A review of ceramic bearing materials in total joint arthroplasty.

Bearings made of ceramics have ultra-low wear properties that make them suitable for total hip arthroplasty (THA) and total knee arthroplasty (TKA). When compared to cobalt chrome (CoCr)-on-polyethylene (PE) articulations, ceramics offer drastic reductions in bearing wear rates. Lower wear rates result in fewer wear particles produced by the articulating surfaces. In theory, this should reduce the risk of periprosthetic osteolysis and premature implant loosening, thereby contributing to the longevity of total joints. In addition to ceramics, other alternative bearing couples, such as highly cr…

[Undiagnosed posterior dislocations of the shoulder joint. A preliminary report].

Posterior dislocations are infrequent injuries of the proximal humerus, counting for 1-4% of all shoulder dislocations. Frequently, they are related to epileptic seisures or electric current injuries or accompany complete anatomical neck fractures of the proximal humerus. Very frequently, they remain undiagnosed and their late diagnosis results in poor functional outcomes. The authors assess the surgical procedure and its results in five cases of primarily undiagnosed posterior dislocations. In two cases, transposition of the m. subscapularis was performed, in one case hemiarthroplasty, and in

Biomechanics of the elbow joint in tennis players and relation to pathology.

Elbow injuries constitute a sizeable percentage of tennis injuries. A basic understanding of biomechanics of tennis and analysis of the forces, loads and motions of the elbow during tennis will improve the understanding of the pathophysiology of these injuries. All different strokes in tennis have a different repetitive biomechanical nature that can result in tennis-related injuries. In this article, a biomechanically-based evaluation of tennis strokes is presented. This overview includes all tennis-related pathologies of the elbow joint, whereby the possible relation of biomechanics to pathol

Synovial chondromatosis of the temporomandibular joint: CT and MRI findings.

We report a rare case of synovial chondromatosis of the temporomandibular joint (TMJ) in a 31-year-old man. CT examination showed a mixed soft tissue mass with small calcifications near the right TMJ joint space. MR images revealed a heterogeneous mass on the different sequences and, after contrast administration, originating in the right TMJ. Cytology showed chondroid cellularity. The lesion was surgically removed and final histological study demonstrated the diagnosis of a synovial chondromatosis. We highlight the importance of the complementary findings from CT and MR, especially the import

[Spontaneous temporomandibular joint herniation into the external auditory canal].

OBJECTIVES: We report a rare case of spontaneous temporomandibular joint herniation into the external auditory canal. MATERIAL AND METHODS: A 42-year-old woman consulted for recurrent right otalgia. RESULTS: The clinical examination revealed a white mass of the anterior wall of the bony external auditory canal, very close to the tympanic annulus, which was replaced by an invagination of the skin of the canal when the patient opened her mouth, consistent with temporomandibular joint herniation into the external auditory canal. CONCLUSION: Inflammatory, malignant, and traumatic lesions are kn

Tibial intraneural ganglia in the tarsal tunnel: Is there a joint connection?

Intraneural ganglia are rare entities, and, as such, their pathogenesis has been extremely controversial. Recent evidence from intraneural g … is that tibial intraneural ganglia occurring within the tarsal tunnel region arise from neighboring joints, we analyzed 3 patients retrospectively, all of whom had magnetic resonance (MR) imaging and operative intervention. [Infectious monoarthritis in the subtalarian joint due to N. gonorrhoae].

We report a case of gonococcal monoarthritis in the ankle of a young female. A blind tap of synovial fluid from the talocrural joint was without bacterial growth. The patient received anti-inflammatory treatment but did not improve. In an ultrasound-guided puncture of the subtalarian joint, the synovial fluid culture revealed N. gonorrhoeae. Due to the increasing incidence of gonorrhoea in Denmark, gonococcal monoarthritis should be suspected in any case of monoarthritis in young, sexually active patients. If small joints are affected, ultrasound-guided puncture is recommended to ensure correc…

A new surgical technique for the ligament reconstruction of the trapeziometacarpal joint.

Isolated traumatic dislocation of the trapeziometacarpal joint is rare compared with fracture-dislocation of the joint. The mechanism of injury is usually axial loading on a flexed thumb metacarpal, leading to dorsal dislocation of the joint. Closed reduction with immobilization is an acceptable method of treatment if the joint is stable after the reduction. Otherwise, early ligamentous reconstruction is recommended to reduce the likelihood of secondary arthritis. Various surgical techniques have been used to reestablish the ligamentous integrity of the joint; however, these techniques usually

Joint distribution approaches to simultaneously quantifying benefit and risk.

BACKGROUND: The benefit-risk ratio has been proposed to measure the tradeoff between benefits and risks of two therapies for a single binary measure of efficacy and a single adverse event. The ratio is calculated from the difference in risk and difference in benefit between therapies. Small sample sizes or expected differences in benefit or risk can lead to no solution or problematic solutions for confidence intervals. METHODS: Alternatively, using the joint distribution of benefit and risk, confidence regions for the differences in risk and benefit can be constructed in the benefit-risk plan

[The acetabular transposition in surgical treatment of hip joint dysplasia].

From 1997 through 2005, 118 patients aged from seven through sixteen years with unstable dysplastic hip joints (126 hip joints) were operated upon by an acetabular transposition technique after triple pelvic osteotomy. The dynamics of principal roentgenological values were studied. Based on CT and 3D reconstructions a blood supply in the hip joint region and the birth canal measurements were evaluated. In all patients a satisfactory clinical result has been obtained. All the patients have attained a social and professional adaptation. Neither blood supply disorders around the hip joint nor aff…

A single surgeon's experience of treatment of Lisfranc joint injuries.

This paper assesses the outcomes and complications of Lisfranc joint injuries treated at a regional trauma centre under the care of a single surgeon. We performed a retrospective study of all patients that underwent ORIF of a Lisfranc joint injury over a 5-year period. Case note review, radiology review, and questionnaire assessment were performed. We analysed for correlations between outcome and injury type, mechanism of injury, and presence of other ipsilateral limb injury and pure ligamentous injury. All injuries were classified according to a system described by Myerson et al. (Types A, B1

[Surgical treatment of temporomandibular joint: apropos of 94 cases].

INTRODUCTION: The temporomandibular joint (TMJ) is a complex entity subjected to repeated stress with several symptoms. About one-third of people have at least one of those symptoms but only few (3 to 7%) need treatment. The aim of this retrospective study was to evaluate the results of temporomandibular joint surgery in 94 patients. PATIENTS AND METHODS: Several data were used for decision-making and the surgical technique was adapted to the etiology. The type of postoperative physiotherapy performed depended on the type of pathology. RESULTS: Most patients who underwent surgery between 198

Osteoarthritic femoral articular cartilage of knee joint in man.

Osteoarthritis is the most common of the various articular disorders affecting man. The present study was conducted to observe the microstructure of osteoarthritic human femoral articular cartilage by light microscopy. Fifteen osteoarthritic cartilage specimens obtained from patients with primary osteoarthritis (52-80 years) undergoing total knee replacement were processed for paraffin sections. 5 m thick sections were observed under light microscope. The articular surface appeared to be very irregular and fibrillated in all the specimens. The cartilage did not show the normal zonation. Variou

Prevalence of incontinence in women with benign joint hypermobility syndrome.

The objective of this study was to determine whether the prevalence of urinary and anal incontinence in women with benign joint hypermobility syndrome (BJHS) was more than in a control group without this condition. The International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) and the Manchester Health Questionnaire were sent to 30 patients with benign joint hypermobility syndrome and 30 healthy women without this condition and results were compared. It was found out that the prevalence of urinary incontinence in the study group we surveyed was 18/30 and in the control group

[Primary hypertrophic osteoarthropathy as a rare cause of joint pain].

Primary hypertrophic osteoarthropathy (HOA) is a rare cause ofjoint pain. The diagnosis is often delayed, due to mild complaints. However, finger clubbing combined with joint pain should suggest considering HOA in the diagnosis. The progression of toes and fingertips widening is slow and can be overlooked by a patient. It is common that patient seeks physician's aid only when the joint pain occurs. The presented case is an example of delayed diagnosis. Due to the fact that secondary HOA coexists with some diseases associated with serious prognosis it is recommended to exclude other possible ca

Anaesthesia for a patient with congenital methaemoglobinaemia and temporomandibular joint ankylosis.

We report a 10-year-old child with congenital methaemoglobinaemia and temporomandibular joint ankylosis who presented for ophthalmic surgery. He had had two previous anaesthetics in another institution. The first was short and uneventful, and no abnormality was noted. During the second anaesthetic, low haemoglobin oxygen saturation on pulse oximetry was noted after an inhalational induction. The diagnosis was not apparent and the management of the apparent hypoxaemia markedly complicated the clinical course. The patient required prolonged intensive care and tracheostomy. The tracheostomy was u
