Titanium or polymethylmethacrylate in cervical disc surgery? A prospective study.

In 200 patients with either isolated protrusion of disc or with spondylosis a cervical discectomy in one or two levels was performed from a ventral approach. In 100 patients the removed disc was replaced with an implant of polymethylmethacrylat (PMMA). In 100 patients interbodyfusion was done with a titanium cage. Both groups were analysed in a prospective study. Clinical outcome was assessed after surgery. Results were similar in terms of complications and clinical outcome. As the PMMA surgery took longer and the costs of titanium are higher, there is no marked advantage of one implant materi

Anteroposterior diameter of the vertebral canal in cervical region: comparison of anatomical, computed tomographic, and plain film measurements.

The purpose was to compare the computed tomographic and plain film measurements with those of anatomical specimens to determine the antero-posterior diameter of the spinal canal in cervical region. Antero-posterior diameters of 75 cervical vertebral canals (15 sets of C3-C7) were measured anatomically at two different levels. Computed tomographic and plain film measurements were also obtained at the corresponding levels. Considering anatomical measurements as the gold standard, plain film and computed tomographic measurements were statistically compared. Interobserver and intraobserver differe

[Spinal epidural abscess secondary to pharyngeal trauma due to food].

INTRODUCTION: In recent years the incidence of spinal epidural abscesses has increased. Early diagnosis is very important, since once neurological deficit has become established this is often permanent. The commonest aetiological agent is Staphylococcus. Pain and fever are often the only symptoms before acute neurological deterioration. CASE REPORT: A 76 year old hypersensitive, diabetic man presented complaining of pain and a sensation of warmth in both arms. He had no fever or other symptoms. Some three weeks before admission to hospital a fish bone had become stuck in his pharynx, causing

Cervical myelopathy caused by C3-C4 spondylosis in elderly patients: a radiographic analysis of pathogenesis.

STUDY DESIGN: A radiographic analysis of elderly patients with cervical spondylotic myelopathy, particularly those with involvement of the C3-C4 level. OBJECTIVES: To elucidate the potential contributors to the higher incidence of pathology at C3-C4 in the elderly. SUMMARY OF BACKGROUND DATA: In this study, the elder patients showed a greater predilection for involvement of the C3-C4 lesion compared with their younger counterparts. No previous study has addressed C3-C4 pathology in elderly patients. METHODS: This study included 18 patients, 10 men and 8 women, with cervical spondylotic myel

An apparent reduction in the incidence and severity of spinal cord injuries in schoolboy rugby players in the western Cape since 1990.

OBJECTIVE: To determine the impact of the 1990 rugby law changes in South African schoolboy rugby on the number of schoolboys suffering paralysing spinal cord injuries in the subsequent eight rugby seasons (1990-1997) in the former Cape Province (now the Western Cape, but including Port Elizabeth and East London). METHODS: The study was a retrospective analysis of all patients with rugby-related spinal cord injuries admitted to the Conradie and Libertas Spinal Units, Cape Town, between 1990 and 1997. Data were initially collected annually from patient files. From 1993 patients were interviewe

The optimal radiologic method for assessing spinal canal compromise and cord compression in patients with cervical spinal cord injury. Part I: An evidence-based analysis of the published literature.

STUDY DESIGN: An evidence-based analysis of published radiologic criteria for assessing spinal canal compromise and cord compression in patients with acute cervical spinal cord injury. OBJECTIVES: This study was conducted to determine whether literature-based guidelines could be established for accurate and objective assessment of spinal canal compromise and spinal cord compression after cervical spinal cord injury. SUMMARY OF BACKGROUND DATA: Before conducting multicenter trials to determine the efficacy of surgical decompression in cervical spinal cord injury, reliable and objective radiog

[Dorsal sensory level as a false localizing sign in cervical myelopathy].

INTRODUCTION: Cervical myelopathy is a common disorder of the spinal cord. The most frequent symptoms are muscle weakness and spasticity starting in the legs. Occasionally the sensory levels are several spinal cord segments below the cervical lesion which makes diagnosis difficult. CLINICAL CASE: We present the case of a 31 year old man who presented (after exercise) with paresthesia and numbness below the left costal margin in the left hemiabdomen and the left leg. On physical examination there was painful superficial tactile thermic hypoaesthesia at the level of D10 on the left side, togeth

[Symptomatic arteriovenous fistula in a patient with neurofibromatosis type I].

This paper reports the case of a 54-year-old woman who had a history of neurofibromatosis (NF I) presenting progressive quadriplegia and urinary incontinence due to a cervical arteriovenous fistula (AVF). MRI revealed a huge flow void mass in the cervical subcutaneous tissue as well as within the spinal canal. These flow voids originated in the left vertebral artery. A high intensity lesion was observed in the spinal cord adjacent to the flow void. Angiography revealed that the cervical AVF was fed by the third segment of the left vertebral artery with a rich communication with the intraspinal

Cervical osteomyelitis following tonsillectomy.

We present a case of fatal cervical osteomyelitis following an elective tonsillectomy in a previously fit young man. Following induction of general anaesthesia, and prior to surgery, the patient received bilateral glossopharyngeal nerve blocks with 0.5% bupivacaine and adrenaline 1:200,000. The initial recovery was uneventful but persistent throat and neck pain developed at home which was diagnosed as a throat infection and possible hyperextension injury of the neck. It is impossible to say how much the dissection of chronically infected tonsils or the infiltration of local anaesthetic into or

The cervical spine in Saethre-Chotzen syndrome.

Twenty patients with a diagnosis of Saethre-Chotzen syndrome had their cervical spine radiographs reviewed. Radiologic abnormalities including vertebral fusion were present in 9 of the 20 patients. Fusion of both the vertebral bodies and the posterior elements was noted, although the latter site was more common. C2-3 was the level most commonly involved, although other levels were recorded. Analysis of sequential radiographs in nine patients revealed evidence of progression in seven patients. In those studies in children aged under 2 years, only 1 of 18 films showed evidence of fusion, while i

Posttraumatic cervical osteophytosis causing progressive dysphagia.

Dysphagia is a commonly encountered patient complaint. The differential diagnosis for dysphagia is extensive. One long-recognized etiology of dysphagia is cervical osteophytosis. Degenerative joint disease, ankylosing spondylosis, and diffuse idiopathic skeletal hyperostosis (DISH) can all cause cervical osteophyte formation. We describe a patient with dysphagia and a large cervical osteophyte. Our case illustrates cervical osteophytosis associated with a history of previous cervical spine trauma. Evaluation and management strategies are discussed.

Finite element modeling of cervical laminectomy with graded facetectomy.

In this study, an anatomically accurate three-dimensional finite element model of the human lower cervical spine (C4-C6) was used to study the biomechanical effects of cervical laminectomy with and without graded facetectomy. The intact finite element model was validated under flexion, extension, lateral bending, and axial torsion load vectors of 1.8 Nm magnitude. The moment rotation response of the finite element model matched well with experimental data. The gross external (angular motion) and the internal (superior and inferior intervertebral disc stress) responses were delineated under the

[Myelopathy caused by hypertrophy of the posterior longitudinal ligament of the cervical spine in a patient with long-term hemodialysis].

A 48-year-old female was admitted to our hospital because of progressive gait deterioration over a period of 4 months. She had been treated by hemodialysis for 14 years after bilateral nephrectomy. She originally suffered achondroplasia. Otherwise she was a healthy woman. Neurological examination showed weakness and hypesthesia below C5, accelerated deep tendon reflexes and inability in walking. T1 weighted magnetic resonance imaging revealed severe cord compression from C4 to C6 due to an epidural isointensity mass located in the ventral surface of the dura. This mass was enhanced by a contra

Tuberculosis of the spine.

Chemotherapeutic agents remain the cornerstone of treatment for spinal tuberculosis. The arguments that surgery is necessary for biopsy and to allow for removal of infection and its byproducts have been usurped by advanced biopsy techniques and trials demonstrating resolution of infection and most abscesses using chemotherapy. Although chemotherapy alone has been demonstrated to be effective for some neurologic deficits, those lesions that may be safely treated without surgery remain to be fully defined. Modern imaging techniques have allowed more precise diagnosis and identification of neural
