An in-vitro biomechanical comparison of annular ligament repair and reconstructions to restore radial head stability in anterior Monteggia fractures.

BACKGROUND: Persistent or recurrent instability of the radial head (RH) remains a challenge in treating anterior Monteggia fractures despite anatomic restoration of the ulna. RH instability may be caused by the pull of the biceps muscle with rupture of the annular ligament and other soft tissue stabilizers of the proximal radius. Currently, the optimal method to stabilize the proximal radius is unknown. The purpose of this study was to compare annular ligament repair with three different ligament reconstructions in restoring anterior RH stability. METHODS: Eight cadaveric upper extremities we

Clinical and radiographic outcome of open reduction and internal fixation of 3- and 4-part proximal humerus fractures augmented with impaction bone grafting and a magnesium-based bone filler.

BACKGROUND: Open reduction and internal fixation (ORIF) of proximal humerus fractures (PHFs) is a challenging operation with high rates of loss of reduction, screw cut out, avascular necrosis, and subsequent unplanned reoperation. Augmentation of the repair with synthetic bone fillers and other alternatives has shown promise in decreasing adverse outcomes. Our aim is to report radiographic and clinical outcome of impaction grafting with cancellous allograft chips and injection of magnesium-based bone filler for ORIF augmentation of PHFs. METHODS: All patients that underwent ORIF for a 3- or 4

Mid-term outcomes of microfracture for the treatment of focal, full-thickness cartilage defects isolated to the humeral head.

BACKGROUND: While microfracture has been shown to be an effective treatment for chondral lesions in the knee, evidence to support its use for chondral defects in the shoulder is limited to short-term outcomes studies. The purpose of this study is to determine if microfracture provides pain relief and improved shoulder function in patients with isolated focal chondral defects of the humeral head at a minimum 5-year follow-up. METHODS: Patients who underwent microfracture procedure for isolated focal chondral defects of the humeral head with a minimum follow-up of 5 years between 02/2006 and 08

Does coracoclavicular augmentation additional to hook plate fixation provide benefits in acute unstable acromioclavicular dislocation? A meta-analysis.

BACKGROUND: Acromioclavicular joint (ACJ) dislocation is a common shoulder injury. In treating acute unstable ACJ dislocation, a hook plate (HP) is a straightforward and popular option for ensuring proper reduction and rigid fixation while promoting AC and coracoclavicular (CC) ligament healing. Surgeons typically remove the HP to prevent subacromial impingement and acromial osteolysis; however, concerns about redislocation after implant removal remain. Therefore, additional CC augmentation may be helpful in combination with HP fixation. The aim of this meta-analysis is to compare the outcomes

Closed reduction and percutaneous pinning vs open reduction and internal fixation in pediatric lateral condylar humerus fractures displaced by > 4 mm: an observational cross-sectional study.

BACKGROUND: Although open reduction and internal fixation (ORIF) is recommended for lateral condylar humerus fractures (LCHFs) displaced by > 4 mm, several studies have reported the use of closed reduction and percutaneous pinning (CRPP) to treat LCHFs with significant displacement. However, little is known about the clinical differences between these two surgical techniques. This study aimed to compare the therapeutic effects of CRPP and ORIF in treating LCHFs displaced by > 4 mm. METHODS: We retrospectively reviewed pediatric LCHFs displaced by > 4 mm treated with either CRPP or ORIF at our

Results of Arthroscopic Bankart Repair for Anterior-Inferior Shoulder Instability at 13-Year Follow-up.

BACKGROUND: Anterior-inferior shoulder instability is a common injury in young patients, particularly those practicing overhead-throwing sports. Long-term results after open procedures are well studied and evaluated. However, the long-term results after arthroscopic repair and risk factors of recurrence require further assessment. HYPOTHESIS: Arthroscopic Bankart repair results are comparable with those of open repair as described in the literature. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A total of 180 shoulders with anterior-inferior shoulder instability were stabilized

A new evaluation system to predict the sequelae of late obstetric brachial plexus palsy.

Obstetric brachial plexus palsy has two distinct categories: (1) early obstetric brachial plexus palsy (or obstetric brachial plexus palsy in an infant) and (2) late obstetric brachial plexus palsy with deformity (or obstetric brachial plexus palsy in a child). Both early and late obstetric brachial plexus palsy lack a uniform evaluation system, and this makes correlation between them difficult. Clinical evaluation of obstetric brachial plexus palsy in infants is difficult, but in children it is easier. Here, we utilized a new evaluation system, called "Score of 10," to evaluate 121 late obste

Management of Seymour (Distal Phalangeal) Fractures with Nail Bed Involvement.

BACKGROUND: Seymour fractures are a unique type of pediatric distal phalangeal fracture that can be easily misdiagnosed as a simple nail-bed injury or mallet finger1-3. Because of this potential for misdiagnosis, clear communication with consulting physicians regarding physical examination findings, such as nail plate avulsion and radiographic findings, is necessary. Seymour fractures involve the open physis and the germinal matrix, which become interposed in the fracture site, increasing the likelihood of infection. Open fractures are often displaced as a result of the distracting forces of t

Integrated network pharmacology and AlphaFold modeling reveal ESR1 as a key target of Huanglian Jiedu decoction for ameliorating sepsis-induced coagulopathy.

ETHNOPHARMACOLOGICAL RELEVANCE: Huanglian Jiedu Decoction (HLJDD) was initially documented in the Elbow Reserve Emergency Prescription, an ancient Chinese medical text created in the Eastern Jin Dynasty. This decoction is composed of four herbs: Huanglian (HL, Coptis chinensis Franch.), Zhizi (ZZ, Gardenia jasminoides Ellis), Huangqin (HQ, Scutellaria baicalensis Georgi), and Huangbo (HB, Phellodendron chinense Schneid.). Modern pharmacological research shows that HLJDD exerts multiple therapeutic effects, including antibacterial and anti-inflammatory properties, improvement of coagulation fun

Influence of Stride Length on Pelvic-Trunk Separation and Proximal Plyometrics in Baseball Pitching.

Pelvis and trunk counter-rotation are key factors known to effect throwing arm kinematics in baseball pitching, where energy or momentum is transferred from the lower extremities through to the trunk during the pitching cycle. The purpose of this study was to retrospectively analyze previously recorded motion capture data of 19 skilled competitive pitchers to test the a priori hypothesis whether different stride lengths affect transverse pelvis and trunk biomechanics. A blinded randomized crossover design was used where pitchers threw two simulated games at ±25% from desired stride length (DSL

The use of cold therapy unit in the postoperative period influences pain and narcotic use following arthroscopic rotator cuff repair.

BACKGROUND: Opioid overdose is a leading cause of injury-related death in the United States, prompting calls for reform in surgical prescription practices. Cold therapy units (CTUs) have been widely accepted as an adjunct to pharmacologic therapy for pain management, but these devices are not consistently reimbursed by insurance companies and have not been extensively studied in arthroscopic shoulder surgery. The purpose of our study is to assess the impact of a standardized CTU protocol on postoperative pain and opioid consumption following arthroscopic rotator cuff repair (ARCR), along with
