Hyperreflexia in a patient with motor axonal Guillain-Barré syndrome.

We report a patient who presented after an episode of diarrhoea with ascending, symmetrical weakness without sensory loss, and without sphincter or other autonomic dysfunction. On clinical examination there were no cranial nerve deficits. Hyperreflexia of tendon jerks without other upper motor neurone signs was found. Electrophysiological examination demonstrated acute distal symmetrical motor axonal polyneuropathy. No electrophysiological signs of peripheral nerve demyelination or central nervous system involvement were found. Albuminocytologic dissociation was present in the cerebrospinal fl

Unraveling the central nervous system pathways underlying responses to leptin.

Here we summarize recent progress in the biology of leptin, concentrating on its central nervous system (CNS) actions. The product of the ob gene, leptin is a circulating hormone produced by white adipose tissue that has potent effects on feeding behavior, thermogenesis and neuroendocrine responses. Leptin regulates energy homeostasis, as its absence in rodents and humans causes severe obesity. We consider the physiological mechanisms underlying leptin action, along with several novel hypothalamic neuropeptides that affect food intake and body weight. The molecular causes of several other obes

Runner's diarrhea and other intestinal problems of athletes.

Lower gastrointestinal tract symptoms occur frequently in runners. The most common complaints are bowel urgency and diarrhea. Many etiologies have been proposed, including enteric fluid and electrolyte imbalance, autonomic nervous system stimulation, ischemia and mechanical trauma. The evaluation should include a review of the athlete's training program and diet, as well as a review of any preexisting gastrointestinal disease. A stool examination for occult blood should be performed in the athlete who complains of diarrhea. Treatment begins with a reduction in the intensity of workouts, follow…

Redefining the role of digoxin in the treatment of atrial fibrillation.

Atrial fibrillation (AF) encompasses a variety of discrete clinical syndromes, including paroxysmal, chronic, acute, and postoperative. Digoxin, long considered the mainstay of therapy for rate control in all types of AF, appears to have only modest electrophysiologic effects, which are mediated primarily by the autonomic nervous system. Digoxin has less potency than the calcium antagonists or beta-blocking drugs with respect to atrioventricular nodal blockade. Although less potent than calcium antagonists or beta-blocking drugs on the atrioventricular node, digoxin provides positive inotropic…

Lambert-Eaton syndrome.

LES is an autoimmune disorder of the neuromuscular junction in which autoantibodies directed against voltage-dependent Ca2+ channels block nerve-evoked Ca2+ entry at the motor nerve terminal. The pathogenic IgG is likely to produce a similar inhibitory effect on the Ca2+ channel function in other cholinergic synapses of the autonomic nervous system. This pathophysiology is sufficient to account for the distinctive clinical, immunologic, and electrophysiologic manifestations in patients with LES. Etiology of this disease is uncertain but in view of its frequent association with small cell lung …

Orthostatic hypotension: therapeutic alternatives for geriatric patients.

Symptomatic orthostatic hypotension is a serious problem in the elderly because it can precipitate falls and fractures, myocardial infarctions, and strokes. Several disorders may cause symptomatic orthostatic hypotension including age-related changes in physiology, disorders of the autonomic nervous system, drugs, and a decrease in circulating blood volume. Orthostatic hypotension is defined as a fall in systolic pressure of at least 20-30 mm Hg and a fall in diastolic pressure of at least 10-15 mm Hg upon rising, with symptoms of cerebral ischemia. Management includes a search for reversible …

A new technique to determine the correlation between the QT interval and heart-rate for control and SIDS babies.

The ability of the autonomic nervous system to alter the QT interval in response to heart rate changes is essential to cardiovascular control. An accurate way to determine the relation between QT intervals and their corresponding RR intervals is described. A computer algorithm measures the RR intervals using digital filtering and cross-correlating the QRS sections of consecutive waveforms. The QT intervals is calculated by choosing a section of, the ECG that includes the T wave and cross-correlating it with all the consecutive T waves. At least 4000 pairs of QT-RR intervals are computed for ea…

7-(Trifluoromethyl)-4-aminoquinoline hypotensives: novel peripheral sympatholytics.

A family of 7-(trifluoromethyl)-4-aminoquinolines that are hypotensive agents and that act by a novel sympatholytic mechanism is described. Structure-activity relationships in this series have been elucidated. Some of the more potent hypotensives were evaluated for safety in the mouse. A candidate, 1-[(4-fluorophenyl)sulfonyl]-4-[4-[[7-(trifluoromethyl)-4- quinolinyl]amino]benzoyl]piperazine hydrochloride (losulazine hydrochloride) has been selected for clinical development. Losulazine hydrochloride is a hypotensive agent in the rat, cat, and dog. At acute effective hypotensive doses, it does

Norepinephrine-induced pulmonary petechiae in the rat: an experimental model with potential implications for sudden infant death syndrome.

Autonomic nervous system abnormalities have been implicated in infants dying of or considered at high risk of sudden infant death syndrome (SIDS). In a rat model, norepinephrine (0.02 mg/kg) caused systemic hypertension and numerous pulmonary petechiae, the latter a common finding in SIDS. Petechiae were not seen below the diaphragm. The animals were killed by tracheal occlusion 5 min after the intravenous administration of norepinephrine. The number of pulmonary petechiae was greatly reduced by alpha-adrenergic blockade (phentolamine) and dopaminergic blockade (haloperidol) but not by beta-ad…

[Neuropathy with contractures evoking the stiff-man syndrome. Latent solitary plasmacytoma].

A further case of apparently primary sensorimotor neuropathy is reported. The patient, a 31-year-old man, developed the syndrome after a 4-year history of a solitary vertebral plasmocytoma revealed by post mortem examination. Circulating monoclonal immunoglobulin and specific anomalies on nerve biopsy were absent. The condition became progressively worse, with initial and predominant autonomic nervous system anomalies. After 3 years, contractures of the limbs and paravertebral muscles, with painful paroxysmal attacks suggestive of the stiff-man syndrome appeared. Pathology confirmed the severi…

[Disturbances of micturition after general surgical operations (author's transl)].

Impaired micturition after general surgical operations can be due to three causes, which occur individually or in combination. The first comprises side effects of anesthetics upon the autonomic nervous system. Inhibition of the abdominopelvic reflex, initiating micturition, due to the operative trauma represents the scond cause. Both the vegetative and the mechanical traumas, lead to decompensation of the urinary bladder, whose function is impaired by neuropathic factors and outflow obstruction. Pulmonary, cerebral, and abdominal insufficiency prolong the effects of both traumas. The third cau…

Chronic idiopathic intestinal pseudo-obstruction with malabsorption, a scleroderma-like disorder.

A case of intestinal pseudo-obstruction in a 76-year-old man who presented with a six-year history of intermittent vomiting and abdominal pain and a 1(1/2)-year history of diarrhea is described. Investigation demonstrated aperistalsis of the lower esophagus and an unresponsive atonic small intestine. Marked malabsorption was present and appeared due to intestinal bacterial overgrowth consequent to the hypomotility. Autonomic nervous system testing failed to reveal any definite abnormality. Although this disorder is markedly similar to scleroderma involving the gastrointestinal tract, there was…

The Role of Sensory and Sympathetic Nerves in the Development and Bone Metabolism of Mandible.

The nervous system plays a crucial role in regulating bone remodeling, with both sensory and autonomic nerves distributed within bone tissue. In the mandible, the inferior alveolar nerve (IAN) and sympathetic nerves provide the primary innervation. Numerous studies have demonstrated their essential functions in mandibular development and bone metabolism. Focusing on mandibular innervation, we synthesize current evidence to demonstrate the modulatory effects of sensory and sympathetic nerves on bone growth and regeneration, and further elucidate the underlying mechanisms. Notably, we discuss mu

Little Known Facts, Controversies and Misconceptions About Cranial Motor Nuclei (and a New Classification for All Motoneurons, Cranial, and Spinal).

Cranial nerves represent a notoriously complex province of the neuroanatomical landscape of the vertebrates. Here, we offer a selection of the anatomic, genetic, and developmental features of their efferent component that are often misrepresented, ignored or controversial, as a complement to more exhaustive treatments of the subject. Our description reveals that efferent (or "motor") neurons in vertebrates represent a vague anatomic category (such as that of interneurons) rather than a true neuron type; That motor neurons fall into three bona fide types, segregated on the rostro-caudal axis of

Harmonic Analysis of Electrical Activity in Pelvic and Lumbar Regions of Healthy Men.

To noninvasively assess activity of autonomic nervous system (ANS) in pelvic as well as in left and right dorsal lumbar regions of healthy men (n = 26) in the neighborhood of prostate and kidneys, respectively, the study measured the voltaic (voltage differences) and ohmic (electrical impedances) signals. The recorded signals were processed with Fourier transform yielding the ohmic and voltaic spectral peaks in the ranges of Mayer frequency (M = 0.03-0.2 Hz) and in the Traube-Hering frequency of controlled respiration rate (TH = 0.35 Hz), which reflected activity of sympathetic and parasympath…

Treatment of hypothalamic obesity in people with hypothalamic injury: new drugs are on the horizon.

Hypothalamic obesity (HO) is a complex and rare disorder affecting multiple regulatory pathways of energy intake and expenditure in the brain as well as the regulation of the autonomic nervous system and peripheral hormonal signaling. It can be related to monogenic obesity syndromes which often affect the central leptin-melanocortin pathways or due to injury of the hypothalamus from pituitary and hypothalamic tumors, such as craniopharyngioma, surgery, trauma, or radiation to the hypothalamus. Traditional treatments of obesity, such as lifestyle intervention and specific diets, are still a the…

Multiple strategies enhance the efficacy of MSCs transplantation for spinal cord injury.

Spinal cord injury (SCI) is a serious complication of the central nervous system (CNS) after spine injury, often resulting in severe sensory, motor, and autonomic dysfunction below the level of injury. To date, there is no effective treatment strategy for SCI. Recently, stem cell therapy has brought hope to patients with neurological diseases. Mesenchymal stem cells (MSCs) are considered to be the most promising source of cellular therapy after SCI due to their immunomodulatory, neuroprotective and angiogenic potential. Considering the limited therapeutic effect of MSCs due to the complex path
