Stoma-free precision sphincter-preserving surgery with J-pouch for metastatic low rectal neuroendocrine tumor.

R0 resection and maximal preservation of anal function without a permanent stoma remains challenging in patients with a rectal neuroendocrine neoplasm (R-NEN) within 5 cm of the anal verge and lymph node metastasis in the iliac vascular region. A 22-year-old man presented with a 1-year history of paroxysmal palpitations and cold sweats. Colonoscopy revealed a 2-cm mass with well-defined margins located 5 cm from the anal verge. Biopsy revealed a neuroendocrine tumor. Endoscopic submucosal dissection and pathology revealed a G2 R-NEN with negative margins but vascular invasion. Enhanced MRI and

Association of repeated endoscopic inflammation with dysplasia and colorectal cancer in ulcerative colitis.

BACKGROUND AND AIM: ulcerative colitis (UC) confers an increased risk of dysplasia and colorectal cancer (CRC), largely driven by sustained mucosal inflammation. While inflammatory burden is a known risk factor, the specific impact of repeated endoscopic inflammation and contemporary therapies exposure warrant further investigation. We aimed to investigate the impact of cumulative inflammation in the risk of dysplasia and CRC development in UC. METHODS: a retrospective cohort study of adult UC patients diagnosed between 1975 and 2023 was performed at a tertiary IBD center. Repeated colonoscop

Development and internal validation of a nomogram to predict colorectal cancer risk in patients with diverticulitis.

AIM: Diverticulitis, considered a benign condition, has emerging evidence linking it to colorectal cancer (CRC). Current guidelines lack standardised risk stratification tools to guide post-diverticulitis surveillance. This study aims to create and validate a nomogram to predict CRC risk in patients with diverticulitis. METHOD: This retrospective cohort study included 1546 patients diagnosed with diverticulitis at a UK tertiary hospital between January 2021 and December 2023. Patients aged ≥ 18 years who underwent endoscopic assessment following a diverticulitis episode were included. A logis

Impact of an Artificial Intelligence-Based Computer Aided Detection System (CADe) on Colonoscopies Performed in Hawai'i.

The demand for colonoscopies has increased over the last few years due to an aging population combined with a post COVID-19 backlog as well as an ongoing rise in younger patients with colorectal cancer (CRC). Quality indicators ensure that high quality colonoscopies are being performed by endoscopists, thus decreasing the risk of post-colonoscopy CRC arising from lesions missed on the index exam. One common outcome measure is known as the adenoma detection rate (ADR), which is the likelihood that an endoscopist will detect a pre-cancerous polyp during colonoscopy. The Artificial Intelligence (

Molecular functions, clinical correlates, and therapeutic prospects of B7-H3 expression in colorectal cancer: A systematic review.

Colorectal cancer (CRC) is the second leading cause of cancer-related death. The tumor immune microenvironment plays a critical role in tumor progression and immune evasion. B7-H3 (CD276), a member of the B7 family of immune checkpoint molecules, is frequently overexpressed in a variety of malignancies, including CRC. A systematic search of the literature was performed to identify studies evaluating B7-H3 in CRC. Data were extracted on study characteristics, B7-H3 expression and detection methods, molecular and signaling pathways, and clinical outcomes. Study quality and risk of bias were asse

Comparative Performance of Image-Enhanced Endoscopy for Adenoma and Sessile Serrated Lesion Detection: A Network Meta-Analysis.

BACKGROUND AND AIMS: Image-enhanced endoscopy (IEE) technologies have been developed to improve colorectal polyp detection, but their comparative efficacy remains uncertain. We performed a network meta-analysis to compare the efficacy of major IEE modalities on colorectal polyp detection. METHODS: We searched MEDLINE, Embase, and CENTRAL for randomized controlled trials comparing seven IEE modalities (autofluorescence imaging, blue-light imaging, flexible spectral imaging color enhancement, i-scan, linked color imaging [LCI], narrow band imaging [NBI], and texture and color enhancement imagin

Remodeling of Colorectal Cancer Extracellular Matrix after Radiotherapy.

Colorectal cancer (CRC) is a common and aggressive malignancy with poor prognosis. The efficacy of radiotherapy is often limited by the development of radioresistance, which can be attributed to various factors, including the extracellular matrix (ECM). The effect of radiotherapy on the ECM proteome remains poorly understood. We investigated changes in the proteome composition of CRC tumors after radiation therapy. Quantitative LC-MS/MS analysis of the purified ECM fraction was performed, that was supplemented by transcriptomic analysis of clinical samples obtained from patients after neoadjuv

BRAF V600E-Mutated Metastatic Colorectal Cancer: Practical Management in the Post-BREAKWATER Era.

INTRODUCTION: BRAF V600E-mutated metastatic colorectal cancer (mCRC) is a biologically distinct subtype characterized by right-sided predominance, frequent overlap with serrated-pathway biology, and historically poor outcomes with conventional chemotherapy. Therapeutic progress has been driven by the recognition that BRAF inhibition alone is inadequate in colorectal cancer because adaptive epidermal growth factor receptor (EGFR)-mediated feedback rapidly restores MAPK signaling. METHODS: This narrative review evaluates the clinical development and practical integration of BRAF-targeted therap

Engineered E. coli biosensor with dual-module sensing and signal amplification for non-invasive detection of fecal bile acid salts in colorectal cancer screening.

In light of the growing global burden of colorectal cancer (CRC), there is an urgent need for sensitive and non-invasive diagnostic tools. Here, we developed a novel whole-cell biosensor in Escherichia coli for the specific detection of fecal bile acid salts as biomarkers linked to CRC progression. Our platform integrated two modular sensing architectures: a synthetic hammerhead ribozyme switch tailored for primary bile acids (100-300 μM), and an engineered two-component system (VtrA/VtrC) from Vibrio parahaemolyticus that selectively responded to secondary bile acid salts such as deoxycholic

Engineering Small Extracellular Vesicles for Colon-Targeted Delivery: Microenvironment-Responsive Design, Therapeutic Mechanisms, and Clinical Translation.

Small extracellular vesicles (sEVs) are endogenous lipid-bilayer nanovesicles that combine cargo protection, biological membrane compatibility, and modifiable surface recognition. For colon-targeted delivery, their value depends on engineering them to withstand gastric acidity and proteolysis, negotiate the mucus barrier, and respond to colon-specific cues, including the distal gastrointestinal pH profile, microbiota-derived enzymes, inflammatory reactive oxygen species, and lesion-associated receptors such as integrins, CD44, and folate receptors. This review critically compares sEVs with con

Research Advances in Nanodrugs Targeting Lipid Metabolism Reprogramming for Colorectal Cancer Therapy.

Colorectal cancer (CRC) remains a major global health burden, with limited efficacy of conventional treatments due to tumor heterogeneity, drug resistance, and systemic toxicity. Lipid metabolism reprogramming has emerged as a hallmark of CRC progression, influencing tumor growth, metastasis, and therapy resistance. Concurrently, nanomedicine offers innovative platforms for targeted drug delivery to to inhibit tumor progression by intervening in the lipid metabolism, including modulation of lipid synthesis, uptake, storage, and cholesterol metabolism. This review provides an integrated perspec

Statin Initiation and Mortality After Colorectal Cancer Treatment.

IMPORTANCE: Observational studies have suggested improved survival among patients with colorectal cancer (CRC) who start statin therapy after diagnosis, but such studies are vulnerable to time-related biases and informative censoring. OBJECTIVE: To estimate the association between statin initiation after CRC treatment and CRC-specific and all-cause mortality using target trial emulation. DESIGN, SETTING, AND PARTICIPANTS: This nationwide cohort study used Korean National Health Insurance Service claims data from January 1, 2005, through December 31, 2015. Inhibition of PFKFB3 in Macrophages Has a Dual Effect on Tumor-Regulating Lipid Metabolism.

Colorectal cancer is the third most common cancer worldwide, making lymph node recovery critical for treatment decisions and prognosis. Within the colorectal tumor microenvironment, the metabolic programming of tumor-associated macrophages (TAMs) can drive both pro- and anti-tumor responses, yet the specific glycolytic pathways governing their pro-metastatic conversion present promising therapeutic targets. This study investigated the role of glycolysis activating enzyme 6-phosphofructo-2-kinase/fructose-2,6-biphosphatase 3 (PFKFB3) in mediating TAMs metabolic polarization, and its potential a

Lentinula edodes as a Source of Bioactive Compounds with Therapeutical Potential in Intestinal Inflammation and Colorectal Cancer.

Inflammatory bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), is a rising global health issue. Chronic intestinal inflammation is an important risk factor for colorectal cancer (CRC). Despite significant progress in IBD and CRC treatment, numerous patients remain resistant to standard pharmacotherapy or experience severe side effects that prevent them from continuing treatment. There is evidence suggesting that bioactive substances in Lentinula edodes have immunomodulatory and anticancer properties. This fungus is currently classified as a functional food, consi

NRF2 as a ferroptosis gatekeeper in colorectal cancer: implications for therapy.

Colorectal cancer (CRC) is one of the primary cancer concerns for global health, with high incidence and mortality rates worldwide. The natural history of CRC is a complicated and multistep process characterized by genetic alterations with various biological features, including genomic instability, excessive cell proliferation, angiogenesis, and metastasis. This review focuses on the delicate interaction between ferroptosis, a recently unveiled form of iron-dependent cell death, the nuclear factor erythroid 2-related factor 2 (NRF2), an important transcriptional factor that controls the pharma

Determinants of healthy lifestyle behaviours in colorectal cancer survivors: a systematic review.

PURPOSE: Identifying and selecting determinants of health behaviours is an important step in the design of behaviour change interventions. Many colorectal cancer (CRC) survivors experience disease- and treatment-related complaints, which may make it difficult to implement behavioural changes. In this systematic review, we aimed to identify determinants of a healthy lifestyle, i.e. dietary behaviours and physical activity, in CRC survivors who finished treatment. METHODS: We searched Web of Science, PubMed and PsychINFO, to retrieve quantitative and qualitative studies on determinants of a hea

Hologenomic analysis of rectal mucus sampling for detection of adenomatous polyps and colorectal cancer.

Colorectal cancer (CRC) is the fourth most common cancer and the third leading cause of cancer-related mortality worldwide, with incidence rising among younger populations. The significant clinical and economic burden highlights the need for minimally invasive technologies capable of detecting pre-malignant and early-stage disease. Although liquid biopsy approaches have advanced, they have not achieved sufficient performance for clinical adoption when compared with colonoscopy, the current diagnostic gold standard. CRC is a mucosal pathology, yet current diagnostic methods have not leveraged m

Clinical application prospects of traditional Chinese medicine as adjuvant therapy for metabolic reprogramming in colorectal cancer.

Colorectal cancer (CRC) is the second leading cause of cancer-related deaths globally and the third most commonly diagnosed malignancy, posing a major threat to public health. Clinical manifestations such as altered bowel habits (e.g., constipation, diarrhea, or pencil-thin stools), rectal bleeding, and abdominal pain or bloating may indicate CRC. A hallmark of CRC is metabolic reprogramming, which enables tumor cells to meet the bioenergetic and biosynthetic demands of rapid proliferation and survival. This reprogramming encompasses dysregulated glycolysis, amino acid metabolism, and lipid me
