A multidisciplinary approach is mandatory in defining the cause and directing the treatment of childhood urinary incontinence. Both pediatricians and pediatric urologists should carefully evaluate a child with incontinence for possible overlapping etiologies, before labeling him or her as a refractory case. We report an epileptic boy with dysfunctional elimination syndrome (DES) with associated rare complications of giggle incontinence and idiopathic urethritis, proving the need for a good voiding diary, patient history and structured symptom scores.

Male urethral diverticulum is a rare condition that poses significant management challenges when complicated by stenosis, false passage, and urinary incontinence. We report a 31-year-old male with refractory urinary incontinence following pelvic fracture urethral injury treated by primary urethral realignment. Chronic penile clamp usage led to progressive mid-urethral diverticulum enlargement. Imaging confirmed posterior urethral stenosis, diverticulum, and a sphincter-bypassing false passage. Surgical management included false passage obliteration, end-to-end urethroplasty, and diverticulecto…

Numerous surgical options are available for the management of stress urinary incontinence in women, including urethral bulking agents, pubovaginal slings, retropubic bladder neck suspensions, midurethral slings, and even artificial urinary sphincters. We discuss the incidence, etiology, diagnosis, evaluation, and management of bladder outlet obstruction after anti-incontinence surgery in women. This problem presents with a wide range of symptoms. Urodynamic evaluation is not always diagnostic but can be helpful. Management options range from conservative to invasive. The optimal timeline for i…

Neurophysiological measurements were carried out in 180 patients presenting to a Joint Incontinence Clinic with a history of lower urinary tract dysfunction. A full urodynamic assessment was performed along with neurophysiological measurement of the voiding reflex arcs. The majority of patients were shown to have significantly abnormal responses on neurophysiological measurement when compared with normal controls. Two particular groups, patients with acute retention and females with stress incontinence, were shown to have markedly poorer results than patients with other urodynamic problems. Th

Weakness of the pelvic floor is in the majority a disease of women. The most frequent complaints are urinary incontinence and visceral descent. The usual methods of gynaecological and urological operations to cure these impairments are demonstrated. Because several patients with a lax pelvic floor suffer from anorectal incontinence a simultaneous interdisciplinary treatment is proposed.

Urinary incontinence in 80 men was classifed by combined radiological and urodynamic techniques. 28 cases of pure stress incontinence were evaluated for grading of sphincteric loss. Clinical and sphincterometric data were correlated. Functional urethral length, urethral closure pressure at rest and with voluntary sphicter activation, and pressure transmission under stress were used to estimate the remaining sphincteric functions. Theoretical guidelines are drawn for the use of operative sphincter repair based on the grade of stress incontinence.

(Case) A 31-year-old woman noticed lower abdominal pain and urinary incontinence after voiding one month after third cesarean section. Cystoscopy and cystourethrography demonstrated a vesicouterine fistula at the posterior wall of the bladder. She complained of paroxysmal lower abdominal pain and slight incontinence without urge after voiding several times a month, which presented before and after menorrhea. Breast-feeding was ended 1 year after the labor, and then she underwent LH-RH agonist to keep amenorrhea. Urinary incontinence resolved completely within five months of hormonal therapy. D…

The majority of urogynaecological problems can manifest during pregnancy or as a direct result of pregnancy and delivery. Those most commonly occurring during pregnancy are urinary tract infection, filling and voiding disorders, urinary incontinence, pelvic organ prolapse and faecal incontinence. The development of these may be as a result of physiological changes that occur in pregnancy or as a result of previous pregnancies. There may also be urogynaecological sequelae that occur as a result of trauma sustained during delivery. These include perineal and anal sphincter trauma, bladder or ure…

Vaginal delivery is known to be associated with trauma to the pelvic floor (PF) and its muscles and ligaments. In some cases, this can lead to issues with urinary incontinence and pelvic organ descent postpartum, either immediately or in the woman's later years. Some factors during a vaginal delivery can be modified in attempts to preserve as much functional integrity to the PF as possible. A trend has evolved in recent years regarding maternal requests for primary cesarean section (CS), with one goal being the reduction in PF trauma. However, evidence to date refutes this as a foolproof pract…

The purpose of this study was to investigate symptoms of estrogen deficiency, androgen excess and urinary incontinence in a representative sample of middle-aged women. A sample consisting of 1746 women aged 40-66 was randomly selected. Of those initially selected, 1413 (81%) were eventually investigated. Gynecological data were obtained by means of questionnaires. 53% of the menstruating and 79% of the naturally postmenopausal women reported one or more symptoms indicating estrogen deficiency. 40% of the postmenopausal women had experienced marked vasomotor symptoms. Symptoms of androgen exces…

Urge incontinence (also known as overactive bladder) is a common form of urinary incontinence, occurring alone or as a component of mixed urinary incontinence, frequently together with stress incontinence. Because of the pathophysiology of urge incontinence, anticholinergic/antispasmodic agents form the cornerstone of therapy. Unfortunately, the pharmacological activity of these agents is not limited to the urinary tract, leading to systemic adverse effects that often promote nonadherence. Although the pharmacokinetics of flavoxate, propantheline, scopolamine, imipramine/desipramine, trospium …

PURPOSE: Faecal incontinence is a common and important multifactorial disorder with a range of treatment options. Over the last two decades, neuromodulation via sacral nerve stimulators has been shown to be effective for both faecal and urinary incontinence, although associated with complications. Peripheral neuromodulation, via the posterior tibial nerve, is widely used in urinary incontinence; however, its use in faecal incontinence, whilst evolving is limited to eight small heterogeneous studies.
REVIEW: These eight studies are discussed in the context of the methodology and underlying neu…

OBJECTIVE: To evaluate prospectively the short- and long-term results of treatment with vaginal cones for women with urinary stress incontinence and to assess predictive parameters.
PATIENTS AND METHODS: Fifty women (mean age 49 years, range 28-76) with differing severities of stress incontinence (mean parity 2.4 and mean 60-min pad-test 20.7 g) were treated with cones for 15 min twice a day for 8 weeks. Treatment results were evaluated immediately after and at a mean of 3 years after the treatment. Parameters were analysed to determine those with predictive value for a successful outcome.

PURPOSE: We prospectively assessed the role of nerve sparing surgery on urinary continence after open radical retropubic prostatectomy.
MATERIALS AND METHODS: We evaluated a consecutive series of 536 patients who underwent open radical retropubic prostatectomy with attempted bilateral, unilateral or no nerve sparing, as defined by the surgeon, without prior radiotherapy at a minimum followup of 1 year with documented assessment of urinary continence status. Because outlet obstruction may influence continence rates, its incidence and management was also evaluated.
RESULTS: One year after surg
