Rapid recurrence and malignant transformation of pilocytic astrocytoma in adult patients.

Pilocytic astrocytoma is a slow-growing, circumscribed glioma that most frequently occurs within the pediatric population. In general, surgical resection for pilocytic astrocytoma is thought to be curative with tumor recurrence or malignant transformation being relatively rare. However, there have been very few studies specifically looking at the prognosis for adult patients diagnosed with pilocytic astrocytoma. To evaluate the frequency of recurrence and malignant transformation of pilocytic astrocytoma in adults, we performed a retrospective analysis of all adult patients who underwent surgi

Wideband energy reflectance patterns in preoperative and post-operative otosclerotic ears.

The purpose of this study was to examine patterns of energy of reflectance (ER) in preoperative and post-operative conditions in otosclerotic ears. It was also within the scope of this paper to investigate whether the changes in ER pattern post-operatively correlate to the improvement in hearing as measured by pure-tone audiometry. ER was measured in 15 surgically confirmed otosclerotic ears (mean age: 44 years) before and after the stapes surgery. The most prominent change in ER pattern following the surgery in majority of the subjects was a sharp and deep drop in ER value between 700-1000 Hz

[Massive haemoptysis: management and treatment. What is the role of surgery?].

OBJECTIVE: To define the role of interventional radiology and surgery respectively, in the treatment of massive haemoptysis. GENERAL CONSIDERATIONS: For the management of massive haemoptysis in non-terminal pathologies an intensive care facility and a multi-disciplinary team are necessary. It is of paramount importance to identify rapidly the pulmonary or bronchial source of the bleeding. CT scanning and bronchoscopy are essential to localise the bleeding and determine its cause. Initial management. An attempt to control the initial bleeding to allow localisation of its origin and determine th

Regional Anaesthesia Approaches in Head and Neck Surgery: Current Evidence and Clinical Applications.

General approaches of head and neck surgery involve varied procedures with developing perioperative care and a renewed effort on regional anaesthesia (RA) for intraoperative and postoperative analgesia. Due to the rich innervation and focus on enhanced recovery after surgery (ERAS), RA techniques, particularly ultrasound-guided ones, are becoming popular as part of an opioid-sparing multimodal analgesic regimen. However, the evidence base is heterogeneous and synthesised clinical guidance is needed. This narrative review, performed according to the Scale for the Assessment of Narrative Review

A Case of Heterotopic Pregnancy Resulting in a Live Birth Through Laparoscopic Surgery.

Heterotopic pregnancy (HP) is a condition in which intrauterine and ectopic pregnancies occur simultaneously and is extremely rare. However, its incidence has markedly increased with the use of assisted reproductive technology (ART). HP can be life-threatening due to intraperitoneal bleeding; therefore, accurate diagnosis is crucial. The coexistence of intrauterine pregnancy complicates the detection of ectopic pregnancy, making HP one of the most challenging diagnoses in modern gynecological practice. Here, we describe a case in which HP was diagnosed through laparoscopic surgery, followed by

Stability Evaluation of Different Oblique Lumbar Interbody Fusion Constructs in Normal and Osteoporotic Condition - A Finite Element Based Study.

Introduction: In developed countries, the age structure of the population is currently undergoing an upward shift, resulting a decrease in general bone quality and surgical durability. Over the past decade, oblique lumbar interbody fusion (OLIF) has been globally accepted as a minimally invasive surgical technique. There are several stabilization options available for OLIF cage fixation such as self-anchored stand-alone (SSA), lateral plate-screw (LPS), and bilateral pedicle screw (BPS) systems. The constructs' stability are crucial for the immediate and long-term success of the surgery. The a

Assessment of risk factors for cerebral oxygen desaturation during neonatal and infant general anesthesia: an observational, prospective study.

BACKGROUND: Cerebral oxygen saturation (rSO2c) decrease from baseline greater than 20 % during infant cardiac surgery was associated with postoperative neurologic changes and neurodevelopmental impairment at 1 year of age. So far, there is no sufficient evidence to support the routine monitoring of rSO2c during general surgical procedures in children. We aimed to find out the frequency of cerebral desaturation 20 % or more from baseline and to identify possible predictors of change in cerebral oxygen saturation during neonatal and infant general surgery. METHODS: Forty-four infants up to 3 mo…
