrant construction worker, suffering from excessive daytime sleepiness for at least 6 months. Electroencephalogram recordings during his waking states showed that 10-Hz and 60-microV alpha activity was present prominently in the occipital regions. From the multiple sleep latency test, it was found that stages 1-2 NREM sleep episodes appeared repetitively without any REM episodes, and that the mean sleep latency was 10.2 min. These findings support the diagnosis that this patient suffers from subwakefulness syndrome.

Arousal and ventilatory responses during sleep in children with obstructive sleep apnea.

Abnormal central regulation of upper airway muscles may contribute to the pathophysiology of the childhood obstructive sleep apnea syndrome (OSAS). We hypothesized that this was secondary to global abnormalities of ventilatory control during sleep. We therefore compared the response to chemical stimuli during sleep between prepubertal children with OSAS and controls. Patients with OSAS aroused at a higher PCO2 (58 +/- 2 vs. 60 +/- 5 Torr, P < 0.05); those with the highest apnea index had the highest arousal threshold (r = 0.52, P < 0.05). The hypercapnic arousal threshold decreased after treat
