BACKGROUND: Rhinosinusitis is a well-recognised clinical syndrome affecting patients of all ages and gender. FESS has now become a well-established strategy, comprising several techniques, for the treatment of chronic rhinosinusitis refractory to medical treatment.

Rhinosinusitis is a common medical problem. Complications involving the orbits or brain are not frequently met. Concomitant involvement of both orbits and the brain is very rare. We here describe a 6-year-old girl who suffered from acute rhinosinusitis and initially, subperiosteal abscesses in both orbits. Bilateral functional endoscopic sinus surgery and drainage of the subperiosteal abscesses through the upper eyelids were done. Pus cultures showed methicillin-resistant Staphylococcus aureus. Aspirin (acetylsalicylic acid) and other NSAIDs are responsible for many adverse effects. Among them, pseudo-allergic reactions (urticaria/angioedema, asthma, anaphylaxis) affect up to 9% of the population and up to 30% of asthmatic patients. Esterase D phenotypes have been determined in 162 patients with psoriasis vulgaris, 181 patients with atopic diseases and 110 healthy controls. All these samples are from the area of Rheinland-Pfalz (West Germany). The two patient samples show somewhat higher frequencies of Es D 1 and lower of Es D 2-1 phenotypes as compared to controls. These differences are, however, statistically not significant. The Es D1 frequency in controls (0.9045) is in accordance with those reported from other European populations.

Diagnosis of allergic rhinoconjunctivitis, particularly in chronic forms, remains sometimes difficult. Systemic tests such as skin or blood tests may be not sufficient to assess local sensitization. Local tests are needed, performed from nasal secretion or tears. Measurement of total IgE in the local secretions of 41 patients suffering from rhinoconjunctivitis show that diagnosis contribution was upgraded from 61 to 88%. Very interestingly, in some cases, local tests were the only argument for allergy.
