Optimizing topical antifungal therapy for superficial cutaneous fungal infections: focus on topical naftifine for cutaneous dermatophytosis.

Superficial cutaneous fungal infections (SCFIs) are commonly encountered in clinical practice in the United States, and comprise infections of the skin by dermatophytes and yeasts. The most common organisms causing SCFI are dermatophytes, especially Trichophyton spp. With the exception of onchomycosis and tinea capitis, most cases of SCFIs are amenable to properly selected topical antifungal therapy used over an adequate period of time.

A variety of topical antifungal agents are available for the treatment of SCFIs, and they encompass a few major chemical classes: the polyenes (ie, nys

Small N designs for rehabilitation research.

Rehabilitation research presents unique and challenging problems to investigators during both the design and analysis periods. Statistical issues regarding sample size requirements for an adequately powered study may be in direct conflict with realistic recruitment and subject retention goals. Issues of underpowered studies, sample size requirements, and recruitment goals plague rehabilitation research. Randomized clinical trials (RCTs) are typically narrow in scope and thus lack generalizability to everyday, yet specific, clinical problems; they are also costly and time-consuming and require

Antibiotics for trachoma.

BACKGROUND: Trachoma is the world's leading infectious cause of blindness. In 1997 the World Health Organization (WHO) launched an Alliance for the Global Elimination of Trachoma by the year 2020, based on the 'SAFE' strategy (surgery, antibiotics, facial cleanliness and environmental improvement). OBJECTIVES: To assess the evidence supporting the antibiotic arm of the SAFE strategy by assessing the effects of antibiotics on both active trachoma (primary objective) and on Chlamydia trachomatis (C. trachomatis) infection of the conjunctiva (secondary objective). SEARCH STRATEGY: We searched C

[Pharmacotherapy of patients with chronic heart failure. Analysis of multicenter international studies].

Leterature data and own investigations are summarized in the article. New experimental facts about mechanism of action of cardiac glycosides (especially digoxin) were obtained last year. It was shown that in human organism there are endogenous cardiosteroids (digitalis-like substances, endogenous oubain and other). They are in different organs (adrenal glands, hypothalamus, pituitary gland) and have influence on different organs and systems. Indications, contraindications, side effects, interactions with other drugs have been studied by cardiologists. Results of international multicentere stud

Pharmacological management of Huntington's disease: an evidence-based review.

INTRODUCTION: Despite the increasing body of published reports on pharmacological interventions in Huntington's disease (HD), an evidence based review (EBR) of treatment studies has not yet been published. METHOD: Systematic literature searches were done using Medline (1965-August 2005), the central database in the Cochrane Library (1969-August 2005), and reference lists published in review articles and other clinical reports. Randomized controlled trials (RCTs) were classified as level-I-studies in this paper. Level-II evidence was assigned to non-randomized, controlled clinical studies. Lev

The value of clopidogrel versus aspirin in reducing atherothrombotic events: the CAPRIE study.

Atherothrombotic disease is a growing health problem, and is increasingly more costly to manage. Clopidogrel is an advanced, specific adenosine diphosphate receptor antagonist, which has been shown to be a highly potent antiplatelet agent. Data from the Clopidogrel versus Aspirin in Patients at Risk of Ischaemic Events (CAPRIE) study have demonstrated the significantly superior clinical benefit of clopidogrel over aspirin for secondary prevention of atherothrombotic disease, with a relative risk reduction in myocardial infarction, stroke or vascular death of 8.7% (95% confidence interval 0.3,

[Surgical decompression in space-occupying cerebral infarct; notification of a randomized trial].

Patients with a hemispheric infarct and massive space-occupying brain oedema have a poor prognosis. Despite intensive conservative treatment, the case fatality rate may be as high as 80%, and most survivors are left severely disabled. Non-randomised studies suggest that decompressive surgery substantially reduces mortality and improves the functional outcome of survivors. The 'Hemicraniectomy after middle cerebral artery infarction with life-threatening edema trial' (HAMLET) is a newly-conceived randomised multi-centre clinical trial that compares the efficacy of decompressive surgery to impro

The Mirena levonorgestrel system.

The levonorgestrel intrauterine system is a safe, efficacious, long-term contraceptive device. Additionally, it results in a decrease in the volume of menstrual blood loss in women with normal periods and those with menorrhagia. It may also have a role in the treatment of other benign gynecological disorders, such as adenomyosis or endometriosis, and it has been advocated as a means of delivering progestogen to the endometrium as part of combined hormone replacement therapy. Despite the beneficial effect on menstrual volume, compliance is sometimes limited by breakthrough bleeding during the f

Is it safe to cardiovert atrial fibrillation without anticoagulation if the transoesophageal echocardiogram is normal?

The risk of thromboembolism after unprotected DC cardioversion for chronic atrial fibrillation is 5%. The use of transoesophageal echocardiographic guidance probably reduces this risk to around 1.3%. However, after conventional courses of anticoagulation, the risk is only 0.3 to 0.8%. Until there is further information from large randomised trials, the routine use of transoesophageal echocardiography cannot be recommended. However it should be considered in acute onset atrial fibrillation of less than 2 days duration in the presence of underlying structural heart disease or in individual patie

Fluconazole therapy for histoplasmosis. The National Institute of Allergy and Infectious Diseases Mycoses Study Group.

We assessed the efficacy of oral fluconazole (200-800 mg daily) in the treatment of non-life-threatening acute pulmonary histoplasmosis, chronic pulmonary histoplasmosis, or disseminated histoplasmosis in patients without human immunodeficiency virus infection. Of 27 evaluable patients, two had progressive acute pulmonary histoplasmosis, 11 had chronic pulmonary histoplasmosis, and 14 had disseminated histoplasmosis. Median durations of treatment in each of the three groups were 6 months, 7 months, and 11 months, respectively. Nineteen patients were treated with 400 mg of fluconazole daily (tw

Testing the efficacy of antisecretory drugs in patients with haematemesis and melaena.

Despite the logical expectation of effectiveness, and wide use, of antisecretory drugs in the initial management of patients with hematemesis and melaena, convincing evidence of benefit is lacking.

Lifestyle interventions as adjuvant treatments for depression: An umbrella review of systematic reviews and meta-analyses.

BACKGROUND: Depression is a high-prevalent mental health disorder that significantly affects quality of life and remains a leading global contributor to disability. Although pharmacological and psychological interventions are effective, many patients fail to achieve complete remission of depressive symptoms. Current evidence suggests that lifestyle-based interventions may offer a promising strategy as an adjuvant treatment for depression. OBJECTIVE: To evaluate the effectiveness of lifestyle-based interventions as adjuvant treatments for reducing depressive symptoms in adults. METHODS: We co

Efficacy of dance movement therapy for weight management of overweight or obese adult patients: protocol for a systematic review and trial sequential meta-analysis.

BACKGROUND: Obesity has become a global health crisis and is in urgent need of effective, sustainable interventions. Although conventional approaches such as dietary modification and aerobic exercise demonstrate efficacy, long-term maintenance of weight loss is still challenging. Dance movement therapy (DMT) is emerging as a promising intervention, combining physical activity with psychological and social engagement to potentially enhance compliance and holistic health outcomes. However, existing studies on DMT for weight management are limited, with inconsistent results. METHODS AND ANALYSIS

Efficacy and safety of auricular therapy in the treatment of post-stroke constipation: A protocol for systematic review and meta-analysis.

BACKGROUND: Constipation is one of the common gastrointestinal complications after stroke. It not only aggravates the condition of stroke, but also brings huge medical burden to patients, and has a negative impact on the quality of life of patients. Auricular therapy, as a part of Chinese traditional acupuncture and moxibustion, has been found to be effective in the clinical treatment of constipation. However, no systematic review has investigated the efficacy and safety of auricular therapy in the treatment of post-stroke constipation. Therefore, the aim of this systematic review is to assess

Exploring the characteristics, methods and reporting of systematic reviews with meta-analyses of time-to-event outcomes: a meta-epidemiological study.

BACKGROUND: Time-to-event analysis is associated with methodological complexities. Previous research identified flaws in the reporting of time-to-event analyses in randomized trial publications. These hardships impose challenges for meta-analyses of time-to-event outcomes based on aggregate data. We examined the characteristics, reporting and methods of systematic reviews including such analyses. METHODS: Through a systematic search (02/2017-08/2020), we identified 50 Cochrane Reviews with ≥ 1 meta-analysis based on the hazard ratio (HR) and a corresponding random sample (n = 50) from core cl
