PURPOSE: To assess the use of Tc-99m sestamibi to localize recurrent and metastatic parathyroid carcinoma.
METHODS: A patient with a history of parathyroid carcinoma that was resected 6 months before had high serum calcium and high serum parathyroid hormone levels. Tc-99m sestamibi imaging was performed to localize the recurrence.
RESULTS: Tc-99 sestamibi imaging detected the recurrence and the metastatic lymph nodes. These findings were confirmed surgically and pathologically.
CONCLUSION: Tc-99m sestamibi can be useful in diagnosing and localizing metastatic parathyroid carcinoma.

BACKGROUND: Primary operations for hyperparathyroidism performed by experienced surgeons has a success rate of about 95% without any preoperative localization. After unsuccessful cervicotomy, localization studies must be performed in order to define the surgical approach. BACKGROUND: Pancreatitis is associated with primary hyperparathyroidism (PHPT) in 1.5-7% of cases. The relationship of cause and effect between the two diseases has been debated.

55 prospectively documented patients aged 20-84 (median 67) years (47 women, 8 males) underwent surgery for primary hyperparathyroidism (pHPT). The most frequent symptoms and associated conditions were nephrolithiasis (42%) and neuropsychiatric symptoms (39%). A controlled study of the effects of the potent vitamin-D metabolite, 1, 25-dihydroxycholecalciferol (1,25[OH]2D3), and vitamin D3 was done in 18 non-dialysed patients with chronic renal failure (C.R.F.). Patients with a creatinine clearance below 35 ml/min and mild renal osteodystrophy were selected. Urinary cyclic AMP excretion per 24 h or per g creatinine in primary hyperparathyroidism (1 degrees HPT) has been evaluated by several authors with conflicting results. Excretory urography is not an infallible diagnostic procedure and should not be relied upon exclusively. A normal study must never result in the rejection of additional data derived from history, physical examination and laboratory tests. Two case histories are presented to underscore this caveat. Excretory urography must be supplemented additional radiographic evaluations when the conventional study fails to completely explain the clinical situation.
