Swallowing-induced atrial tachycardia is a rare phenomenon generally assumed to be caused by abnormal parasympathetic reflexes arising in the esophagus or pharynx. We describe a patient with intractable swallow tachycardia. Since certain features of the case suggested a mechanical rather than a reflex mechanism, he was treated by intrapleural repositioning of the esophagus to effect physical separation of esophagus and left atrium. Ten months after an uncomplicated procedure, the patient remains asymptomatic and free of arrhythmia.

Neurological evaluation of a family living in rural Louisiana showed that six living members have splasticity, hyperactive stretch reflexes, and pathological reflexes. Ten deceased family members had been similarly affected. All affected persons were offspring of consanguineous marriages, and sex distribution was equal, indicating that inheritance was autosomal recessive. Although undetected clinically, conduction abnormalities suggestive of demyelination in the visual pathways were revealed by special tests.

BACKGROUND: The American Society of Hematology (ASH) recommends prophylactic limited red blood cell (RBC) antigen matching for C, E, and K for patients with sickle cell disease (SCD). At our institution, reflexive extended antigen matching is employed: no antigen matching is undertaken initially; in the event of RBC alloimmunization, extended antigen matching (C/c, E/e, K, Fya/Fyb, Jka/Jkb, S/s) is performed prospectively.
STUDY DESIGN AND METHODS: We compared alloimmunization rates and costs of reflexive extended antigen matching with other RBC antigen-matching strategies. A 5-year retrospec…

Activation of efferent renal sympathetic nerve activity (ERSNA) increases afferent renal nerve activity (ARNA), which then reflexively decreases ERSNA via activation of the renorenal reflexes to maintain low ERSNA. The ERSNA-ARNA interaction is mediated by norepinephrine (NE) that increases and decreases ARNA by activation of renal α(1)-and α(2)-adrenoceptors (AR), respectively. The ERSNA-induced increases in ARNA are suppressed during a low-sodium (2,470 ± 770% s) and enhanced during a high-sodium diet (5,670 ± 1,260% s). We examined the role of α(2)-AR in modulating the responsiveness of ren…

OBJECTIVE: To assess the effects of premedication with buprenorphine on the characteristics of anaesthesia induced with ketamine/medetomidine.
STUDY DESIGN: Prospective crossover laboratory study.
ANIMALS: Six female New Zealand White rabbits.
METHODS: Rabbits received, on occasions separated by 7 days, either buprenorphine (0.03 mg kg(-1)) or saline subcutaneously (SC) as premedication, followed 1 hour later by SC ketamine (15 mg kg(-1)) and medetomidine (0.25 mg kg(-1)) (K/M). At pre-determined time points reflex responses and cardiopulmonary parameters were recorded and arterial blood sa…

On-farm euthanasia of poultry is a necessity for minimizing disease spread and removing sick or injured birds to maintain optimum animal wel … methods on 8 and 12-week-old turkey hens. Each method was assessed for impact on loss of brain stem reflexes, euthanasia success, and torn skin. The cervical dislocation techniques were also analyzed via radiograph for proper dislocation. Furthermore, each device was assessed for physical parameters. Turkeys (n = 1,400) were euthanized on 20 sampling day …

BACKGROUND AND PURPOSE: Deep sedation is associated with acute brain dysfunction and increased mortality. We had previously shown that early-assessed brainstem reflexes may predict outcome in deeply sedated patients. The primary objective was to determine whether patterns of brainstem reflexes might predict mortality in deeply sedated patients. The secondary objective was to generate a score predicting mortality in these patients.
METHODS: Observational prospective multicenter cohort study of 148 non-brain injured deeply sedated patients, defined by a Richmond Assessment sedation Scale (RASS)…

Recent events bring the importance of respiratory health to the forefront of our collective attention. In this issue of Cell, a new study by Prescott and Umans et al. reveals how a dedicated laryngeal sensory motor reflex circuit protects our airways from aspirated foods or liquids.

Click-evoked vestibulocollic reflexes were studied in a patient with a unilateral Tullio phenomenon (sound induced vestibular symptoms) and the findings were compared with those of a group of normal subjects. Compared with normal subjects, the reflexes elicited from her symptomatic side were large and had an abnormally low threshold, but retained a normal waveform. The click-evoked responses in this patient show changes consistent with her symptomatology and are indicative of a pathological increase in the normal vestibular sensitivity to sound.

Autonomic function assessed by tests of cardiovascular reflexes was studied in 25 patients with systemic sclerosis and 10 patients with primary Raynaud's phenomenon. A comparison was made with 13 normal healthy subjects. Significant abnormalities in the cardiovascular reflexes were found in systemic sclerosis, both in the CREST (calcinosis, Raynaud's phenomenon, oesophageal dysmotility, sclerodactyly, telangiectasia) variant and also in those patients with diffuse involvement. There was sympathetic and parasympathetic dysfunction. These findings suggest that autonomic neuropathy is a feature o…

This is an electromyographic study of the activity in flexor and extensor muscles of the big toe in 22 patients with a Babinski sign and 49 controls, after mechanical or electrical stimulation of the sole. The results indicate: (1) the Babinski sign is mediated by the extensor hallucis longus (EHL), and not by the extensor hallucis brevis; (2) electrical stimuli may fail to activate the EHL in these patients, and conversely may evoke EHL reflexes in control subjects; (3) in skin reflexes, electrical and mechanical stimuli are not freely interchangeable.

Cerebellar ataxia with neuropathy and vestibular areflexia syndrome (CANVAS) is characterized by neuropathological changes such as loss of Purkinje cells and degeneration of the posterior column of the spinal cord. In the peripheral nervous system, CANVAS is associated with loss of ganglion cells in the dorsal root and vestibular ganglia. Some patients may show degeneration of the inferior olivary nucleus, corticospinal tracts, and the facial and trigeminal ganglia. Further research is warranted to determine whether differences in lesion distribution are attributable to differences in genetic …

Spasticity is usually observed along with paralysis, hyperreflexia, Babinski sign and abnormal associated movements associated with dysfunction of central motor tracts. In spasticity, exaggeration of the stretch reflex results in increased resistance during passive movements. Therefore, spasticity is pathophysiologically described as increased muscle tone whose pathognomonic sign is decreased passivity. Resistance is more strongly felt during rapid passive movements than during slow movements. The resistance felt at the beginning of the passive movement abruptly diminishes, which is well known…

This is the first description of the "startle reaction" observed in young children and of the technique of its stimulation. On the basis of the study of 38 children with the given symptom and of 30 control children aged up to 6 months the independence and pathologic nature of the startle reaction have been proved. 12-month follow-up of both groups has allowed the authors to draw up the conclusion about the prognostic significance of the symptom for the development of epileptic phenomena in children. A principle of preventive therapy for the prophylaxis of convulsive manifestations and reducing

In patients with discogenic lumbar radiculitis with a unilateral affection of L5 or S1 root motor and central responses of the medial group of plantar muscles were recorded. In S1 root pathology the central response consisted in 25% of the cases in repeated antidromal discharges, which testified to an increased excitability of the corresponding alpha-motoneurons. In patients with L5-radiculitis a predominance of the reflex component in the response was typical. Since this phenomenon was lacking in combined L5 and S1 root lesions, it was interpreted as a sign of a decreasing function of the seg…
