Employing in vitro and in vivo models of catheter-associated infection, biofilm formation of Pseudomonas aeruginosa in artificial urine and bactericidal activity of several classes alone or in combination with a macrolide antibiotic were investigated. In the course of continuous urine flow, bacteria adherent to a Teflon catheter formed microcolonies at 6 h and a biofilm at 24 h. Fifty patients had residual urine measured by catheterisation immediately before laparoscopic cholecystectomy. The median amount of urine in the bladder was 100 ml (range 5-500 ml). This urine volume does not distend the bladder enough to risk inadvertant perforation during insertion of a subumbilical port. Urinary bladder catheterisation is unnecessary before laparoscopy especially if the bladder is impalpable.

BACKGROUND AND STUDY AIMS: The accuracy of transurethral bladder catheter pressure in reflecting intra-abdominal pressure is well defined in experimental studies and case reports but not in controlled clinical trials.
PATIENTS AND METHODS: We compared bladder pressure with insufflator pressure during laparoscopic cholecystectomy in 40 patients. Measurements were made at four pressure levels of the insufflator: 0, 5, 10 and 15 mmHg.
RESULTS: When the insufflator displayed 0, 5, 10, and 15 mmHg (0, 6.8, 13.6 and 20.4 cmH2O), and the mean bladder pressures measured 2.5 +/- 1.4, 7.3 +/- 1.5, 12.

A snail-headed catheter retriever was designed to remove the Double-J* catheter from female patients. The catheter can be removed within a few minutes using topical anesthesia at the outpatient department without endoscopic instrumentation and lithotomy position. No infection or bleeding developed following the procedure and all patients tolerated it well. The cost-effective retriever can be used as a convenient alternative for Double-J catheter removal from female patients.

The compliance of practice to national guidelines on urethral catheter care has been examined. Questionnaires on the practices used with patients under their care were sent to 1350 nursing staff. Replies were received from 1153 individuals, 692 from hospitals, 345 from nursing homes and 116 from home care. PURPOSE: The effectiveness of transrenal ureteral occlusion was evaluated.
PATIENTS AND METHODS: Transrenal ureteral occlusions were performed in 83 ureters of 76 patients. Thirty-one ureters were occluded with use of tissue adhesive, which was secured in place with Gianturco coils in 21. Fifty-two ureters were occluded by means of silicone-filled, detachable latex balloons. Average follow-up was 3.6 months (range, 1 week to 38 months) for patients treated with the tissue adhesive and 7.9 months (range, 1 week to 61 months) for patients treated with the detachable balloons.

On any given day more than 100,000 nursing home patients have indwelling urethral catheters in place. We prospectively followed 47 such patients for almost 18 patient-years and observed 249 catheter replacements. Urethral closure with suprapubic catheterization has been used in 50 patients with urethral destruction due to an indwelling catheter. Thirty-seven of the patients were dry after the initial operation. Long-term complications were seen in 33 patients. Those that did occur were not life threatening. This relatively minor procedure offers a good alternative to formal diversion in this group of severely disabled patients.

Ultrasound diagnosis of bilateral hydronephrosis, hydroureters, and megacystis together with oligohydramnios secondary to urethral stenosis was made in a male fetus at 29 weeks' gestation. In utero decompression of the obstructed urinary tract and expansion of the amniotic fluid was achieved by placing an external bladder-amniotic fluid shunt using two catheters, one inserted into the fetal bladder and the other placed in the amniotic sac. The infant was delivered by cesarean section at 35 weeks' gestation and had a favorable outcome.
