Fungal disease in the immunocompromised host.

A variety of superficial and deep mycoses may affect the immunocompromised patient. Among the superficial mycoses, candidal infections are common in all groups, but dermatophyte and pityrosporum infections may also be found. Although not primarily dangerous, they may lead to secondary bacterial infections and morbidity. Of the systemic mycoses, candidiasis, aspergillosis, and mucor-mycosis are frequently lethal and require early diagnosis and aggressive antifungal treatment. Endemic mycoses, such as histoplasmosis and coccidioidomycosis, may result in severe and often fatal infections in those

Interleukin-12: role of interferon-gamma in IL-12 adverse effects.

Interleukin 12 (IL-12) activates natural killer and T cells with the secondary synthesis and release of interferon-gamma (IFN-gamma) and therefore enhances a Th1- or cell-mediated immune response. The immunostimulatory property of IL-12 may be used clinically in cancer and viral diseases. Safety studies in rodents and primates demonstrated a rather small therapeutic window with hepato- and splenomegaly, myelodepression, and lung edema. Investigations in mutant mice deficient for the IFN-gamma receptor (IFN-gamma R-/-) revealed that both the biologic and toxic effects are indirect and largely d

[Necrotizing fasciitis].

Necrotizing soft tissue infection or necrotizing fasciitis is a rapidly progressive inflammation that leads to necrosis of subcutaneous tissues and fascia with secondary necrosis of skin. It is usually attributed to group AB hemolytic streptococci but may be caused by anaerobic bacteria. It occurs with higher frequency in elders, diabetics, alcoholics and patients with impaired immunity. Report mortality rates range from 14 to 80%. An early and timely surgical debridement with remotion of all necrotic tissue is mandatory. Wounds must be closed with splint skin grafts as soon as the local viabi

Concurrent lymphangioma, immune-mediated thrombocytopenia, and von Willebrand's disease in a dog.

Lymphangioma, immune-mediated thrombocytopenia (IMT), and von Willebrand's disease (vWD) were diagnosed by histology, hematology, and a coagulation profile in a 14-month-old, female dachshund. Clinical and laboratory findings included ecchymotic inguinolabial swelling, thrombocytopenia, positive platelet factor-3 assay, prolonged buccal mucosal bleeding time, and subnormal von Willebrand factor antigen concentration and factor VIII activity. The IMT resolved with immunosuppressive glucocorticoid therapy. Histologic examination identified lymphangioma which was too extensive for surgical excisi

Cutaneous manifestations of fungal infection.

Fungal infection of the skin and subcutaneous tissue may result from either direct contact or inoculation injury (primary infection) or from hematogenous spread from a primary focus of disease (secondary infection). The parainfectious lesions of erythema nodosum and erythema multiforme are manifestations of the host's immune response to the invading fungus, particularly Histoplasma capsulatum and Coccidiodes immitis. In some patients, skin lesions may be the only sign of a systemic fungal infection, and prompt recognition of these lesions may facilitate early diagnosis and treatment. This arti

Antenatal diagnosis and treatment of fetal hydrops secondary to pulmonary extralobar sequestration.

Bronchopulmonary sequestration with associated non-immune hydrops has been previously reported with generally poor prognosis for the neonate. The vast majority of sequestrations are discovered postnatally, and less than half are correctly identified as sequestration before birth.Until recently, treatment for pulmonary sequestration was limited to postnatal respiratory support and resection of the sequestered lung. But aggressive respiratory support does not succeed in salvaging infants with underlying pulmonary hypoplasia. In our report, we describe the antenatal treatment of pleural effusions

[Hormonal interactions and glucocorticoid receptors in patients with the nephrotic syndrome].

As many as 27 children aged 6 to 15 years with morphologically verified nephropathies were examined. Four variants of changes in the thyroid status, characteristic of children with different variants of nephrotic syndrome were distinguished: 1) biochemical signs of primary hypothyroidism, 2) biochemical signs of secondary hypothyroidism, 3) low content of T3, 4) dysfunction of the hypophyseal and thyroid system. It is shown that the low level of steroid receptors, thyroid hormones that the low level of steroid receptors, thyroid hormones (T3 and T4) and cortisol is typical of children with the

Graft-versus-host disease in the central nervous system. A real entity?

Graft-versus-host disease (GVHD) classically involves the skin, intestines, liver, esophagus, and tongue. clinically apparent disease involving the heart, lungs, kidneys, and central nervous system (CNS) is frequently secondary to other complicating factors. This report describes a case of an infant with severe combined immune deficiency (SCID) who developed unusual manifestations of GVHD following a bone marrow transplant (BMT). These were complete heart block and respiratory insufficiency in the absence of significant pulmonary disease. He lived 133 days post-transplantation. At autopsy, the

Home hyperalimentation for common variable hypogammaglobulinemia with malabsorption secondary to intestinal nodular lymphoid hyperplasia.

Common variable hypogammaglobulinemia (immunodeficiency), a disorder characterized by late-onset immunoglobulin deficiency and lack of humoral immunity, has a variable association with bronchiectasis, cholelithiasis, nodular lymphoid hyperplasia, gastrointestinal neoplasia, megaloblastic anemia, and malabsorption. The patient described in this report had all of the above except neoplasia. In addition, he had calcium oxalate renal stones probably secondary to his malabsorption. The first case demonstrating the beneficial effect of home hyperalimentation in patients with severe malabsorption ref

Ocular findings in human babesiosis (Nantucket fever).

Human babesiosis (Nantucket fever) is a rare, tick-borne intraerythrocytic parasitic disease characterized by fever, lymphadenopathy, arthralgias, and hemolytic anemia. A 34-year-old woman, who had previously undergone surgical removal of her spleen, was hospitalized because of presumed hepatitis. The many retinal nerve fiber layer infarcts and serologic abnormalities suggested collagen disease. The diagnosis of babesiosis was established by the demonstration of intraerythrocytic parasites and a greatly elevated titer to Babesia microti. The infection was treated with chloroquine, orally admin

Nature of cryoglobulinaemia.

Cryoimmunoglobulins seem to differ from non-cryoimmunoglobulins in not having carbohydrate groups, most probably sialic acid residues. It is proposed that cryoimmunoglobulinaemia is a physiological event and that desialylated immunoglobulins are a normal byproduct of the immune system. The rise in serum levels of cryoimmunoglobulins is either a pre-secretory event which follows enhanced stimulation of B lymphocytes or a post-secretory event secondary to the generation of serum neuraminidase-like activity by invading microorganisms or their products. It is also proposed that the liver is the ma

A dendritic cell targeted vaccine induces long-term HIV-specific immunity within the gastrointestinal tract.

Despite significant therapeutic advances for HIV-1 infected individuals, a preventative HIV-1 vaccine remains elusive. Studies focusing on early transmission events, including the observation that there is a profound loss of gastrointestinal (GI) CD4(+) T cells during acute HIV-1 infection, highlight the importance of inducing HIV-specific immunity within the gut. Here we report on the generation of cellular and humoral immune responses in the intestines by a mucosally administered, dendritic cell (DC) targeted vaccine. Our results show that nasally delivered α-CD205-p24 vaccine in combination

Association of Viraemic Phase Viral Load, Antibody Responses, and Immune Biomarkers With Severe Dengue.

Severe dengue is influenced by the virus serotype, viral load, host exposure status, immune response, host genetics, and other host factors. The objective of the present study was to identify a panel of prognostic markers for severe dengue during the viraemic phase. We investigated 326 real-time RT-PCR positive dengue cases to find out the association of viral load, antibody titers, and immune status. Plasma levels of 27 cytokines, chemokines, and endothelial cell-related factors were estimated in a subset of 206 patients. Viral load was lower in DENV-3 cases compared to DENV-1 and DENV-2 case

Mild traumatic brain injury in Drosophila melanogaster alters reactive oxygen and nitrogen species in a sex-dependent manner.

Traumatic brain injury (TBI) is an outside force causing a modification in brain function and/or structural brain pathology that upregulates brain inducible nitric oxide synthase (iNOS), instigating increased levels of nitric oxide activity which is implicated in secondary pathology leading to behavioral deficits (Hall et al., 2012; Garry et al., 2015; Kozlov et al., 2017). In mammals, TBI-induced NO production activates an immune response and potentiates metabolic crisis through mitochondrial dysfunction coupled with vascular dysregulation; however, the direct influence on pathology is compli

Analysis of early failures after intravesical instillation therapy with bacille Calmette-Guérin for carcinoma in situ of the bladder.

OBJECTIVE: To analyse the clinical and therapeutic consequences of early treatment failure after bacille Calmette-Guérin (BCG) instillation therapy for carcinoma in situ of the bladder. PATIENTS AND METHODS: A total of 115 patients with carcinoma in situ (Tis) of the bladder were treated by intravesical instillation of living BCG vaccine (Immun BCG Pasteur F). Twenty five patients had primary Tis and 90 had secondary Tis with synchronous or prior superficial papillary tumours. All papillary tumours were resected before instillation of BCG. All patients completed one series of 6 weekly instill

Diagnostic pitfall: NSAID-induced membranous nephropathy with PLA2R positivity, IgG1-predominant deposits, C1q positivity, and concurrent acute interstitial nephritis-A case report.

Serum anti-phospholipase A2 receptor (PLA2R) antibody is widely used to diagnose primary membranous nephropathy (MN), but PLA2R positivity also occurs in secondary MN, especially nonsteroidal anti-inflammatory drug (NSAID)-induced MN, leading to a common diagnostic pitfall. We report a 71-year-old female with long-term unsupervised NSAID administration for polymyalgia rheumatica, presenting with recurrent nephrotic syndrome and acute kidney injury. Initial laboratory tests revealed severe hypoalbuminemia (15.7 g/L), elevated serum creatinine (3.98 mg/dL), massive proteinuria (10.24 g/24 h), an

Association between traumatic brain injury and risk of developing infections in the central nervous system and periphery.

Traumatic brain injury (TBI) is defined as structural damage or physiological disruption of brain function resulting from mechanisms such as direct impact, rapid acceleration or deceleration, penetrating injury, or exposure to blast waves. These biomechanical insults initiate a complex cascade of secondary injury processes, including neuronal cell death, neuroinflammation, oxidative stress, microglial activation, and blood-brain barrier dysfunction. These events contribute to the loss of homeostasis in both the central and peripheral nervous systems. Importantly, these pathophysiological chang

T-cell lymphomas in recipients of CAR-T cells: assessing risks and causalities.

The US Food and Drug Administration announcement in November 2023 regarding reports of the occurrence of secondary T-cell lymphomas in patients receiving chimeric antigen receptor T cells (CAR-Ts) for B-cell malignancies resulted in widespread concern among patients, clinicians, and scientists. Little information relevant to assessing causality, most importantly whether CAR retroviral or lentiviral vector genomic insertions contribute to oncogenesis, was initially available. However, since that time, several publications have provided clinical and molecular details on 3 cases showing clonal CA

Post-renal acute kidney injury complicated by urinary tract obstruction due to massive blood clots and severe thrombocytopenia in a patient with systemic lupus erythematosus: A case report.

Systemic lupus erythematosus (SLE) is often seen with antiphospholipid antibody syndrome (APS), and these conditions may occur concurrently with severe immune thrombocytopenia (ITP) and even acute kidney injury (AKI); however, post-renal AKI due to bleeding is uncommon. Here, we describe a case of post-renal AKI and anuria in a patient with SLE and APS, which were attributable to urinary tract obstruction due to massive blood clots caused by secondary ITP. A 50-year-old Japanese woman was admitted to our hospital with anuria, abdominal tenderness, purpura in the trunk and in both legs, and sev

Fatal hemophagocytic lymphohistiocytosis-induced multiorgan dysfunction secondary to Burkholderia pseudomallei sepsis: A case report.

BACKGROUND: Burkholderia pseudomallei (B. pseudomallei) is a short, straight, medium-sized Gram-negative bacterium that mostly exists alone, without a capsule or spores, has more than three flagella at one end, and actively moves. B. pseudomallei confers high morbidity and mortality, with frequent granulocytopenia in B. pseudomallei sepsis-related deaths. However, mortality may be related to hemophagocytic lymphohistiocytosis (HLH) secondary to B. pseudomallei infection. CASE SUMMARY: A 12-year-old female was referred from a local hospital to the pediatric intensive care unit with suspected s
