Therapeutic approach in a case of Pearson's syndrome.

Mitochondrial cytopathy is a multisystemic disease that requires different pharmacological and specialist approaches; although most therapies are usually of scarce effectiveness. We describe a clinical management of a very young girl with Pearson's syndrome that developed the symptoms of Kearns-Sayre syndrome. Many of symptoms were temporarily improved by the replacement therapy with hydrocortisone introduced to treat the partial adrenal insufficiency. During her life, she showed an ample clinical spectrum of symptoms because of multiple organs involvements: firstly bone marrow and, thereafter

[Sudden sensorineural hearing loss during pregnancy].

Sudden sensorineural hearing loss is defined as sudden, idiopathic, usually unilateral deafness developed at most within 72 hours in previously healthy person. This paper presents very rarely occurring case of sudden sensorineural hearing loss in 23-year-old healthy nulliparous women during an uncomplicated pregnancy. The possibility of therapy of that sudden deafness in pregnant women is restricted by reason of unknown cause of disease. It has been hypothesized that hearing loss may be associated with increased activation of both blood coagulation and fibrinolysis ('hypercoagulable state') wh

Nonsyndromic isolated unilateral cochlear nerve aplasia without narrow internal auditorymeatus: a previously overlooked cause of unilateral profound deafness in childhood.

OBJECTIVES: Juvenile or adolescent unilateral profound sensorineural deafness (worldwide prevalence, 0.1% to 0.2%) has been attributed to postnatal viral infection, sudden deafness, prenatal and perinatal problems including maternal rubella and viral infections, congenital innerear anomalies, and other factors. Herein, 2 cases are reported and another potentially important cause of unilateral profound hearing loss is proposed. METHODS: Two nonsyndromic cases of a presently "very rare" cause of pediatric unilateral deafness are presented as a retrospective case study. RESULTS: The 2 patients

Intermittent pressure therapy of intractable Ménière's disease using the Meniett device: a preliminary report.

HYPOTHESIS: Treatment with the Meniett device, which applies intermittent micropressure pulses to the inner ear through a tympanostomy tube, is effective in controlling vertigo in people with intractable Ménière's disease. STUDY DESIGN: Short-term, preliminary descriptive report. METHODS: Ten patients with intractable vertigo despite adequate medical therapy elected to use the Meniett device. After placement of a standard tympanostomy tube, the patient self-administers the Meniett device three times daily. RESULTS: The follow-up ranged from 3 to 11 months with an average of 8 months. All 10

Sensorineural hearing loss as the initial manifestation of polyarteritis nodosa.

A 74-year-old male was referred for the sudden onset of bilateral sudden deafness. The patient had no history of any disease or trauma to the head. Pure tone audiometry revealed bilateral moderate, to severe, sensorineural hearing loss. Auditory brain stem responses (ABRs) showed normal peak and interpeak latencies. These audiological findings suggested that his hearing loss could be attributed to inner ear lesions. However, we felt an alternative explanation for this sudden deafness was likely to exist because the patient also had a month-long fever of unknown origin (FUO) and weight loss of

New approach for cochlear implantation: cochleostomy through the middle fossa.

The middle fossa approach was used in 11 patients with profound bilateral hearing loss for insertion of a cochlear implant. Fibroadhesive otitis media (n = 1), bilateral cavity radical mastoidectomy (n = 1), autoimmune inner ear disease (n = 2), previous cranial trauma (n = 1), genetic prelingual deafness (n = 5), and otosclerosis (n = 1) were the causes of deafness. A cochleostomy was performed on the most superficial part of the basal turn, and the electrode array was inserted up to the cochlear apex. Speech perception tests (1-9 months after cochlear implant activation) yielded better resul

Hearing loss related to ototoxicity in children with cancer.

Children undergoing treatment for cancer often receive agents that put them at risk for ototoxicity. Platinum-based chemotherapy, aminoglycoside antibiotics, loop diuretics, and radiotherapy are all capable of inducing inner ear damage, which may result in significant sensorineural hearing loss. Frequent audiological monitoring is necessary for the early detection of changes in hearing thresholds. Age-appropriate modification of audiological testing is essential to obtain accurate results and provide maximum comfort for pediatric patients. When hearing loss is identified promptly, consideratio

Improved detection of germline mutations in the von Hippel-Lindau disease tumor suppressor gene.

von Hippel-Lindau disease (VHL) is an inherited neoplastic disorder characterized by the development of tumors in the eyes, brain, spinal cord, inner ear, adrenal gland, pancreas, kidney, and epididymis. The VHL tumor suppressor gene was identified in 1993. Initial studies reported the detection of germline mutations in the VHL gene in 39-75% of VHL families. We used tests that detect different types of mutations to improve the frequency of detection of germline mutations in VHL families. The methods included quantitative Southern blotting to detect deletions of the entire VHL gene, Southern b

[Physiopathologic consequences of underwater diving and medical management of divers].

Underwater diving is a very closely medically managed activity. Performing it, the human organism is under the physical laws of pressure and following consequences. The expiratory flows are significatively reduced, enhancing the risk of alveolar hypoventilation at exertion, the central nervous system is the privileged target during inopportune tissue degassing related accidents (leaving 20% of sequellae), barotraumatic injuries threaten middle and inner ear or lung (pulmonary barotrauma is the most severe accident), the toxicity of gas under pressure (i.e. oxygen, nitrogen) exposes to specific

Immunologic alterations in patients with sensorineural hearing disorders.

The autoimmune etiology suspected for some forms of hearing loss, supported by the clinical efficacy of steroid therapy, is thought to involve immune complexes, autoantibodies directed to the inner ear and/or cellular effectors. We report a study performed in 57 individuals with sudden deafness (n = 17, group 1) or progressive sensorineural hearing impairment (n = 40, group 2). A severe depletion in CD3+ and CD4+ peripheral lymphocytes was observed in group 1 and a marked decrease of CD8+ cells levels was observed in both groups. Group 2 patients frequently had anti-nuclear and anti-thyroid an

[Hearing disorders in patients with adhesive otitis media].

This paper presents the results of audiological examinations of 76 patients, aged 18 to 60 years, with adhesive nonperforative otitis media. The hearing function was measured in terms of threshold tonal audiometry, ultrasound hearing sensitivity, lower limit of sound frequency perception, discomfortable loudness, dynamic range of the hearing field, speech intelligibility in the masking noisy environment. In 30% of cases hypoacusis accompanying adhesive otitis media showed symptoms that were similar to those of hypoacusis resulting from inner ear pathologies (descending audiometric curves with

Van Gogh had Menière's disease and not epilepsy.

We intend to correct the historical error that Vincent Van Gogh's medical problems resulted from epilepsy plus madness, a diagnosis made during his life but for which no rigid criteria are apparent. Review of 796 personal letters to family and friends written between 1884 and his suicide in 1890 reveals a man constantly in control of his reason and suffering from severe repeated attacks of disabling vertigo, not a seizure disorder. His own diagnosis of epilepsy was made from the written diagnosis by Dr Peyron, the physician at the asylum of St Remy (France), wherein on May 9, 1889, Van Gogh vo

[Callosal disconnection syndrome caused by left hemisphere infarction].

A 49-year old right-handed taxi-driver experienced right upper limb weakness and global aphasia following internal carotid artery occlusion. Five months later, aphasia and hemiparesis had resolved but he complained of difficulties in his daily activities, termed "inner conflict". Specific testings disclosed typical features of a callosal syndrome: left unilateral ideomotor apraxia, left hand agraphia, left tactile dysnomia, right hand constructional difficulties, and left ear extinction on dichotic listening. Naming and recognition of tachistoscopically presented images or words was similar to

Usher syndrome: an otoneurologic study.

Usher syndrome is an autosomal recessive disorder characterized by severe hearing loss or deafness and retinitis pigmentosa. Eleven families with 25 affected members were studied. The test battery included genetic studies, clinical examination, audiological, ophthalmologic, and otoneurological tests, and magnetic resonance imaging. Sixteen affected persons had profound hearing loss or were considered anacusic, with absent bilateral vestibular responses. These patients had varying degrees of retinitis pigmentosa. These 16 patients were considered to have type I Usher syndrome. Nine persons were

Effects of aging on normal hearing loss and noise-induced threshold shift in albino and pigmented guinea pigs.

In a previous investigation into noise-induced hearing loss by comparing 2-month-old albino with pigmented guinea pigs, albinos displayed significantly greater shifts in cochlear microphonic (CM) threshold and less recovery than the pigmented animals 7 days after noise exposure. The present study compared the responses of 14-month-old albino and pigmented guinea pigs to the same noise parameters used previously. Thresholds for the first detectable elicitation of CM for three pure tones were recorded prior to, at 90 min and at 7 days after a 45-min exposure to 126 dB broadband noise. Before exp

Auditory brainstem responses in young children with Down's syndrome.

Auditory brainstem evoked responses (ABRs) were studied in 37 Japanese infants and children with Down's syndrome to determine the level of the lesion causing their unresponsiveness to sound stimuli. ABR thresholds, peak latencies of waves I and the peak interval latency of wave I-V were measured. For comparison with ABR thresholds, behavioral audiometry was performed. Thirty-two percent of the patients (12 cases) with Down's syndrome showed no responses by ABR as well as behavioral audiometry. The other ABR abnormalities with respect to age-matched controls were classified into 6 types: shorte

Cochlear Functional Changes and CGRP Modulation in a Chronic Nitroglycerin Migraine-Like Rat Model.

Auditory and vestibular symptoms in vestibular migraine, cochlear migraine, and Ménière's disease overlap, yet mechanisms linking migraine biology to inner ear dysfunction remain unclear. Calcitonin gene-related peptide (CGRP) is a key mediator of trigeminovascular activation and a major therapeutic target in migraine, but its contribution to cochlear dysfunction is not well defined. Using a repeated nitroglycerin (NTG) paradigm in male Wistar rats, we investigated whether a systemic migraine-like state affects cochlear function and whether these effects are dependent on CGRP. Rats received sa

Neomycin-induced deafness in neonatal mice.

BACKGROUND: Hearing impairment is a rising public health issue, and current therapeutics fail to restore normal auditory sensation. Animal models are essential to a better understanding of the pathophysiology of deafness and developing therapeutics to restore hearing. NEW METHODS: Wild-type CBA/CaJ neonatal mice P2-5 were used in this study. Neomycin suspension (500 nl of 50 or 100 mg/ml) was micro-injected into the endolymphatic space. Cochlear morphology was examined 3 and 7 days after injection; hair cell (HC) loss, supporting cell morphology, and neurite denervation pattern were assessed

Efferent feedback slows cochlear aging.

The inner ear receives two types of efferent feedback from the brainstem: one pathway provides gain control on outer hair cells' contribution to cochlear amplification, and the other modulates the excitability of the cochlear nerve. Although efferent feedback can protect hair cells from acoustic injury and thereby minimize noise-induced permanent threshold shifts, most prior studies focused on high-intensity exposures (>100 dB SPL). Here, we show that efferents are essential for long-term maintenance of cochlear function in mice aged 1 year post-de-efferentation without purposeful acoustic ove

Quantitative Temporal Bone Analysis of Stria Vascularis, Spiral Ganglion, and Vestibular Ganglion Cells in Sickle Cell Disease at Young Ages.

OBJECTIVES: Sensorineural hearing loss (SNHL) and vestibular symptoms in sickle cell disease (SCD) may result from hypoxic damage of inner ear structures, including the stria vascularis (SV), spiral ganglion cells (SGC), and vestibular ganglion neurons (VGN). SV cross-sectional area, SGC counts, and VGN counts were compared between SCD and control groups and correlated with temporal bone histopathology and clinical data. STUDY DESIGN: Hematoxylin and eosin-stained slides from 36 temporal bones were analyzed, including both ears from 6 SCD cases and 12 controls matched for age, sex, and race.
