Surgical site infection in upper extremity fracture: Incidence and prognostic risk factors.

Upper extremity fractures (UEF) occurred in about 0.67% of males and females at some point in their lifetime. Surgical treatment has the advantage of good functional recovery, however, the occurrence of surgical site infection (SSI) affects the clinical outcome of operation. Currently, there are few studies focused on SSI of UEF. Consecutive patients with UEF and underwent surgeries from January 2010 to February 2021 were recruited. Demographic data, surgical related variables and laboratory test index of these patients were extracted and collected from the electronic medical records and pictu

Influencing Factors and Application of Spontaneous Imbibition of Fracturing Fluids in Tight Sandstone Gas Reservoir.

This work is based on high-precision fluid spontaneous imbibition experiments to quantitatively study the imbibition rate, imbibition capacity, and imbibition curve characteristics of fracturing fluids in tight sandstone reservoirs. The objective of the work is to explore the influence of tight sandstone physical characteristics, fracturing fluid composition, salinity, viscosity, surface tension on fracturing fluid imbibition, and further analyze its main controlling factors. To evaluate imbibition characteristics more deeply, the pore throat structure and micromorphology of tight sandstone be

Towards valid 'serious non-fatal injury' indicators for international comparisons based on probability of admission estimates.

BACKGROUND: Governments wish to compare their performance in preventing serious injury. International comparisons based on hospital inpatient records are typically contaminated by variations in health services utilisation. To reduce these effects, a serious injury case definition has been proposed based on diagnoses with a high probability of inpatient admission (PrA). The aim of this paper was to identify diagnoses with estimated high PrA for selected developed countries. METHODS: The study population was injured persons of all ages who attended emergency department (ED) for their injury in

Static Compressive Behavior and Failure Mechanism of Tantalum Scaffolds with Optimized Periodic Lattice Fabricated by Laser-Based Additive Manufacturing.

Porous tantalum (Ta) scaffolds have been extensively used in the clinic for reconstructing bone tissues owing to their outstanding corrosion resistance, biocompatibility, osteointegration, osteoconductivity, and mechanical properties. Additive manufacturing (AM) has an advantage in fabricating patient-specific and anatomical-shape-matching bone implants with controllable and well-designed porous architectures through tissue engineering. The sharp angles of strut joints in porous structures can cause stress concentration, reducing mechanical properties of the structures. In this study, porous T

Spectrum of microarchitectural bone disease in inborn errors of metabolism: a cross-sectional, observational study.

BACKGROUND: Patients diagnosed with inborn errors of metabolism (IBEM) often present with compromised bone health leading to low bone density, bone pain, fractures, and short stature. Dual-energy X-ray absorptiometry (DXA) is the current gold standard for clinical assessment of bone in the general population and has been adopted for monitoring bone density in IBEM patients. However, IBEM patients are at greater risk for scoliosis, short stature and often have orthopedic hardware at standard DXA scan sites, limiting its use in these patients. Furthermore, DXA is limited to measuring areal bone

The impact of anatomic alignment on radiocapitellar pressure following radial head arthroplasty.

BACKGROUND: The incidence of radial head fractures is increasing, and radial head arthroplasty (RHA) is being more frequently used as treatment for irreparable fractures. Our objective was to compare radiocapitellar pressure between the native joint and 2 radial head prosthesis conditions: (1) a prosthetic head that was aligned to the forearm axis of rotation and (2) the same prosthesis with an axisymmetric nonaligned head. METHODS: Ten cadaveric specimens received a pressfit radial head prosthesis (Align; Skeletal Dynamics) for both prosthetic testing conditions. Anatomic alignment (AL) was

Comparison of intraoperative flat panel imaging and postoperative plain radiography for the detection of intraarticular screw displacement in volar distal radius plate ostheosynthesis.

OBJECTIVES: To investigate if intraoperative 3D flat panel imaging improves the detection of radiocarpal intraarticular screw misplacement (RCSM) in comparison to standard postoperative x-ray. METHODS: In a study on cadaver specimens, we evaluated the sensitivity and specificity to detect RCSM using X-ray, intraoperative 3D-fluoroscopy as well as the digital volume tomography. The gold standard reference was computed tomography. RESULTS: Sensitivity for the detection of RCSM for X-ray was 58% and specificity 88%. For DVT, the sensitivity to detect RCSM was 88% and the specificity 53%. For 3D

Distal radius osteotomy in the elderly patient using angular stable implants and Norian bone cement.

PURPOSE: To report our results after testing the combination of two technologies--angular-stable locking screw implants and Norian SRS cement--in corrective osteotomies of the distal radius in the elderly. This technique eliminates donor site bone-graft morbidity and expands the indications of corrective osteotomies to older patients with osteoporotic bone. METHODS: Our retrospective series include 6 patients (5 women and 1 man) with an average age of 60 years. Three patients had corrections through a dorsal approach, 1 through a volar approach, and 2 through a combined approach. Two correcti

Subchondral Shelf Support Plating for Impacted Radial Head Fractures: Surgical Technique and Case Series.

Subchondral bone impaction at the level of the radial neck, usually present at the anterior or lateral quadrant, can be an associated feature in radial head fractures. Buttress plating principles suggest that subchondral zones of impaction are best supported by an implant positioned directly on the site of impaction to resist secondary collapse. The anatomical considerations limit the standard radial head plating technique to a narrow safe zone. Therefore, irrespective of the zone of impaction, the standard plating zone remains the same, which may not provide the best possible mechanical const

A Novel Algorithmic Approach to Chronic Forearm Longitudinal Instability.

Essex-Lopresti Lesions are rare injuries that are often missed in the acute setting. Delayed intervention may lead to chronic wrist and elbow pain and overall poor outcomes. The literature currently supports treatments that involve shortening of the ulna to reduce the relative degree of ulnar impaction, followed by attempted reduction of the distal radioulnar joint. Although such techniques may help to temporarily reduce wrist pain secondary to ulnar impaction, they do not address the proximal migration of the radius and ipsilateral radial head dislocation at the elbow. Subsequent procedures a

Intramedullary Nailing System in Unstable Both Bone Diaphyseal Fractures in Children.

Background Forearm fractures in pediatric population is usually managed conservatively. Unstable fractures need operative intervention like closed or open intramedullary nailing or open reduction and internal fixation with plates and screws. However, there is no consensus regarding the method of treatment according to age group. Objective To standardize the use of elastic nailing system as effective form of treatment in terms of function, cost and lower complications as compared to widely used titanium nails in developed countries.Method Sixty eight pediatric patients with both bone forearm di

Outcomes of long-arm casting versus double-sugar-tong splinting of acute pediatric distal forearm fractures.

INTRODUCTION: The traditional treatment after closed reduction of distal radius (DR) and distal both bone (DBB) forearm fractures has been application of a long-arm cast (LAC) or a short-arm cast (SAC). Splinting is another option that avoids the potential complications associated with casting. The purpose of this study is to evaluate the maintenance of reduction of DR or DBB fractures placed in a double-sugar-tong splint (DSTS) compared with a LAC in a pediatric population. METHODS: This is an IRB-approved, prospective, randomized trial. Patients aged 4 to 12 years with DR or DBB fractures t

A radiographic assessment of pediatric fracture healing and time since injury.

Past studies and pediatric bone physiology indicate that younger individuals may heal at a faster rate. Additionally, in adults upper limb fractures heal faster than lower limb fractures; this trend is expected for pediatric fractures. This study aims to evaluate and compare rates of fracture repair in children based on age and skeletal element. Six stages are used to describe the bone repair process in 294 radiographs of tibial and radial fractures from 107 infants and young children. Healing rates are examined using ANOVA and Welch's t-test with a 95% confidence interval. Results indicate th

[New principles of using biodegradable implants in the treatment of fractures].

So far the operative treatment of bone fractures has resolved itself to immobilisation and bone fragment joining with the help of metal implants such as nails, screws, plates and wires. Once the bone had healed, they had to be removed. This action bears some risk, e.g. extremity disfunction, wound infection. This requires re-hospitalization and increases social cost of the treatment. These reasons gave rise to research on new, biodegradable materials. One of them is "Biofix" manufactures as threads or nails, employed when immobilising bone fragments. Application of "Biofix" allows early rehabi
