[Hypocalcemic dilated cardiomyopathy: rare cause of heart failure].

Hypocalcemic dilated cardiomyopathy is a rare cause of heart failure in adults. We report a case in a 19-year-old woman who developed congestive heart failure induced by severe chronic hypocalcemia disclosing primary hypoparathyroidism. Complete regression of the clinical signs was achieved with vitamin-calcium treatment, but left ventricular systolic dysfunction persisted eight months after treatment.

A study of systolic time intervals in lepra reaction.

Systolic time intervals (STI) were measured in 20 control subjects and 20 cases of lepromatous leprosy in lepra reaction. Significant differences in the pre-ejection period (PEP), PEP/LVET and isovolumic contraction (IVCT) were observed between the groups. The abnormalities of STI observed in patients of lepra reaction are characteristic of left ventricular dysfunction in patients of lepra reaction.

Right ventricular plasticity in a porcine model of chronic pressure overload.

BACKGROUND: Ventricular-arterial coupling is a measure of the relationship between ventricular contractility and afterload. We sought to determine the relationship between ventricular-arterial coupling and right ventricular (RV) remodeling in a novel porcine model of progressive pulmonary hypertension (PH). METHODS: Chronic PH was induced in pigs by ligation of the left pulmonary artery (PA) followed by 5 weekly injections of cyanoacrylate to progressively obstruct the right lower lobe arteries (PH group, n = 10). At 6 weeks, 5 PH animals underwent reperfusion of the left lung through conduit

Impact of Adjustable Pulmonary Artery Band on Outcomes of Anomalous Left Coronary Artery From the Pulmonary Artery (ALCAPA) Repair.

Objective: Anomalous left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital anomaly resulting in myocardial ischemia, global left ventricular (LV) dysfunction, and mitral regurgitation (MR). Surgical placement of a pulmonary artery band (PAB) at ALCAPA repair may assist with recovery of patients with severe LV dysfunction. We assessed the outcomes of patients with ALCAPA and severe dysfunction treated with, and without a PAB. Methods: We performed a retrospective analysis of cases presented to our center between January 2007 and December 2023. Baseline and follow-up data

Early cardiac dysfunction in children and young adults with perinatally acquired HIV.

OBJECTIVE: To define the prevalence of early cardiac dysfunction in children and young adults with perinatally acquired HIV and predictors of cardiac function. DESIGN: Cross-sectional design. METHODS: Early cardiac dysfunction was defined as left ventricular (LV) global longitudinal strain z-score less than -2 or myocardial performance index at least 0.5 with normal LV ejection fraction. Regression models were fitted to assess the relationship between measures of cardiac function and HIV RNA levels, clinical variables, and markers of inflammation. RESULTS: Six hundred and forty-three indivi

Stent fracture and embolization: an unusual late complication of neonatal coarctation of aorta stenting with bioresorbable vascular scaffold.

BACKGROUND: Critical neonatal coarctation of the aorta (CoA) may present with cardiogenic shock and severe ventricular dysfunction, rendering immediate surgical repair high risk. In such situations, transcatheter interventions may be used as a temporary stabilizing strategy. CASE PRESENTATION: A 10-day-old male neonate with critical CoA presented in cardiogenic shock and severe left ventricular dysfunction. Balloon angioplasty resulted in only transient improvement due to elastic recoil, necessitating rescue implantation of a bioresorbable vascular scaffold. This led to rapid haemodynamic sta

Reverse Remodeling in Late Presenter Peripartum Cardiomyopathy: Strain and Myocardial Work Analysis.

Peripartum cardiomyopathy (PPCM) is a condition of myocardial systolic dysfunction with dilated cardiomyopathy phenotype which occurs in the last months of pregnancy to several months postpartum period. There are several predictors of poor recovery in PPCM, especially severe left ventricular (LV) systolic dysfunction at initial presentation. We reported a case of a 41-year-old woman with late presenter PPCM and the presence of poor predictors of recovery from echocardiographic parameters. Echocardiography showed severely reduced ventricular function and global hypokinetic LV with severe tricus

Echocardiographic assessment of the cardiac anatomy in patients with multifocal atrial tachycardia: a comparison with atrial fibrillation.

Although multifocal atrial tachycardia (MAT) has been recognized since 1968, few data exist on its associated anatomic correlates. Using echocardiography, the authors describe the cardiac anatomy observed in hospitalized patients with MAT. Because MAT closely resembles atrial fibrillation (AF), these echocardiographic data are compared with those from an age- and sex-matched population with AF. There were 25 patients in each of the MAT and AF groups. Biatrial enlargement was present in both groups. However, the atrial enlargement was mild and significantly less in the MAT group. The MAT group

Doppler echocardiographic evaluation of diastolic dysfunction.

Recently, considerable effort has been directed toward the application of Doppler methods for detecting diastolic dysfunction. Recordings of transmitral filling velocity as obtained from pulsed-wave Doppler have been extensively investigated as a noninvasive method for determining left ventricular diastolic filling properties. Although Doppler parameters appear to reflect volumetric transmitral flow, close correlations with invasive descriptors of diastolic performance have not been found. Furthermore, Doppler transmitral velocities are sensitive to changes in ventricular preload, afterload, a

Association of Pre-Transplant Left Ventricular Longitudinal Strain With Long-Term Cardiovascular Outcomes Following Kidney Transplantation.

PURPOSE: Cardiovascular disease (CVD) remains the leading cause of death among kidney transplant recipients (KTRs). Current guidelines recommend using left ventricular global longitudinal strain (GLS) for the early identification of cardiac dysfunction, but its value in KTRs has not been well studied. METHODS: We performed a single-center retrospective cohort study of adult KTRs who underwent KT between January 2015 and January 2024 and had an adequate-quality pre-transplant echocardiogram. Patients without follow-up at our institution or with atrial fibrillation were excluded. GLS was measur

Dilated cardiomyopathy - guidelines for personalization of care.

The term "dilated cardiomyopathy" (DCM) refers to a family of diseases characterized by complex interactions between environment and genetic predisposition. Diagnostic tools such as cardiac magnetic resonance imaging should be systematically implemented in clinical practice to define the etiological cause and undertake a specific treatment. We present the case of a young man with DCM and severe left ventricular dysfunction. The patient has a family history of sudden cardiac death (SCD). He is affected by a psychiatric pathology and has a history of alcohol and drug addiction. Many diagnostic h

Clinical impact of diastolic dysfunction and atrial fibrillation before and after initiating hemodialysis in patients with end-stage kidney disease.

BACKGROUND: Diastolic dysfunction with left ventricular hypertrophy and myocardial fibrosis is an important characteristic of uremic cardiomyopathy in end-stage kidney disease (ESKD). Few studies explored the relationship between changes in diastolic dysfunction and the risk of mortality or cardiovascular outcome in patients with ESKD. We investigated the clinical impact of diastolic dysfunction and atrial fibrillation (AF) on patients starting hemodialysis (HD). METHODS: A total of 718 patients who started HD between 2010 and 2020 were included. We classified patients according to the pre- a

Sex Disparities in Longitudinal Use and Intensification of Guideline-Directed Medical Therapy Among Patients With Newly Diagnosed Heart Failure With Reduced Ejection Fraction.

BACKGROUND: Guideline-directed medical therapies (GDMTs) are the mainstay of treatment for heart failure with reduced ejection fraction (HFrEF), but they are underused. Whether sex differences exist in the initiation and intensification of GDMT for newly diagnosed HFrEF is not well established. METHODS: Patients with incident HFrEF were identified from the 2016 to 2020 Optum deidentified Clinformatics Data Mart Database, which is derived from a database of administrative health claims for members of large commercial and Medicare Advantage health plans. The primary outcome was the use of optim

Clustering regional patterns of left ventricular longitudinal strain in systemic sclerosis-related pulmonary hypertension.

BACKGROUND: Systemic sclerosis-related pulmonary hypertension (SSc-PH) is a heterogeneous disease, often complicated by concomitant left ventricular (LV) dysfunction. However, the contribution of heterogeneity in LV dysfunction is unclear. The objective in this study was to identify regional clusters of LV longitudinal strain via echocardiography to determine how subgroups of LV dysfunction contribute to mortality in SSc-PH. METHODS: We performed a retrospective observational study on 124 patients with SSc-PH in which LV longitudinal strain was collected over a 16-segment model. We applied K-

Impact of aortic valve stenosis on myocardial deformation in different left ventricular levels: A three-dimensional speckle tracking echocardiography study.

BACKGROUND: Global systolic left ventricular (LV) myocardial function progressively declines as degenerative aortic valve stenosis (AS) progresses. Whether this results in uniformly distributed deformation changes from base to apex has not been investigated. METHODS: Eighty-five AS patients underwent three-dimensional (3D) echocardiography in this cross-sectional study. Patients were grouped by peak jet velocity into mild (n = 32), moderate (n = 31), and severe (n = 22) AS. 3D speckle tracking derived strain, rotation, twist, and torsion were obtained to assess global LV function and myocardi
