Synovial sarcoma of the esophagus.

A 14-year-old girl presented with dysphagia and weight loss. Barium swallow, computed tomography, and endoscopy showed a polypoid intraluminal mass in the cervical esophagus. The tumor was locally resected and postoperative chemotherapy and radiotherapy were administered. Histological examination of the surgical specimen established a diagnosis of synovial sarcoma. The patient remains free of discernible disease 30 months after surgery. Esophageal sarcomas are rare tumors. This appears to be the second reported case of esophageal synovial sarcoma in a child.

A direct intervention program for chronic neurogenic dysphagia secondary to brainstem stroke.

Little objective documentation is available regarding the efficacy of therapies for oropharyngeal dysphagia. Information specifying efficacy of treatment for chronic dysphagic conditions is almost nonexistent. This report describes a direct therapy program for chronic neurogenic dysphagia resulting from brainstem stroke, and provides information on immediate and long-term clinical outcome. Changes in swallowing physiology reflect goals of therapy. Long-term follow-up shows that functional benefits are long lasting without related health complications.

Mid-esophageal traction diverticulum: rare cause of massive upper gastrointestinal bleeding.

Traction diverticula of the mid-esophagus are usually incidental findings on barium swallow or upper endoscopy. They are thought to arise secondary to adjacent inflammation in the mediastinum, usually from granulomatous infection such as histoplasmosis. They are usually asymptomatic. Rarely, erosion or extension of the inflammatory process into the adjacent lung or bronchial arteries can result in clinical symptoms such as pneumonia or gastrointestinal bleeding. Diagnosis is often delayed due to the rarity of clinical symptoms. We present a case of massive upper gastrointestinal bleeding due t

[Recurrent syncope during deglutition. An uncommon form of sinusal dysfunction].

The authors report the case of a 69 year old man with a 16 year history of syncope occurring only while swallowing liquids. Two episodes were observed during a hospital admission to the intensive care unit for unstable angina and allowed documentation of prolonged sinus arrest (7 sec) causing syncope. In the light of this case and a review of the literature, the physiopathological role of deglutition in the genesis of cardiac conduction defects and arrhythmias is discussed and the new classification of sinus node dysfunction proposed by Bashour in 1985 is recalled.

Megalocornea, macrocephaly, mental and motor retardation (MMMM).

Two patients with macrocephaly, mild mental retardation and megalocornea are reported. Hypotonia, poor coordination and swallowing difficulties were present. One patient was obese and the other had scoliosis. Both had large fleshy ears and long fingers. The spectrum of the mental retardation megalocornea syndrome is not fully defined. These two patients resemble a previously reported case, and although there are distinct differences from patients with familial or sporadic Neuhauser syndrome, these cases may represent clinical variability of that syndrome.

Myofunctional and dentofacial relationships in second grade children.

One hundred and thirty-three second graders in rural public school were assessed on a number of dental, skeletal, and oral muscle function measures. Correlational analyses were conducted in order to determine whether specific myofunctional variables were associated with dentofacial development. Significant relationships were observed between open mouth posture and a narrow maxillary arch and long facial height. Labial and lingual rest and swallow patterns were also related to poor coordination of lip and tongue movements.

Hypoglossia congenita.

Hypoglossia and aglossia are rare congenital malformations, especially when found as isolated abnormalities. In view of their usual association with other anomalies of the face, oral cavity, and distal extremities, an accurate investigation is required. We describe a 2-year-old girl with isolated hypoglossia and severe dental disease. Clinical understanding of the changes in the mechanisms of oral suction, mastication, swallowing, and speech, as well as the existing dental occlusion, requires a multidisciplinary team approach so that a more effective treatment can be administered.

[Deglutition-induced supraventricular tachycardias].

We describe the cases of two patients who exhibited a supraventricular tachycardia induced by swallowing. In both cases we found a systemic hypertension, other cardiac or gastroesophagic abnormalities were absent. The electrophysiologic studies demonstrated in the first case a supraventricular tachycardia with 1 : 1 AV-conduction and an origin in the right atrium, in the second case an atrial tachycardia with AV-block originating in the right atrium too. The pathogenetic mechanism is discussed. In both cases after treatment with amiodarone a complete cessation of the tachycardias was reached.

[Tubal manometry: the question of normal values, values in chronic mesotympanic purulent middle ear infections].

We investigated tubal function by equalization manometry in 68 patients with unilateral traumatic perforation of the tympanic membrane, and in 116 patients with chronic mesotympanic suppuration (85 dry and 31 discharging ears). Our criterion of normal tubal function is the ability to equalize a difference in pressure of +/- 30 hPa by means of at most 7 swallowings. This result was achieved in only 25% of the dry perforations and in none of the patients with discharging ears. Normal tubal function may only be expected in 25% of patients with chronic otitis media.

Surgical aspects of international drug smuggling.

The internal concealment of cocaine and other drugs in packets by "body packers"--those who swallow packets of drugs or hide them in their vagina or rectum--to avoid detection by customs officials has been increasing in both the United States and Europe. The types of package and how they are concealed are changing as the traffickers become more sophisticated in their methods. The latest parcels are less likely to burst, but obstruction of the bowel may occur. Awareness of the problem is important for staff of emergency medical services near international ports of arrival.

Infantile colic.

Infantile colic must be differentiated from other causes of recurrent irritability in infants. Several causes of colic have been described, including family stress, aerophagia (air swallowing), and the ingestion of iron supplements and cow's milk. Recent studies suggest that the elimination of cow's milk, and in some cases soy milk, from infants' diets when they are bottle fed or the elimination of cow's milk from mothers' diets if infants are breast fed decreases colic. Medications are of doubtful value in the treatment of this entity.

[Aspiration accidents in dental practice].

Lack of symptoms does not necessarily mean that a small foreign body has been swallowed. Symptoms may not be present after aspiration of a foreign body which lodges in the bronchial tract. In any case, the event must be clarified roentgenologically. The patient should be removed from the practice. Ominous symptoms, on the other hand, indicate that a large foreign body has lodged in the trachea or behind the larynx. The patient should be immediately escorded to the hospital in a sitting position. The administration of a sedative is indicated in some cases.

The occurrence of salmonellae in free-flying-avifauna: isolation and antibiogram.

The occurrence of salmonellae in a variety of free-flying birds was investigated. Of 790 intestinal-content-samples examined, 20 yielded different Salmonella serotypes, which included 10 strains of S. saint-paul, 4 of S. bareilly, 3 of S. weltevreden, 2 of S. typhimurium and 1 of Salmonella E1 group. Common Mynah, house-sparrow, swallow, grey-partridge, parrot and crow were found positive for the presence of salmonellae. Antibiogram of the isolates was studied against 14 common chemotherapeutic agents.

Jejunoaortic fistula from ingested seamstress needle.

A 27-year-old man was evaluated for sudden onset of massive hematemesis and hematochezia. A bleeding site was not seen on esophagogastroduodenoscopy. There was little blood in the stomach, which suggested that the bleeding site was below the ligament of Treitz. Angiography demonstrated the presence of an intra-aortic metallic foreign body that resembled a sewing needle. At operation, a chronic jejunoaortic fistula that contained the sewing needle was found and repaired. The patient had no recollection of having swallowed the needle, and it is presumed that he ingested it in infancy. He had an

[Foundations of the neurophysiology of the masticatory organ].

The most important research findings in the field of neuro-physiology of the masticatory system are described, e.g. the clinico-neurological aspects of the swallowing reflex, cortical and subcortical control systems interfering with mastication and especially the two masticatory reflexes taking place at the level of the brain stem, the jaw-opening reflex and the jaw-jerk reflex. A short neuro-anatomic and electrophysiological description of the nucl. tractus mesencephalici, the nucl. supratrigeminalis and the dental pressosensors completes this survey. It concludes with a discussion of possibl

Hernia through the foramen of Winslow.

The authors present a patient who has ascarid-containing jejunum herniated through the foramen of Winslow and incarcerated. Some remarkable anatomic abnormalities were noticed upon operation. These consisted of a short ascending colon, an unusually large foramen of Winslow, and smallness of the greater omentum. Before reduction, resection of the herniated and incarcerated segment of jejunum were performed. Barium enema and swallow are valuable diagnostic acids, as well as the plain film of the abdomen, but we prefer gastrographin study. With it the dangers of perforation and peritonitis are av

Comparative Anatomy of the Hyoid Apparatus in Various Dog Breeds: Insights From Clinical Imaging.

BACKGROUND: Studying the hyoid bone in dogs is of significant importance in veterinary surgery and anatomy, as it aids in understanding how variations in this structure may affect tongue mobility, swallowing and vocalisation across breeds. OBJECTIVES: This study aims to provide preliminary insights into the relationship between structural differences in the hyoid bone and breed-specific functional adaptations in tongue shape and movement by analysing shape variations within the hyoid apparatus. METHODS: Hyoid bones from computed tomography images of 26 dogs were modelled, and principal compo

Association among oral health, social participation, and higher-level functional capacity in Japanese older people.

BACKGROUND: Maintaining a high level of functional ability, which is essential for independent living in older adults, is influenced by social participation. Oral health has been associated with higher-level functional ability; however, whether social participation is related to this association remains unclear. This study aimed to investigate the association among oral health, higher-level functional ability, and social participation. METHODS: This cross-sectional study included 154 participants aged ≥65 years (mean age: 82.6 ± 5.5 years) in an underpopulated area. Higher-level functional ab

Exploring Factors Related to Dysphagia Among Community-Dwelling Older Adults: A Scoping Review.

Dysphagia, or difficulty swallowing, is common but often underdiagnosed among community-dwelling older adults, particularly those without neurological, neuromuscular, or structural disorders. This scoping review aimed to map the existing evidence on factors related to dysphagia in this population. The review followed Arksey and O'Malley's methodological framework and the PRISMA-ScR guidelines. Embase, Ovid-MEDLINE, and CINAHL were searched up to June 2025, with search strategies developed in consultation with a professional librarian. Eligibility criteria were predefined by the research team b
