Uncomplicated diverticular disease is usually asymptomatic. When abdominal pain or discomfort related to defecation, altered bowel habit, and flatulence occur, they are likely a result of a coexistent irritable bowel. Nonetheless, diverticula are subject to serious complications. Diverticular hemorrhage may be massive and require emergency angiography and segmental resection. Intramural gastric diverticula are true diverticula, entirely contained within the wall of the stomach without deformity of the serosa. All of the 13 known reported cases are at the greater curvature of the distal antrum. This is true of our three cases as well. The unique location and characteristic change in shape with peristalsis distinguish them from pathological entities.

Sigmoid diverticula are pseudodiverticula, protrusions of the mucosa between the muscularis, so that they pouch out ball shaped under the serosa or in the fatty connective tissue of the sigmoid region. 55 patients suffering from diverticulosis of the sigmoid colon underwent surgery, 32 of them in a state of emergency (peritonitis, ileus); mortality rate 22%. In 28 patients a colostomy was performed, which later on could be removed to restore the continuity of the colon. The therapy of choice in the early stage of diverticulosis is resection of the sigmoid colon performed as a ono-stage procedure (mortality rate les than 1%).

The effects of an increase in dietary fiber include: (a) More frequent stools, (b) more voluminous stools, (c) an alteration in the fecal flora, (d) an increase in fecal sterol excretion, and, it appears likely, (e) a reduction in intraluminal pressures in the sigmoid colon.
