The BacT/Alert (BTA) (Organon Teknika, Durham, NC) and Isolator 10 (ISO) (Wampole Laboratories, Cranbury, NJ) blood culture systems were evaluated for their ability to detect aerobic and facultatively anaerobic microorganisms in blood of adult patients. For each culture 8 mL of blood was inoculated into both the aerobic standard BTA bottle and the ISO tube. Of 7,259 paired culture sets, 1,168 organisms were recovered, and 667 (57.1%) of these were considered clinically significant. Patients with fungemia, mainly due to Candida albicans, had cultures repeated from arterial and venous sites to determine yeast cell clearance during fungemia. Of the 48 patients, 37 had repeat positive cultures (36 arterial and venous samples; one venous culture). Additionally, 24 patients had arterial and venous samples cultured quantitatively. A 46-year-old woman from Colombia, without any medical comorbidities, presented with marked yellowish discoloration of both big toenails, suggestive of onychomycosis, and malalignment of the nail plate. Culture of nail scrapings grew dark-pigmented, mucoid, yeast-like colonies identified as Exophiala alcalophila by ITS sequencing. Broth microdilution identified the isolate as resistant to fluconazole and the echinocandins. The patient refused antifungal treatment. This case highlights the importance of black yeast-like fungi as etiological agents of onychomycosis.

We report a case of a 66-year-old male patient presented to our pneumology ward with the diagnosis of neutropenic pneumonia. Therapy with granulocyte colony stimulating factors (G-CSF) and intravenous antibiotics was initiated as usual in this condition. The unexpected and acute onset of left-sided abdominal pain and sings of hypovolemic shock led us to a challenging diagnosis, rarely considered in non-traumatic patients. After pathological evaluation of the spleen, spontaneous splenic rupture due to G-CSF was our final diagnosis.

We present a three-year-old girl with respiratory failure due to hereditary pulmonary alveolar proteinosis caused by abnormal alpha chain of the granulocyte-macrophage colony-stimulating factor receptor. Both the patient and an asymptomatic seven-year-old sister were homozygous for the same mutation in CSF2RA. We speculate that the Mycoplasma pneumoniae pneumonia might have triggered the clinical presentation. While a good response to serial partial lung lavage was noticed, the ultimate outcome is uncertain.

In this study, the in vitro adherence capabilities of Pseudomonas aeruginosa and Candida albicans clinical isolates to urinary catheters were investigated. Quantitative analysis was performed by colony-forming unit counts and scanning electron microscopy. Results demonstrated that the adherence of P. aeruginosa to urinary catheters was enhanced in the presence of C. albicans, while C. albicans adherence was not significantly affected. Schistosomiasis was diagnosed in two Portuguese soldiers who had been deployed to Portuguese colonies in Africa. The first veteran was diagnosed as having schistosomiasis 34 years after returning from Angola, and the second veteran was found with Schistosoma haematobium infection 40 years after returning from Mozambique. The patient with Schistosoma mansoni had an active infection, because eggs were recovered with living miracidia. The second patient had developed urothelial cancer, but eggs recovered were calcified.

A case of disto-lateral onychomycosis of the third toe of the right foot of a 45-year-old man is reported. The agent of infection was Ulocladium botrytis, a mycete not hitherto reported to infect humans. The patient underwent mycological examination consisting of direct microscope observation and culture of pathological material. Diagnosis was based on the macro- and microscopic characteristics of the colonies. Fragments of the latter were also examined by scanning electron microscope. Clinical and mycological recovery was achieved after 3 months of topical therapy with ciclopiroxolamine.

An indirect ELISA, using local strains of Mycoplasma agalactiae and Mycoplasma mycoides subspecies mycoides large colony (MmmLC), was applied to evaluate the seroprevalence of M agalactiae and MmmLC in flocks of goats on each of the Canary Islands. In total 3890 samples of serum were collected from 204 flocks. The results indicated that the seroprevalence of both organisms is high on all the islands; average values of 55 per cent and 67 per cent were recorded, respectively, for M agalactiae and MmmLC.

A 69-year-old woman presented with a one-year history of dysphagia and a hard overgrowth of the right palatine tonsil visible in oral examination. Computerized Tomography scan showed a big calcified lesion inside the tonsil not seen in a pharyngoscopy. Tonsillectomy was performed and a giant tonsillolith was found inside the resected piece. The calculus was 3 x 2 x 2.3 cm and with an irregular-pyramidal shape. Normal tonsillar tissue with many colonies of Actionomyces inside the tonsillar crypts was the definitive pathological feature.
