An unusual case of rapidly progressive contractures: Case report and brief review.

An 8-year-old boy, diagnosed as cervical dystonia, was referred to our tertiary center. After a trivial trauma he had developed painful lumps in the axial region, which was followed by restricted movements of neck, shoulder, and abdominal muscles over 4 months. He had kyphoscoliosis, torticollis, rigid abdomen, and multiple muscle contractures. He also had short great toes. A detailed skeletal survey showed calcification in the soft tissues surrounding the shoulder anterior chest wall, thorax, and paraspinal muscles; there was also beaking of vertebrae, which was confirmed by CT thorax. This r

Predictors of responses to psychotherapy referral of WTC utility disaster workers.

This study examined male utility disaster workers' responses to referral for trauma-specific psychotherapy. Among 328 workers offered referral for symptoms related to the World Trade Center (WTC) attacks during psychological screening, approximately 48% chose to accept, 28% chose to consider only, and 24% chose to decline. Analyses examined predisposing factors, i.e., age, race/ethnicity, marital status, education, previous mental health treatment, and previous disorder; as well as illness level; i.e., posttraumatic stress disorder (PTSD), depression, and general psychiatric distress; current

Psychometric properties of the life events checklist.

The Life Events Checklist (LEC), a measure of exposure to potentially traumatic events, was developed at the National Center for Posttraumatic Stress Disorder (PTSD) concurrently with the Clinician Administered PTSD Scale (CAPS) to facilitate the diagnosis of PTSD. Although the CAPS is recognized as the gold standard in PTSD symptom assessment, the psychometric soundness of the LEC has never been formally evaluated. The studies reported here describe the performance of the LEC in two samples: college undergraduates and combat veterans. The LEC exhibited adequate temporal stability, good conver

An active duty woman with anal incontinence after vaginal delivery: an elusive diagnosis.

Anal sphincter injury as a result of birth trauma is the leading cause of fecal incontinence in women presenting to surgical clinics. A 36-year-old active duty woman was referred to the Urogynecology Clinic at Madigan Army Medical Center for evaluation of a deficient perineal body. She reluctantly reported a 12-year history of anal incontinence since the birth of her first child, predating her entry into the naval service. Examination revealed defects of the perineum and anal sphincter resembling a cloacal deformity. Uncomplicated overlapping anal sphincteroplasty and perineorrhaphy were perfo

Neonatal surgical emergencies: stabilization and management.

The transitional period following birth can be complicated by the presence of congenital anomalies requiring emergent surgical management. Newborns with congenital diaphragmatic hernia require immediate intubation and gastric decompression to minimize gastric distention, as well as cautious ventilation to avoid pneumothorax. Newborns with omphalocele and gastroschisis are at risk for bacterial contamination, as well as heat and evaporative losses from exposed viscera. These newborns benefit from the use of a protective bowel bag. Newborns with meningomyelocele require meticulous care to avoid

Delayed diagnosis of concomitant rotator cuff tear and brachial plexopathy in a patient with traumatic brain injury: a case report.

Traumatic brain injury (TBI) is often accompanied by additional trauma that can be obscured by cognitive dysfunction or multiple injuries in the same region of the body. This report describes the case of an unhelmeted motorcycle rider who collided with a telephone pole. He sustained a diffuse subarachnoid hemorrhage, bilateral subdural hematomas (right frontal and left temporal), diffuse axonal injury in the subcortical and periventricular white matter, and a left tibial fracture. After medical and surgical stabilization, he was transferred to a subacute rehabilitation facility and then to a r

Ruptured globe 10 years after radial keratotomy.

PURPOSE: To report 3 cases (3 eyes) of globe rupture following blunt trauma 10 to 13 years after radial keratotomy. METHODS: Cases of traumatic ruptured globe after radial keratotomy were reviewed from a tertiary eye care center. One eye underwent therapeutic penetrating keratoplasty, the second was treated with a bandage contact lens, and the third developed retinal detachment leading to phthisis bulbi. RESULTS: Ruptured globes occurred through the keratotomy incision during activities of daily living (1 eye), assault (1 eye), and sports (1 eye). Two eyes regained a visual acuity of 20/30 o

[The treatment of severe and complicated urethral strictures].

Stricture of the urethra is produced by a cicatrix causing narrowing of the urethral lumen subsequent to trauma or inflammation. The strictures are considered as serious whenever the meatus is involved, and total--in the event of obliteration of the urethra (H. Sachse, 1978). The complications are associated with perineal, scrotal or suprapubic fistulae (C. Devine, P. Devine 1989). This is a report on 37 men presenting severe complicated strictures of the urethra, treated over the period 1981-1995 in the Clinical Center of Urology. According to underlying cause the strictures are divided in: p

Surgical considerations in tracheal stenosis.

From 1969 to 1990, 43 patients with tracheal stenosis were treated at the University of Mississippi Medical Center. Seventy-four percent of these patients (n = 32) had intrinsic tracheal stenosis, most frequently as a complication of prolonged endotracheal trauma. A total of 41 distinctly separate stenotic segments were identified in the 32 patients. The stenoses were considered moderate or severe in 33 (80%) of the 41 cases and the length of the stenotic segment was greater than 1 cm in 23 (56%) of the cases. An overall 70% success rate was achieved following 93 surgical procedures in this gr

Locally applied hypothermia and microvascular muscle flap transfers.

The success rate for wound closure of grade III open tibia-fibula fractures with free muscle flaps is approximately 90%. Complications and loss of free flaps are due mainly to anastomotic problems, local anatomical considerations, or recipient vessel injuries, which prolong the ischemic time of the transferred free muscle tissue. We present the techniques used at the Shock Trauma Center of The Maryland Institute for Emergency Medical Services Systems (MIEMSS), which has allowed us a 100% success rate for the last 80 free muscle transfers performed. This surgical technique involves the use of l

Deaths from falls in children: how far is fatal?

The outcome of injury was determined in 317 children who were brought to a children's trauma center with a history from the caretaker that the child had fallen. Cases in which the clinicians' judgment was that an incorrect history had been given were included along with cases in which the history was not questioned. Seven deaths occurred in 100 children who fell 4 feet or less. One death occurred in 117 children who fell 10 feet to 45 feet. The 7 children who died in short falls all had other factors in their cases which suggested false histories. When children incur fatal injuries in falls of

Fractures of the radial head and lesions of the lower radius and ulna in the adult: the importance of the prosthesis in resection.

Multi-fragmentary fracture of the head of radius is a common lesion of the elbow; and resection of the head is justified either in isolation or to be completed with prosthetic implant replacement. From a study of 25 hospital records of patients treated at the Orthopedic and Traumatology Center of CRAM of Strasbourg with an average post-operative follow-up of 8 years, it appears that the habitual complications secondary to simple resection of the radial head, in particular cubitus-valgus, ascension of radial shaft and dysfunction of the inferior radio-ulnar articulation, are not observed after

Crush injuries to the lower limbs at a major UK trauma centre: a retrospective observational study.

PURPOSE: Crush injuries result from the physical compression of muscles and may lead to crush syndrome. Early fluid resuscitation and surgical intervention is key. Few studies have reported the outcomes of crush injuries in the non-disaster setting. This retrospective study aims to characterise such cases. METHODS: Patients with lower limb crush injuries were identified from an internal database. Non-crush injuries and patients under the age of 18 were excluded. Types of injuries, management, and complications were extracted. RESULTS: 27 patients were included. The right leg (n = 10) was the

Development of a field artificial intelligence triage tool: Confidence in the prediction of shock, transfusion, and definitive surgical therapy in patients with truncal gunshot wounds.

BACKGROUND: In-field triage tools for trauma patients are limited by availability of information, linear risk classification, and a lack of confidence reporting. We therefore set out to develop and test a machine learning algorithm that can overcome these limitations by accurately and confidently making predictions to support in-field triage in the first hours after traumatic injury. METHODS: Using an American College of Surgeons Trauma Quality Improvement Program-derived database of truncal and junctional gunshot wound (GSW) patients (aged 16-60 years), we trained an information-aware Dirich

Risk factors associated with symptoms of post-extubation upper airway obstruction in the emergency setting.

OBJECTIVE: Post-extubation stridor and hoarseness are important clinical manifestations that indicate laryngeal edema due to intubation. In previous studies the incidence of post-extubation stridor and hoarseness ranged from 1.5% to 26.3% in postoperative patients and patients in the intensive care unit. Female sex and prolonged intubation are reportedly risk factors for post-extubation stridor. However, the risk factors for post-extubation stridor and the appropriate endotracheal tube size in emergency settings remain unknown. This study was performed to identify the risk factors for post-ext
