Complement involvement in bone homeostasis and bone disorders.

An integral part of innate immunity is the complement system, a defence system, consisting of fluid-phase and cell surface-bound proteins. Its role to ensure adequate responses to danger factors and thus promoting host defence against pathogens has been well described already for decades. Recently, numerous further reaching functions of complement have been discovered, among these are tissue homeostasis and regeneration, also with respect to the skeletal system. The influence of complement activation on bone was recognised first in pathological conditions of inflamed bone tissue and surroundin

Severe infections in Portuguese patients with rheumatoid arthritis under biologic treatment - a multicenter, nationwide study (SIPPRA-B Study).

INTRODUCTION: Despite years of experience with biological disease modifying anti-rheumatic drugs (bDMARD) in rheumatoid arthritis (RA), little is known about differences in infectious risk among bDMARDs. The aim of this study was to assess the incidence and type of infections in RA patients on bDMARDs and to determine possible predictors. METHODS: A retrospective multicenter cohort study that included patients registered in the Rheumatic Diseases Portuguese Registry (Reuma.pt) with RA, and exposed to at least one bDMARD until April 2021. RA patients under bDMARD and with at least one episode

Prospective evaluation of real-world performance and clinical impact of the Biofire FilmArray joint infection panel.

Limitations of culture-based diagnostic approaches in pathogen detection in joint infections (JI) can be overcome by amplification-based, molecular assays. Recently, a syndromic panel PCR (spPCR) assay (Biofire JI panel; BJA) was approved for pathogen identification from synovial fluid (SF). Here, the performance and the clinical impact of the BJA were assessed in comparison to standard of care diagnostics in a prospective cohort of patients presenting with symptoms consistent with JI. One hundred sixty-five synovial fluids underwent analysis using the BJA. The results were compared with cultu

18F-FDG PET/CT for the diagnosis of septic shoulder arthritis: metabolic uptake pattern and diagnostic performance.

BACKGROUND: Septic arthritis (SA), although rare, is a critical joint-threatening emergency. The shoulder, being the third most common site after knee and hip joints, is predominantly affected by hematogenous spread of infection. 2-deoxy-2-[18F]fluoro-D-glucose (F-18-FDG) positron emission tomography (PET)/computed tomography (CT) has shown promise in identifying infectious foci and modifying treatment plans. This study aims to differentiate the metabolic patterns of septic shoulder arthritis (SSA) from various stages of shoulder osteoarthritis (OA) using F-18-FDG PET/CT and assess its diagnos

C - reactive protein predicts proximal femur osteomyelitis in suppurative hip arthritis.

BACKGROUND: Ultrasound is capable of detecting hip joint effusion and guiding puncture procedures, which is cheap and convenient, but can not identify osteomyelitis. Magnetic resonance imaging (MRI) has high sensitivity and specificity for diagnosing osteomyelitis, but it is time-consuming and expensive. The purpose of this study was to find out certain indicators to identify suppurative hip arthritis with proximal femur osteomyelitis in order to optimize MRI. METHODS: Data of 48 children with acute suppurative arthritis of the hip diagnosed and treated from March 2018 to September 2024 were

A new biochemical score from synovial fluid for diagnosing septic arthritis.

OBJECTIVES: To evaluate the diagnostic performance of synovial fluid (SF) biochemical markers and assess the value of combining their measurements into a composite score for identifying septic arthritis (SA). METHODS: Patients who underwent arthrocentesis with SF biochemical analysis (proteins, glucose, lactate dehydrogenase (LDH), lactate) were included in the initial cohort (IC) (May 2018-May 2021) or the validation cohort (June 2021-March 2023). Using IC data, we compared marker levels in SA vs. non-SA and vs. crystal-related arthritis (C-rA) subgroup. Based on receiver operating character

The Microbiology and Clinical Presentation of Acute Bacterial Arthritis in Houston Area Children <5 Years Old in the Era of Molecular Diagnostics.

BACKGROUND: While Staphylococcus aureus is the most common pathogen causing acute bacterial arthritis (ABA), the microbiology is diverse, particularly in young children. Kingella kingae is a well-known pathogen of ABA and can be particularly difficult to identify. We examined the impact of molecular diagnostics on ABA in a historically methicillin-resistant S. aureus (MRSA) endemic region. METHODS: Cases of ABA in children ≤5 years old between 2015 and 2022 were reviewed. The clinical features of cases were compared by causative pathogen. Trends in utilization of molecular diagnostics and rat

Predictors of bloodstream infection and its impact on mortality in septic arthritis: A 15-year review.

BACKGROUND: Septic arthritis is frequently complicated by bloodstream infection (BSI), which can lead to metastatic infections and sepsis. In the current study, we aimed to identify risk factors for septic arthritis-related BSI and assess its impact on clinical outcomes. METHODS: A retrospective review spanning 15 years (January 2009 to May 2023) was conducted on patients diagnosed with septic arthritis. Data from patients with positive synovial fluid cultures were analyzed. RESULTS: Among 456 patients with septic arthritis, 16.8 % (n = 77) developed BSI. The 90-day mortality rate was signif

Retrospective validation of a rapid Lyme fluorescent immunoassay in differentiating Lyme arthritis from other musculoskeletal presentations in children in a Lyme-endemic region.

UNLABELLED: Lyme arthritis can present similarly to other causes of joint pain and swelling including septic arthritis and other acute and chronic arthropathies of childhood. Septic arthritis, although rare, constitutes an orthopedic emergency and requires early surgical intervention to reduce the risk of permanent joint damage. Currently, results of standard serologic tests to diagnose Lyme disease take days to weeks, which is unhelpful in acute clinical decision-making. Thus, some children with Lyme arthritis are treated empirically for septic arthritis undergoing unnecessary invasive proced

Reliable Diagnostic Tests and Thresholds for Preoperative Diagnosis of Non-Inflammatory Arthritis Periprosthetic Joint Infection: A Meta-analysis and Systematic Review.

OBJECTIVE: The current diagnostic criteria for periprosthetic joint infection (PJI) are diverse and controversial, leading to delayed diagnosis. This study aimed to evaluate and unify their diagnostic accuracy and the threshold selection of serum and synovial routine tests for PJI at an early stage. METHODS: We searched the MEDLINE and Embase databases for retrospective or prospective studies which reported preoperative-available assays (serum, synovial, or culture tests) for the diagnosis of chronic PJI among inflammatory arthritis (IA) or non-IA populations from January 1, 2000 to June 30,

Utility of synovial calprotectin lateral flow test to exclude chronic prosthetic joint infection in periprosthetic fractures: a prospective cohort study.

The diagnosis of periprosthetic joint infection (PJI) requires a combination of various clinical, laboratory, microbiological and histopathological parameters. A concomitant periprosthetic fracture (PPF) further complicates the diagnosis as it causes a confounding local inflammatory response. Synovial calprotectin has been demonstrated as a promising biomarker of PJI. The purpose of the present study was to evaluate the reliability of synovial calprotectin for the pre- or intraoperative diagnosis of PJI in PFF. 30 patients with PPF and implant loosening were included in this prospective study.

Intraosseous Regional Prophylactic Antibiotics Decrease the Risk of Prosthetic Joint Infection in Primary TKA: A Multicenter Study.

BACKGROUND: Recent studies have demonstrated that the administration of regional prophylactic antibiotics by intraosseous (IO) injection achieves tissue concentrations around the knee that are 10- to 15-fold higher than intravenous (IV) delivery of prophylactic antibiotics. It is currently unknown whether the use of regional prophylactic antibiotics for primary TKA would result in a lower risk of prosthetic joint infection (PJI). QUESTIONS/PURPOSES: (1) Is IO injection of prophylactic antibiotics associated with a decreased risk of early (< 12 months) deep PJI compared with traditional IV pro

Contemporary Clinical Isolates of Staphylococcus aureus from Pediatric Osteomyelitis Patients Display Unique Characteristics in a Mouse Model of Hematogenous Osteomyelitis.

Osteomyelitis can result from the direct inoculation of pathogens into bone during injury or surgery or from spread via the bloodstream, a condition called hematogenous osteomyelitis (HOM). HOM disproportionally affects children, and more than half of cases are caused by Staphylococcus aureus. Laboratory models of osteomyelitis mostly utilize direct injection of bacteria into the bone or implantation of foreign material and therefore do not directly interrogate the pathogenesis of pediatric hematogenous osteomyelitis. In this study, we inoculated mice intravenously and characterized the result

Infections and antibiotics during fetal life and childhood and their relationship to juvenile idiopathic arthritis: a prospective cohort study.

BACKGROUND: The aetiology of juvenile idiopathic arthritis (JIA) is poorly understood. It has been shown that use of antibiotics is associated with JIA. However, whether the association is due to increased occurrence of infection in these individuals is unknown. The purpose of this investigation was to measure the association between number of infections and use of antibiotics during childhood with development of JIA. METHODS: In ABIS (All Babies in Southeast Sweden) a population-based prospective birth cohort of 17,055 children, data were collected on infections and antibiotic exposure durin

Septic Arthritis Caused by Mycobacterium Kansasii in a Bone Marrow Transplant Recipient.

We report an 18-year-old female individual with septic arthritis due to Mycobacterium kansasii. Three years and 6 months before arthritis, the patient underwent bone marrow transplantation and developed severe chronic graft-versus-host disease. The arthritis was refractory to medication, and she underwent joint lavage of the right foot, hip joint, and elbow joint. After surgery, her joint symptoms were relieved, and chronic graft-versus-host disease remitted more easily. It is important that we maintain a high index of suspicion for mycobacterial arthritis and diagnose it early when immunosupp

C-reactive protein is not a screening tool for late periprosthetic joint infection.

BACKGROUND: Preoperative diagnosis of periprosthetic joint infection (PJI) is important because of the therapeutic consequences. The aim of the present study is to investigate whether the serum C-reactive protein (CRP) level can be used as a screening tool for late PJI. MATERIALS AND METHODS: A cohort of 390 patients with revision surgery of total hip prostheses (200) or total knee prostheses (190) was assessed for late PJI by determining CRP serum level and performing preoperative aspiration with cultivation and intraoperative tissue analyses with cultivation and histologic examination, usin

Post-traumatic Curvularia sp. arthritis in an immunocompetent adult.

A 44-year-old woman, victim of a road accident in Mali was diagnosed with left knee arthritis. Joint effusion aspiration and subcutaneous surgical biopsies were positive for a melanized asexual ascomycete. Using microscopy and molecular biology, the fungus was identified as Curvularia sp. In vitro antifungal susceptibility was determined by the EUCAST broth microdilution reference technique and by E-test. The patient was treated with liposomal amphotericin B before posaconazole relay. Mycological samples obtained 10 days after starting the antifungal therapy by liposomal amphotericin B were ne

Facing multidrug-resistant pathogens in periprosthetic joint infections with self-administered outpatient parenteral antimicrobial therapy-A prospective cohort study.

A key factor in the successful management of periprosthetic joint infection (PJI) besides the surgical regime is a consistent antimicrobial therapy. Recently, oral versus intravenous (IV) antibiotics for bone and joint infection trial demonstrated the noninferiority of oral antimicrobial therapy compared to IV, implying that an early transition to oral administration is reasonable. It is likely that the international consensus meeting of musculoskeletal Infections and the European Bone and Joint Infection Society will consider these findings. However, rising levels of antimicrobial resistance
