Status Epilepticus in Neonates and Infants.

Status epilepticus (SE) is a common neurological emergency in childhood associated with high mortality and morbidity. Acute management of seizures along with aggressive evaluation for establishing the underlying cause are crucial determinants of outcome. Neonatal status epilepticus carries the burden of poor neurological outcomes and may lead to global developmental delay as well as persistent seizures. The aetiology and pathophysiological mechanisms of SE in neonates and young infants differ compared to older children and adults. The most common causes of SE in neonates includes hypoxic seque

Risk factors and prognostic significance of altered left ventricular geometry in preterm infants.

OBJECTIVE: Left ventricular (LV) hypertrophy (LVH) predicts adverse cardiac events in adults. We sought to determine the risk factors and prognostic significance of altered LV geometry in preterm infants. STUDY DESIGN: In an echocardiographic, single-center, retrospective case-control study we investigated the risk factors and outcomes in patients with altered LV geometry (either increased left ventricular mass index (LVMI) or increased relative wall thickness (RWT)) from a cohort of 503 preterm infants ≤2 kg. RESULT: Altered LV geometry was seen in 180 patients and was predicted by postnata

Successful caspofungin treatment of persistent candidemia in extreme prematurity at 23 and 24 weeks' gestation.

Systemic fungal infection continues to be a major cause of mortality in extremely low-birth-weight premature infants. Amphotericin B has been recommended as the primary treatment; however, its use is limited due to drug-induced nephrotoxicity and amphotericin B-resistant candidemia. Caspofungin therapy was initiated in seven extremely premature infants at 23 and 24 weeks' gestation with persistent systemic candidiasis despite liposomal amphotericin B treatment. The gestational age was 23(+1)-24(+6) weeks, and birth weight was 530-825 g. Of the seven patients, the peripheral blood cultures of s

[Postnatal steroid treatment in preterm infants: risk/benefit ratio].

This review examines the risk/benefit ratio of postnatal steroid treatment in preterm infants and correlates epidemiological data with special emphasis on experimental evidence concening the impact of steroid on brain development. With all regimens, steroid treatment consistently reduced the need for assisted ventilation at 28 days of postnatal age or at term. However, neither oxygen at term nor neonatal mortality has been decreased by this treatment. Conversely, respiratory benefits should be weighed against several adverse effects: hyperglycemia, hypertension, gastrointestinal bleeding or pe

Iatrogenic tracheobronchial perforation in infants.

We describe the clinical and radiographic features of four premature infants with acute iatrogenic tracheobronchial perforation by endotracheal tube (ETT) placement. One tube penetrated posteriorly into the esophagus, one directly through the carina; one perforated the right main bronchus posteriorly; and one breached the site of a recently repaired tracheoesophageal fistula. In only one infant was the intubation procedure clinically recognized to be traumatic. Radiographs obtained for tube placement demonstrated inferior malposition of the ETT in all four babies, decreased pulmonary aeration

[Chronic active hepatitis and pregnancy].

In the course of chronic active hepatitis (CAH) 61 patients (CAH of virus etiology in 75% and of obscure etiology in 25%) had 211 pregnancies which terminated in delivery in 80 (38%) cases, spontaneous abortions in 12 (6%) cases, and medical abortions in 119 (56%) cases. In most cases there was successful termination of pregnancies without deterioration resulting from liver disease including patients treated adequately with immunosuppressive drugs. CAH exacerbations associated with pregnancies and their outcomes were noted in 23 (38%) cases. They were more frequent and more severe in patients

Mode of delivery in the low birth weight fetus. Delivery by cesarean section independent of fetal lie versus vaginal delivery in vertex presentation. A study with long-term follow-up.

In a paired controlled multicenter study of patients in preterm labor of unknown etiology without additional maternal or fetal complications, 59 low birth weight infants in vertex presentation born vaginally were compared with 59 infants delivered by cesarean section. In the early postpartum period, hypothermia and acidosis occurred more often in the vaginal delivery group. The rate of respiratory disorders and need for assisted ventilation did not differ between the groups. Persistent ductus arteriosus occurred in 19% in the vaginal delivery group and in 7% in the abdominal delivery group. At

Heparin for the prevention of intraventricular haemorrhage in preterm infants.

BACKGROUND: Preterm birth remains the major risk factor for the development of intraventricular haemorrhage, an injury that occurs in 25% of very low birth weight infants. Intraventricular haemorrhage is thought to be venous in origin and intrinsic thromboses in the germinal matrix are likely to play a triggering role. Heparin activates antithrombin and promotes the inactivation of thrombin and other target proteinases. The administration of anticoagulants such as heparin may offset the increased risk of developing intraventricular haemorrhage and may also reduce the risk of developing parench

Intermittent auscultation (IA) of fetal heart rate in labour for fetal well-being.

BACKGROUND: The goal of fetal monitoring in labour is the early detection of a hypoxic baby. There are a variety of tools and methods available for intermittent auscultation (IA) of the fetal heart rate (FHR). Low- and middle-income countries usually have only access to a Pinard/Laënnec or the use of a hand-held Doppler device. Currently, there is no robust evidence to guide clinical practice on the most effective IA tool to use, timing intervals and length of listening to the fetal heart for women during established labour. OBJECTIVES: To evaluate the effectiveness of different tools for IA

Maternal epigenetic signatures are associated with small for gestational age births among black women.

Approximately 10% US infants are born small for gestational age (SGA), a condition linked to increased morbidity and mortality. Infants born to Black women are twice as likely to be SGA compared with those born to White women. Although maternal factors, including epigenetic modifications, likely contribute to SGA, the underlying biological mechanisms remain poorly understood. We evaluated whether epigenetic modifications in early pregnancy were associated with SGA among pregnant Black women. We analyzed data from 931 pregnant non-Hispanic Black women (6-13 weeks of gestation) enrolled in the N

Care bundle for preventing intraventricular hemorrhage in premature infants: a best practice implementation project.

INTRODUCTION: Intraventricular hemorrhages remain a major problem in neonatology, as their complications affect neonatal morbidity, mortality, and long-term neurodevelopmental outcomes. AIM: The aim of this project was to prevent intraventricular hemorrhage in premature infants during their first days of life in a neonatal intensive care unit (NICU). METHODS: This pre- and post-implementation clinical audit project used the JBI Evidence Implementation Framework and was conducted in a tertiary-level Spanish NICU with a consecutive sample. A baseline audit was conducted using 13 audit criteria

Increased incidence of necrotizing enterocolitis in very/extremely preterm infants with early hypotension: a retrospective cohort study.

BACKGROUND: The relationship between neonatal early hypotension and necrotizing enterocolitis (NEC) remains uncertain, with inconsistent findings reported in previous studies. The objective of this study was to assess whether early hypotension in very/extremely preterm infants was associated with an increased risk of NEC, aiming to facilitate early detection and prevention. METHODS: A retrospective cohort analysis was performed in preterm infants with gestational ages (GA) less than 32 weeks, admitted to the Seventh Medical Center of the Chinese PLA General Hospital from January 2021 through

Bilateral renal agenesis: fetal intervention and outcomes.

Bilateral renal agenesis (BRA) is a fetal anomaly which leads to anhydramnios and resultant pulmonary hypoplasia. Historically, this anomaly was universally fatal early in the neonatal period due to the severity of the associated lung disease. Over the last 30 years, innovations in fetal therapies-specifically, serial amnioinfusions-have led to instances of infant pulmonary survival and initiation of postnatal dialysis, raising the possibility that early neonatal death may not be inevitable. Amnioinfusions are not without risk, and maternal complications can include prelabor rupture of membran

Predictors of a non-response to prophylactic indomethacin for patent ductus arteriosus in preterm infants.

BACKGROUND: Preterm infants are recommended for prophylactic indomethacin (PIND) to promote closure of patent ductus arteriosus (PDA) and reduce morbidity and mortality. This study investigated the predictive factors of a non-response to PIND for PDA in preterm-birth infants. METHODS: Consecutive preterm-birth infants (gestational age: < 28 weeks) who received PIND between 2009 and 2019 were retrospectively enrolled. Seventy-six eligible participants were classified as PIND responders (N = 42) or non-responders (N = 34). Information on potential confounders in maternal obstetric and perinatal

Neonatal Research at the Archiospedale Santo Spirito in Rome.

Abandoning infants was a heritage of the Roman Empire. Foundling hospitals were established in Italy earlier and in greater number than in other countries; their goal was to prevent infanticides. The Foundling Hospital in Rome, established in the Santo Spirito Church in 1204, paved the way toward modern hospital care and child protection. The Order of the Holy Spirit was appointed by Pope Innocent III to care for foundlings, and set up a network of infant nurseries all over Europe. Poor unmarried pregnant women received obstetric services free of charge. Infants were admitted anonymously by th
