Odontoid balloon kyphoplasty associated with screw fixation for Type II fracture in 2 elderly patients.

Anterior screw fixation is a well-recognized technique that is used to stabilize Type IIB fractures of the odontoid process in the elderly. However, advanced age and osteoporosis are 2 risk factors for pseudarthrosis. Kyphoplasty has been described in the treatment of lytic lesions in C-2. The authors decided to combine these 2 techniques in the treatment of unstable fractures of the odontoid. Two approximately 90-year-old patients were treated for this type of fracture. Instability was demonstrated on dynamic radiography in one patient, and the fracture was seen on static radiography in the o

Compartment syndrome of the foot secondary to fixation of ankle fracture--a case report.

Compartment syndrome in the leg after open reduction and internal fixation (ORIF) of ankle fractures is rare. Compartment syndrome in the foot after ORIF of ankle fractures has not been reported before. A high degree of vigilance must be exercised when managing these fractures in order to avoid complications and morbidity. We report a case in which a bimalleolar fracture of the left ankle resulted in compartment syndrome of the foot.

Guided growth for angular correction: a preliminary series using a tension band plate.

The classic treatment of pathological angular deformities of the extremities is corrective osteotomy; however, osteotomies require hospitalization, pain management, immobilization, and delayed weight bearing. The associated risks, inconvenience, and cost of osteotomy make hemiepiphysiodesis or guided growth an attractive option. Although stapling has a long and proven track record, reported drawbacks related to implant failure, including migration or breakage of staples, have led some to abandon this technique. This report describes a prospective series of 34 consecutive patients who presented

Clinical inquiries. What is the most effective management of acute fractures of the base of the fifth metatarsal?

For acute Jones' fractures in recreationally active patients, early intramedullary screw fixation results in lower failure rates and shorter times to both clinical union and return to sports than non-weightbearing short leg casting (strength of recommendation [SOR]: A, based on 2 randomized controlled trials [RCT]). Non-weightbearing short leg casting achieves union in 56% to 100% of patients but can require prolonged casting (SOR: B, based on 2 prospective cohorts and multiple retrospective, follow-up studies). Stress fractures were not included in this review. For avulsion fractures of the f

[Open valgus alignment osteotomy of the proximal tibia with fixation by medial plate fixator].

High tibial osteotomy (HTO) is an established technique for the treatment of the symptomatic varus malaligned knee. Correction is usually achieved by closed wedge osteotomy from a lateral exposure. This procedure has a certain risk potential regarding peroneal nerve injuries, instability of the osteotomy and secondary loss of correction. We present four technical modifications of HTO which improve safety and reproducibility of this operation. 1) Open wedge osteotomy from a medial exposure avoids lateral muscle detachment, dissection of the peroneal nerve, proximal fibula osteotomy and leg shor

Application of bone graft to the medial side of the first metatarsal head in the treatment of hallux varus.

BACKGROUND: Hallux varus deformity is not frequent, is usually acquired, and is poorly tolerated by patients. A common cause is the resection of an excessive amount of the head of the first metatarsal during an operation performed to correct a hallux valgus deformity. The purpose of this study was to evaluate the results of application of bone graft to the medial aspect of the first metatarsal head in order to restore missing bone after resection of an excessive amount of bone during a bunionectomy. METHODS: Of thirty patients who had a hallux varus deformity that was treated operatively, eig

[Disruption of the pubic symphysis with overriding impacted symphysis. Apropos of a case].

INTRODUCTION: The authors report one case of overriding impacted symphysis by lateral compression injury of the pelvis. MATERIAL AND METHODS: A 30 year old male presented a disruption of the symphysis pubis following a motor vehicle accident with lateral injury. Radiographs of the pelvis revealed a displacement of the right part of the symphysis behind the left one and a protrusion of the left pubic body through the right obturator foramen. CT scan did not show any sacro-iliac disruption but a fracture of the lateral part of the sacrum. An urethral injury with a complete urethral disruption w

Hollow bone cement filled with impacted cancellous bone as a substitute for bone grafts in cervical spine fusion.

Autogenous iliac crest bone is the gold standard for graft material in cervical spinal fusion, but a high incidence of donor site morbidity has been reported. Therefore, to achieve a better fusion rate and decrease the rate of donor site morbidity, we used a novel method whereby hollow bone cement filled with impacted cancellous bone is used as a substitute for bone graft in cervical interbody fusion. From 2003 to 2004, 21 patients with cervical spinal disorders who were treated using this method were included in this study. Fourteen patients underwent single-level grafts and seven underwent t

Osteonecrosis of femoral head after cement augmented TFNA nailing for an intertrochanteric fracture: A case report.

Failure of fixation devices (cut through) in proximal femoral fractures continues to be a concern in severe osteoporosis. Bone cement augmentation using TFNA (Trochanteric Femoral Nail-Advanced™; DePuy Synthes) increases the bone implant interphase and thereby the biomechanical stability, offering better resistance to avoid cut through. Cement augmentation does not improve implant positioning or reduction but only provides mechanical advantage to the osteoporotic bone and was reported as a safer technique in literature. Most commonly reported complication following cement augmented TFN-A inser

Application of mesenchymal stromal cell sheets to prevent medication-related osteonecrosis of the jaw with titanium implants in rats.

Medication-related osteonecrosis of the jaw (MRONJ) is an intractable adverse event. Dental implants are one of the triggering factors of MRONJ, and implant therapy with low MRONJ risk is required. This study aimed to investigate a rat model of MRONJ induced by extraoral placement of titanium materials and the use of mesenchymal stromal cell (MSCs) sheets to prevent MRONJ. Eight-week-old male rats were administered zoledronate and dexamethasone thrice weekly until killing. A week after drug initiation, a titanium screw and a plate were placed on the left buccal side of the mandible. Allogeneic

Management of combined ridge defect and osteotome sinus floor elevation with simultaneous implant placement--a 36-month follow-up case report.

This report demonstrated the management of combined ridge defect and maxillary sinus pneumatization with simultaneous implant placement. One case with vertical and horizontal ridge deficiency and sinus pneumatization in the maxillary premolar area was indicated for ridge augmentation and sinus elevation before implant placement. Implant osteotomy was enlarged using a ridge expansion osteotome to 1 mm short of the sinus floor; sinus elevation was performed using sinus lift osteotomes; the implant was placed; bone graft and resorbable membrane were used to augment the remaining defect. The secon

Technical aspects of performing a hemivertebrectomy for adolescent congenital scoliosis: Illustrative case.

BACKGROUND: Hemivertebrae (HVs) are congenital spinal anomalies that disrupt normal vertebral formation, often leading to progressive spinal deformity. Segmented HV-associated scoliosis presents significant surgical challenges, requiring meticulous planning and precise execution for optimal outcomes. OBSERVATIONS: A 12-year-old female presented with severe lumbar scoliosis (75° focal curve) due to a fully segmented supernumerary HV at L3-4, causing coronal trunk shift, pelvic obliquity, and activity-limiting back pain. A short-segment L1-5 posterior instrumented fusion with vertebral column r
