A multi-country study to co-design and evaluate digital educational resources to support conversations about ending fertility treatment.

STUDY QUESTION: How can educational resources be feasibly co-designed and used to support conversations between staff and patients about ending fertility treatment? SUMMARY ANSWER: Co-design workshops allow for the development of educational resources that account for all stakeholders' perspectives and are considered sensitive, informative, and helpful to support end-of-treatment conversations, but staff and patients have different views about how these can be used within the treatment pathway. WHAT IS KNOWN ALREADY: Ending treatment without children is a common outcome but seldom discussed

The TOMMY trial: a comparison of TOMosynthesis with digital MammographY in the UK NHS Breast Screening Programme--a multicentre retrospective reading study comparing the diagnostic performance of digital breast tomosynthesis and digital mammography with digital mammography alone.

BACKGROUND: Digital breast tomosynthesis (DBT) is a three-dimensional mammography technique with the potential to improve accuracy by improving differentiation between malignant and non-malignant lesions. OBJECTIVES: The objectives of the study were to compare the diagnostic accuracy of DBT in conjunction with two-dimensional (2D) mammography or synthetic 2D mammography, against standard 2D mammography and to determine if DBT improves the accuracy of detection of different types of lesions. STUDY POPULATION: Women (aged 47-73 years) recalled for further assessment after routine breast screen

Adverse drug events in cost-effectiveness models of pharmacological interventions for diabetes, diabetic retinopathy, and diabetic macular edema: a scoping review.

OBJECTIVE: The objective of this review was to examine the role of adverse drug events (ADEs) caused by pharmacological interventions in cost-effectiveness models for diabetes mellitus, diabetic retinopathy, and diabetic macular edema. INTRODUCTION: Guidelines for economic evaluation recognize the importance of including ADEs in the analysis, but in practice, consideration of ADEs in cost-effectiveness models seem to be vague. Inadequate inclusion of these harmful outcomes affects the reliability of the results, and the information provided by economic evaluation could be misleading. Reviewin

Screening mammography for women aged 40 to 49 years at average risk for breast cancer: an evidence-based analysis.

OBJECTIVE: The aim of this review was to determine the effectiveness of screening mammography in women aged 40 to 49 years at average risk for breast cancer. CLINICAL NEED: The effectiveness of screening mammography in women aged over 50 years has been established, yet the issue of screening in women aged 40 to 49 years is still unsettled. The Canadian Task Force of Preventive Services, which sets guidelines for screening mammography for all provinces, supports neither the inclusion nor the exclusion of this screening procedure for 40- to 49-year-old women from the periodic health examination

Meta-analysis of variable-temperature PCR technique performance for diagnosising Schistosoma japonicum infections in humans in endemic areas.

BACKGROUND: As China is moving onto schistosomiasis elimination/eradication, diagnostic methods with both high sensitivity and specificity for Schistosoma japonicum infections in humans are urgently needed. Microscopic identification of eggs in stool is proven to have poor sensitivity in low endemic regions, and antibody tests are unable to distinguish between current and previous infections. Polymerase chain reaction (PCR) technologies for the detection of parasite DNA have been theoretically assumed to show high diagnostic sensitivity and specificity. However, the reported performance of PCR

Social Inequity and Structural Barriers to Completion of Ecological Momentary Assessments for Young Men Who Have Sex With Men and Trans Women Living With HIV in San Francisco.

BACKGROUND: Ecological momentary assessments (EMAs) administered via text messaging facilitate real-time data collection. With widespread cell phone access, EMAs are becoming more available to even the most disenfranchised communities, such as those living with HIV. However, structural barriers disproportionately burden young men who have sex with men (MSM) and trans women (TW) living with HIV and threaten participation in HIV research. OBJECTIVE: We aim to identify structural barriers to completing EMA text surveys nested within a digital HIV care intervention for young MSM and TW living wit

Long-term results of radiotherapy alone for carcinoma of the vulva.

BACKGROUND: Approximately 11,000 people die in Scotland each year as a result of smoking-related causes. Quitting smoking is relatively easy; maintaining a quit attempt is a very difficult task with success rates for unaided quit attempts stubbornly remaining in the single digits. Pharmaceutical treatment can improve these rates by lowering the overall reward factor of nicotine. However, these and related nicotine replacement therapies do not operate on, or address, the spatial and contextual aspects of smoking behaviour. With the ubiquity of smartphones that can log spatial, quantitative and

SmartWand: Validating a Novel Device for Remote Pelvic Floor Physical Therapy.

High-tone pelvic floor dysfunction (HTPFD) affects a substantial proportion of women with chronic pelvic pain and is characterized by hypertonic musculature, myofascial tenderness, dyspareunia, and urinary and defecatory symptoms. Although pelvic floor physical therapy (PFPT) is the first-line treatment, access is severely limited: only 2.47% of U.S. zip codes have even one trained pelvic floor physical therapist, and completion rates at some referral centers are as low as 20%. Existing telehealth and home biofeedback tools support muscle-strengthening exercises but cannot guide patients to th

Automating Quality Measures for Heart Failure Using Natural Language Processing: A Descriptive Study in the Department of Veterans Affairs.

BACKGROUND: Some 222 million women worldwide have unmet needs for contraception; they want to avoid pregnancy, but are not using a contraceptive method, primarily because of concerns about side effects associated with most available methods. Expanding contraceptive options-particularly fertility awareness options that provide women with information about which days during their menstrual cycles they are likely to become pregnant if they have unprotected intercourse-has the potential to reduce unmet need. Making these methods available to women through their mobile phones can facilitate access.

Artificial intelligence in the biomechanics of knee osteoarthritis: biomechanical phenotyping, risk stratification, diagnosis, treatment and clinical translation-a narrative review.

Structural degeneration, pain and functional limitation in knee osteoarthritis (KOA) often diverge, which suggests that radiographic grading alone cannot explain the heterogeneous clinical course of the disease. Lower-limb alignment, gait strategy, muscle function, joint contact forces and real-world activity exposure jointly shape KOA onset, progression and treatment response. Artificial intelligence (AI) methods provide tools for integrating multimodal imaging, gait, wearable-sensor and clinical data, and can be combined with finite element analysis (FEA) and other computational biomechanica
