Arthritis due to tuberculosis, fungal infections, and parasites.

Tuberculous, fungal, and parasitic infections infect millions of people throughout the world. While other problems usually overshadow their rheumatologic manifestations, nearly all these infections can involve bone or joints and may on occasion present with rheumatologic symptoms. The classic model of these diseases presenting as chronic monoarticular arthritis is still generally valid but other presentations, such as tenosynovitis with atypical mycobacterial infections, erythema nodosum with leprosy, coccidioidomycosis and histoplasmosis, and reactive arthritis with schistosomiasis and helmin…

Immunologic abnormalities accompanying acute and chronic viral infections.

A number of viral infections induce profound alterations in immune function; these changes may or may not be related to direct infection of lymphoid cells. The immunologic abnormalities that occur during acute systemic viral infections such as measles or infectious mononucleosis include suppression of responses to tuberculin skin tests in vivo and suppression of mitogen responses in vitro, evidence of activation of the immune system with up-regulation of activation-associated cell-surface markers and soluble cellular products, and evidence of altered immune regulation with autoimmune disease a…

[Virologic diagnosis of maternal-fetal infections (HIV excluded)].

Some viral infections, such as rubella, cytomegalovirus and parvovirus B 19 infections or varicella, are particularly feared in pregnant women in view of the risk of foetal infection. Laboratory tests play an important role since there is no other way of diagnosing the infection and determining whether it is primary or secondary, a distinction which to a great extent conditions the severity of the foetal disease. However, no virological diagnosis can be made unless the examinations needed are prescribed correctly and at the right moment. The diagnosis of rubella is now well codified, but that …

Vulvovaginal human papillomavirus infections: clinical implications and management.

The past 2 decades have witnessed an alarming increase in the incidence of human papillomavirus infections. Clinically evident cases represent only a small portion of the infected population, because millions of people have subclinical or latent infection. Human papillomavirus infection is recognized as a precursor to malignancy. Thus it is important to treat clinically evident infection. Treatment is complicated by the ability of the virus to establish latent infection and the lack of an effective antiviral agent. At present treatment is limited to the destruction of obvious and intraepitheli…

Infections transmitted by large and small laboratory animals.

Although zoonotic spread of infectious disease continues to occur in laboratory animals used in biomedical research, reported outbreaks have been minimized with the advent of rigorous veterinary and husbandry procedures, the use of commercially reared animals, and the institution of appropriate personnel health programs. Maintaining animals in modern facilities with appropriate safeguards against introduction of vermin and biologic vectors is also important in preventing zoonotic disease in personnel. Nevertheless, established zoonotic agents, newly discovered microorganisms, or new animal spe

Cestode infections in animals: immunological diagnosis and vaccination.

Cestode infections in animals are important because several species are zoonotic, causing cysticercosis and hydatidosis in man, and because of the economic losses incurred due to infections in livestock. Information on immunological diagnosis of and vaccination against cestode infection is restricted almost exclusively to the taeniid cestodes in which two groups of mammalian hosts are concerned: the intermediate host infected with the larval parasite and the definitive host infected with the adult tapeworm parasite. Research towards developing serological tests for the diagnosis of larval cest…

Brain abscess and infections aneurysm of extravascular origin.

Acute and chronic ear infection can lead to intracranial involvement. We report on a case in which two typical cerebral complications followed a chronic otitis media. It seemed that there was no causal relationship between brain abscess and meningitis. The meningitis itself was complicated by rupture of an infectious intracranial aneurysm with fatal outcome. We have not been able to find a similar case in the literature. Clinical picture and therapeutic management are discussed.

Blastomycosis: fungal infections of the lung update: 1989.

Three additions to the understanding of blastomycosis have been noted over the past several years. First, Blastomyces dermatitidis has now been grown from soil in association with epidemics of infection, indicating that microfoci of the organism exist in nature. Second, immune markers of previous infections have been used to help confirm that subclinical or asymptomatic cases occur in endemic areas. This is well recognized for other fungal infections, like histoplasmosis, and is increasingly recognized for blastomycosis. Third, alternatives to amphotericin B have been discovered effectively to…

The optimal management of lower urinary tract infections.

Lower urinary tract infection is a common problem for women, and tends to be recurrent. It is not associated with significant long term morbidity. Optimal management for symptomatic infection is short course antimicrobial therapy. For women with frequent, symptomatic, recurrent infection a number of therapeutic options are available including single dose self-treatment, extended low dose prophylaxis, or post-intercourse prophylaxis. For women who do not wish to experience any symptomatic episodes, prophylactic therapy, either continuous or during risk situations particularly post-intercourse,

Prevalence of human papillomavirus infections in term pregnancy.

The prevalence of human papillomavirus (HPV) infections in the lower genital tracts of pregnant and nonpregnant women was studied. Cervical and vaginal exfoliated cells were obtained during a routine pelvic examination in 45 pregnant women in early labor, and in 44 nonpregnant women. A detailed questionnaire with emphasis on sexual history and sexually transmitted diseases was administered. The two groups of women were comparable with respect to mean age, prior pregnancy experiences, and number of lifetime sexual partners. Compared with pregnant women, the nonpregnant women had a more frequent…

The pathogenesis of bovine virus diarrhoea virus infections.

Bovine virus diarrhoea virus (BVDV) disease in cattle ranges from the transient acute infections, which may be inapparent or mild, to mucosal disease which is inevitably fatal. On occasions the acute infections can lead to clinical episodes of diarrhoea and agalactia but as these syndromes cannot be reproduced experimentally, the pathogenesis remains unclear. The immunosuppressive effect of acute BVDV infections can enhance the clinical disease of other pathogens and this may be an important part of the calf respiratory disease complex. Although BVDV antigen has been demonstrated within the ly…

Classification and clinical manifestations of oral yeast infections.

By tradition oral candidosis has been classified into acute pseudomembranous (thrush), acute atrophic, chronic atrophic, and chronic hyperplastic types. However, pseudomembranous candidosis is not always acute but may last for many months. Furthermore, the value of using the term atrophic to describe an erythematous area is limited. Moreover, some of the various types have been associated with other clinical entities, which appear to have a combined bacterial/mycologic etiology. A revision of the classification should be based on the use of clinical terms, and in a previous study of multifocal

Incidence of fungal infections in chronic maxillary sinusitis.

A study conducted on seventy-five cases of chronic maxillary sinusitis presenting at the E.N.T. Outpatients' Department of Dayanand Medical College, Ludhiana revealed the presence of fungi in 8 cases. The return fluids of proof puncture and antral lavage of the sinuses were subjected to standard extraction procedures. Methods of isolation and identification of the causal fungi are described. The role of fungi, particularly Aspergillus, as pathogen is discussed in the context of antibiotic and immuno-suppressive therapy and local predisposing factors.

Viral lower respiratory tract infections in Filipino children.

Viral causes of acute lower respiratory tract infection were studied prospectively between 15 June and 31 October 1984 in 312 Filipino children less than 5 years old living in periurban slums and middle-class housing. The cause was based on viral antigen detection, virus isolation, and antibody assays. There were 131 children (41.2%) who were admitted to the hospital, and 150 (47%) had an infiltrate on chest radiograph. A total of 198 viral infections were confirmed in 162 patients (51.9%), 42.3% with single viral infection and 9.6% with mixed (two or more) infection. The infections were measl…
