Xanthoma disseminatum: an unusual cause of facial and limb deformity.

A rare case of xanthoma disseminatum producing deformity of the head, neck, and extremities is reported. This pathologically benign disease, considered to represent one of the histiocyte proliferative disorders, is manifested by numerous cutaneous xanthomas primarily in flexion areas. Progressive enlargement of these lesions accompanied by marked fibrosis has produced the severe cosmetic and functional deformities described in this patient. Treatment in this case involved excision of xanthomatous contraction bands with split-thickness skin grafting, followed by intensive physical therapy.

Treatment of old introclable lesions of the extensors of the middle finger by resection arthroplasty with implant.

Traumatic boutonniere deformity may become fixed and cannot be reduced when the joint cartilage become altered. We propose a resection arthroplasty with a Swanson's silicone implant as a valuable solution for this problem as it simplifies the joint and the extensor apparatus. We use a transverse approach through the extensor tendon which enables to restore a good balance between bone and tendon and seems to avoid some adhesions. Results of 17 operations with a minimum follow up of 12 months show that 40 degrees of motion have been recovered in an angle sector which corrects the deformity. The

[Dupuytren's disease in clinical practice].

The Authors have investigated the incidence of Dupuytren's disease in the field of internal medicine, they have observed a rather high rate in the old people, especially between 60 and 80 years, and point out the significance of metabolic and genetic factors in the pathogenesis and the development of the disease.

Management of adult poliomyelitis patients in the Solomon Islands.

An account is given of a realistic rehabilitation service for the treatment of crawling adult poliomyelitic patients in an area of the developing world. A classification of the crawling patterns encountered is presented. The simple methods by which rehabilitation was carried out for 120 patients are described. Methods consisted of surgery to straighten and align the lower limbs when necessary, the fitting of inexpensive appliances, and the teaching of crutch walking. At long-term review, 80 percent of the patients had responded satisfactorily to treatment. Rehabilitation in an economically and

Electromyography before and after surgery for hip deformity in children with cerebral palsy. A comparison of clinical and electromyographic findings.

Twenty-three ambulatory children with spastic diplegic cerebral palsy were evaluated clinically and by electromyography before and after hip-muscle surgery. The stretch tests originally designed to distinguish specific muscle tightness and spasticity were found to be non-specific when tested by electromyography. Ambulatory electromyograms using needle electrodes and telemetry generally showed decreased activity in the released muscles and, on occasion, changes in activity in muscles not operated on. These unanticipated changes after release may explain some of the unpredictability of results o

Salvaging the Constricted or Lost Umbilicus After Abdominoplasty With Early Mercedes Release, Silver Nitrate, and Foam Earplug Stenting: Stairway to Heaven.

Umbilical stenosis is an underrecognized complication following abdominoplasty, driven by cicatricial contracture, myofibroblast-mediated centripetal tension, and stalk ischemia. As the central aesthetic focal point of the abdomen, umbilical malposition or deformity is among the most frequent contributors to postoperative dissatisfaction. We present a 40-year-old female who developed umbilical stenosis 9 months following abdominoplasty with native stalk preservation, managed successfully with a minimally invasive outpatient protocol. A Mercedes-pattern incision was employed to release circumfe…

Lateral Intercostal Artery Perforator Flap for Axillary Breast Reconstruction in a Previously Irradiated and Dissected Field.

Reconstruction of axillary defects after oncologic resection is particularly challenging in patients with prior axillary dissection and radi … lla, and the donor site closed primarily. The patient achieved complete healing without necrosis or contracture and maintained full shoulder mobility after adjuvant chemoradiation. The LICAP flap offers a safe, reliable option for axillary reconstruction in previously treated fields.
