Chronic Medial Epicondyle Avulsion: Technique of Fragment Excision and Ligament Reconstruction With Internal Brace Augmentation.

Medial epicondyle fracture nonunions of the elbow may lead to symptomatic instability in the high-demand or overhead athlete. These injuries are challenging to treat surgically because of the small residual bony fragment, the scarred and shortened chronically injured ulnar collateral ligament (UCL), which prevents it from being mobilized and reduced to its native position. To date, most described methods aim at reducing the displaced fragment and achieving union with the humerus. This usually can only be accomplished by releasing of the scarred UCL to mobilize the fragment. The scarred and att…

Distal defect of the humerus, a possible normal variant: a case report.

BACKGROUND: Many normal variants of bones on plain radiographs have been reported. CASE PRESENTATION: In the current report, a 14-year-old Asian girl noticed an occasional slight elbow pain. She had no traumatic episode. Plain radiographs showed a well-defined osteolytic lesion with a sclerotic rim, which was continuous with the normal subarticular bone in the distal humerus. Magnetic resonance imaging revealed that the defect area seen on the plain radiograph showed low-signal to iso-signal intensity on T1-weighted images and slightly high-signal intensity on T2-weighted fat suppression imag…

Risk factors for dislocation of the ulnar nerve after simple decompression for cubital tunnel syndrome.

Anterior dislocation of the ulnar nerve is occasionally encountered after simple decompression of the nerve for treatment of cubital tunnel syndrome. The purpose of this study was to determine whether the incidence of dislocation of the nerve following simple decompression of the nerve is correlated with the patient's preoperative characteristics and/or elbow morphology. We studied 51 patients with cubital tunnel syndrome who underwent surgery at our institution. Intraoperatively, we simulated dislocation of the nerve after simple decompression by flexing the elbow after releasing the nerve in…

Percutaneous K-wire buttress technique for displaced radial neck fracture.

Radial neck fractures are uncommon injuries in adults and more often found in children, where they account for 5-8.5% of elbow injuries. It is generally agreed that an angulation of less than 30 degrees is acceptable. However, anything greater than this should be improved in an attempt to restore normal anatomy and maximize the range of movement. We describe our management of a radial neck fracture in a young lady which was significantly angulated, resulting in a restriction of movements. Attempts at a closed reduction failed and hence we proceeded with percutaneous reduction and buttressing w…

Transient osteochondrosis of the capitellum in a child after humeral supracondylar fracture: a case report.

Supracondylar fracture of the humerus is the most frequently encountered fracture around the elbow in children. Avascular necrosis is an uncommon complication after supracondylar fracture, but fishtail deformity after avascular necrosis of the trochlea is well known. However, transient osteochondrosis of the capitellum after a supracondylar fracture has not been previously reported in the English literature except 1 case abstract. The authors report their experience of transient osteochondrosis of the capitellum in a 3-year-old boy after the closed reduction and percutaneous pinning for a disp…

"Tenomalacia": a new sonographic sign of tendinopathy?

The purpose of this study was to assess tendon compressibility with sonography in extensor tendinopathy and in asymptomatic extensor tendons of the elbow. Sonography of both elbows was performed in eight patients with a clinical diagnosis of unilateral lateral epicondylitis. Tendons were assessed for compressibility by measuring their thickness before and after compression with the transducer. The same manoeuvre was performed while tendon vascularity was assessed with colour Doppler. All eight cases showed increased compressibility of the common extensor tendon on the painful side compared to …

Early mobilization using dynamic splinting with acute triceps tendon avulsion.

Avulsion of the triceps tendon is rare, and no previous report of a completed dynamic splinting rehabilitation plan was found in the literature. A case of a military member and power weightlifter who sustained a triceps tendon avulsion and repair is presented with a six-month follow-up. The rehabilitation program included early active elbow flexion and passive extension using a dynamic elbow splint. The Disabilities of the Arm, Shoulder and Hand (DASH) 1-3 questionnaire was used to monitor the patient's use of his right dominant arm while in the dynamic splint. The DASH scores indicated that h…

Medial epicondylitis.

Medial epicondylitis of the elbow involves pathologic alteration in the musculotendinous origins at the medial epicondyle. Although commonly referred to as "golfer's elbow", the condition may in fact be caused by a variety of sports and occupational activities. Accurate diagnosis requires a thorough understanding of the anatomic, epidemiologic, and pathophysiologic factors. Nonoperative treatment involves rest, ice, nonsteroidal anti-inflammatory agents, and possibly corticosteroid injection followed by guided rehabilitation and return to sport. Operative treatment is indicated for debilitatin…

The treatment of selected fractures of the humeral shaft with the True-Flex nail.

The True-Flex nail was used in 23 selected non-pathological and eight pathological fractures/lesions of the humeral shaft. The overall fracture union rate was 69.5 per cent and the patients achieved a good range of movement of the shoulder and elbow. Nailing did not lead to union of established non-unions or fractures which were previously treated unsuccessfully by surgery despite bone grafting. All patients with pathological fractures/lesions regained good function of the arm and a good range of movement of the shoulder and the elbow. In one case the nail migrated proximally and impinged on t…

Arc-aggregation: a new method of range of motion analysis in hemophilia.

Arc-aggregation is a new graphic method of analysis of progressive loss of range of motion that can be applied to any diarthrodial joint undergoing a specific degenerative process. It is based on the premise that loss of motion (LOM) for a specific condition occurs in a predictable way or pattern. Using this method to analyze a population of 48 hemophilic patients with various stages of joint involvement, we studied 95 knees, 93 ankles, and 83 elbows for a total of 271 joints. We found a predictable pattern of progressive loss of range of motion for each joint. To detect the early onset of joi…

Lateral epicondylitis: Current concepts.

BACKGROUND: Lateral epicondylitis, more commonly referred to as 'tennis elbow', is a common condition seen in general practice. It effects approximately 4-7 per 1000 individuals. Despite this, the aetiology and pathophysiology remain poorly understood. Often presenting as lateral elbow pain, the differential diagnosis includes entrapment syndromes, cervical radiculopathy, osseous pathology and inflammatory conditions. Though in 90% of cases the condition is self-limiting, persistent symptoms can be difficult to manage. OBJECTIVE: In this article, a review of recent English-language journal ar…

Lateral epicondylitis: clinical classification and proposal for treatment.

The authors report their experience in a retrospective study on a sample year (1997) of cases (77 in number) of lateral epicondylitis treated non-surgically at the Rizzoli Orthopaedic Institute. The results for the 63 patients reviewed are very disappointing (healing only in 36.5% of cases). In their opinion, this may be attributed to the lack of an accurate clinical definition of the pathology, and an inadequate treatment protocol. They conclude with the proposal to classify 3 clinical forms for epicondylitis (acute, chronic recurrent, and chronic persistent) and to follow a corresponding tre

Septic olecranon bursitis: recognition and treatment.

BACKGROUND: The superficial location of the olecranon bursa places it at high risk for injury, possibly leading to the entry of bacteria into the bursal sac. Early differentiation between septic and nonseptic olecranon bursitis is paramount to direct therapy, to hasten recovery, and to prevent chronic inflammation. METHODS: A literature review was performed using MEDLINE files from 1967 to the present. Additional references from the bibliographies of these were also utilized. RESULTS AND CONCLUSIONS: Olecranon bursitis is a common condition that requires the treating physician to be aware of
