Intravascular large B cell lymphoma diagnosed by senile angioma biopsy.

An 82-year-old man without notable medical history was admitted to our hospital following subacute deterioration of apettite, disorientation and strange behavior. There was spasticity of the right extremities without weakness. LDH and serum soluble IL2 receptor antibody levels were elevated, and as well as the protein level and IgG levels in the cerebrospinal fluid. CT scanning of the brain revealed a lesion at the left corona radiata. The patient's level of consciousness was worsening, and follow-up study of the brain showed a new lesion in the left occipital lobe. Intravascular lymphomatosis

[Intravascular lymphomatosis: a case report].

We report a case of a 64-year-old man with intravascular lymphomatosis. He had dementia and generalized seizure. Computed tomogram scan and magnetic resonance imaging showed multiple infarctions in the right frontal lobe, right cerebellar hemisphere and left occipital lobe. A month after admission he had a daily high fever and frequent convulsions. The level of serum LDH was progressively getting higher and the level of beta 2-microglobulin in both serum and CSF was high. One and a half months after admission we performed an open biopsy for making a definite diagnosis by pathological examinati

[A 52-year-old man with sensory aphasia and multiple intracranial masses].

We report a 52-year-old-man who developed aphasia, apraxia and agnosia. He was well until December of 1991 when he noted an onset of skin scaling in the extensor sides of his elbow joints; he was admitted to the dermatology service of our hospital and was diagnosed as having pemphigoid. Treatment with PDN 30 to 60 mg/day and azathioprine 50-100 mg was started. While in the hospital there was a insidious onset of right tinnitus and tingling sensation in his right leg in the middle of March, 1991. During next one week, his family noted that the patient responded incoherently in his speech once i

Extranodal marginal zone lymphoma occurring along the trachea and central airway.

Extranodal marginal zone lymphoma is a low-grade B cell lymphoma that presents with an indolent clinicopathologic nature. Although this tumor can occur in various sites, including the gastrointestinal tract and lungs, it develops and spreads extremely rarely along the trachea and central airway. We report a case of extranodal lymphoma of mucosa-associated lymphoid tissue with tracheobronchial involvement. An 83-year-old woman presented with a cough and dyspnea. Bronchoscopic evaluation confirmed diffuse, multiple nodular lesions in both the trachea and large bronchi, and she was diagnosed with

Preclinical discovery and clinical development of glofitamab in relapsed/ refractory large B-cell lymphomas.

INTRODUCTION: Relapsed/refractory (R/R) large B-cell lymphoma (LBCL) remains a clinical challenge, particularly for patients with primary refractory or relapsed disease and ineligible for cellular therapies. T-cell engagers in the form of CD20xCD3 bispecific antibodies (BsAb) offer an effective 'off-the-shelf' therapy option that redirects endogenous T cells against malignant B cells. AREAS COVERED: This review summarizes the preclinical and translational development of glofitamab, a full-length 2:1 BsAb, focusing on R/R LBCL. The authors discuss the molecular design rationale, mechanism of a

Outcomes of Bridging Therapy in Patients With Relapsed/Refractory Large B Cell Lymphoma Receiving Third-Line CAR T.

Bridging therapy (BT) is frequently used in patients with relapsed/refractory (R/R) large B-cell lymphoma (LBCL) undergoing chimeric antigen receptor T cell (CAR T) therapy, but the optimal BT approach remains uncertain. We evaluated BT strategies and their associations with response and survival outcomes. Accordingly, we included patients with R/R LBCL evaluated for third-line or later CD19-directed CAR T at Princess Margaret Cancer Center from 2017 to 2024 (follow-up to 01/2025). BT modalities included chemotherapy, polatuzumab-based chemotherapy, radiotherapy, corticosteroids, and none. We

CRS and ICANS in CAR T-cell therapy for large B-cell lymphoma.

Chimeric antigen receptor T-cell (CAR T-cell) therapy has become an established treatment option for relapsed or refractory large B-cell lymphoma, offering durable remission in some previously incurable cases. However, the effective management of immune-mediated toxicities, particularly cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS), remains critical to ensure safety. Treatment algorithms have been proposed based on the American Society for Transplantation and Cellular Therapy consensus grading system by the Joint Accreditation Committee of t

A case report of lymphoma presenting initially with tracheoesophageal fistula and literature review.

RATIONALE: Extranodal involvement in non-Hodgkin lymphoma most commonly occurs in the gastrointestinal tract, whereas direct esophageal invasion by lymphoma is rare. The coexistence of a tracheoesophageal fistula (TEF) in such cases is even more exceptional, typically indicating a poor prognosis. This case report describes a female patient with advanced-age, large B-cell lymphoma involving both the lungs and esophagus, complicated by a TEF. Following treatment, the patient demonstrated improved physical strength and nutritional status, surviving more than 15 months post-diagnosis. PATIENT CON

Timing of anti-PD-L1 antibody initiation affects efficacy/toxicity of CD19 CAR T-cell therapy for large B-cell lymphoma.

More than half of the patients treated with CD19-targeted chimeric antigen receptor (CAR) T-cell immunotherapy for large B-cell lymphoma (LBCL) do not achieve durable remission, which may be partly due to PD-1/PD-L1-associated CAR T-cell dysfunction. We report data from a phase 1 clinical trial (NCT02706405), in which adults with LBCL were treated with autologous CD19 CAR T cells (JCAR014) combined with escalating doses of the anti-PD-L1 monoclonal antibody, durvalumab, starting either before or after CAR T-cell infusion. The addition of durvalumab to JCAR014 was safe and not associated with i

[A Case of Synchronous Double Cancer of Malignant Lymphoma of the Ileum and Carcinoma of the Jejunum].

The patient was a 72-year-old woman with a history of surgery for uterine, bladder, left breast, and rectal cancers. The patient was admitted to the emergency department due to abdominal pain and vomiting. Abdominal and pelvic computed tomography showed a mass lesion and bowel obstruction at the end of the ileum, and positron emission tomography showed a mass lesion with accumulation in the same area of the pelvis. The patient was referred to our department for surgical treatment. The tumor at the end of the ileum invaded the ileal conduit, abdominal wall, right-sided colon, and mouth of the i

Potential synergy between radiotherapy and CAR T-cells - a multicentric analysis of the role of radiotherapy in the combination of CAR T cell therapy.

BACKGROUND: Chimeric antigen receptor (CAR) T-cell therapy has improved the limited overall survival (OS) of patients with intensively pretreated diffuse large B-cell lymphoma (DLBCL). However, the potentially life-threatening toxicities of CAR T-cells and early relapses remain a challenge. As suggested by smaller monocentric analyses, radiotherapy (RT) in combination with CAR T-cells may have an immunomodulatory effect. METHOD/ RESULTS: In this multicentric retrospective analysis, we investigated potentially synergistic effects of RT and CAR T-cells. Of 78 patients from four centers who rece

Ibrutinib plus RICE or RVICI for relapsed/refractory mature B-cell non-Hodgkin lymphoma in children and young adults: SPARKLE trial.

Part 1 results of the open-label, randomized, global phase 3 SPARKLE trial supported continued assessment of ibrutinib with either modified rituximab, ifosfamide, carboplatin, and etoposide (RICE) or rituximab, vincristine, ifosfamide, carboplatin, idarubicin, and dexamethasone (RVICI) in pediatric patients with relapsed/refractory (R/R) mature B-cell non-Hodgkin lymphoma (B-NHL). We report final results of Part 2 evaluating the efficacy of ibrutinib plus RICE or RVICI vs RICE/RVICI alone. Patients aged 1 to 30 years (initial diagnosis <18 years) were randomized 2:1 to receive ibrutinib with o

Infectious complications of car T-cell therapy: A longitudinal risk model.

BACKGROUND: CAR T-cell therapy, where a patient's own T cells are re-engineered to express a receptor to a target of interest, is becoming an increasingly utilized cancer-directed therapy. There are significant toxicities that contribute to a novel state of immunocompromise, leading to new patterns of infectious complications that require further detailed study. METHODS: We created a single-center cohort of adult recipients of CD19-directed CAR T-cell therapy and assessed infectious outcomes, supportive care received, toxicities, and markers of immune function up to 2 years following CAR T-ce

A case of complete atrioventricular block associated with primary cardiac lymphoma reversed without cardiac pacemaker implantation.

Primary cardiac lymphoma (PCL) is a rare malignant lymphoma that is characteristically confined to the heart and/or pericardium. Here, the case of a 70-year-old male patient with complete atrioventricular block (AVB) associated with PCL is presented. The patient had a 10-month history of palpitation and electrocardiogram (ECG) showed a complete AVB. Additionally, transthoracic echocardiography indicated pericardial effusion where atypical lymphoid cells were identified by pericardiocentesis. Subsequent mediastinal lymph node biopsy revealed non-germinal centre diffuse large B-cell lymphoma. Th

Predict DLBCL patients' recurrence within two years with Gaussian mixture model cluster oversampling and multi-kernel learning.

BACKGROUND AND OBJECTIVE: Diffuse large B-cell lymphoma (DLBCL) is common in adults' non-Hodgkin's lymphoma. Relapse mainly occurs within two years after diagnosis and has a poor prognosis. Relapse after two years is less frequent and has a better prognosis. In this work, we constructed a relapse prediction model for diffuse large B-cell lymphoma patients within two years, expecting to provide a reference for Clinicians to implement individualized treatment. METHOD: We propose a secondary-level class imbalance method based on Gaussian mixture model (GMM) clustering resampling to balance the d

A Model to Estimate Cytokine Release Syndrome and Neurological Event Management Costs Associated With CAR T-Cell Therapy.

Chimeric antigen receptor (CAR) T-cell therapies demonstrated efficacy in relapsed/refractory large B-cell lymphoma (LBCL) but are associated with cytokine release syndrome (CRS) and neurological events (NE). We wanted to estimate the total cost of CRS and NE management among patients with relapsed/refractory LBCL treated with lisocabtagene maraleucel (liso-cel), axicabtagene ciloleucel (axi-cel), or tisagenlecleucel (tisa-cel) in the third- or later-line setting. An economic decision tree model was developed using clinical and economic data to estimate a weighted average per-patient adverse e

[Warm autoimmune hemolytic anemia accompanied by metachronous double primary cancer].

In about half of the cases, autoimmune hemolytic anemia (AIHA) is secondary to an underlying disease, often due to paraneoplastic syndromes. Recently, the number of patients developing metachronous multiple primary malignant tumors (MPMTs) has been increasing due to the aging of the population and the longer survival times of those with malignant tumors. A 78-year-old woman was diagnosed with sigmoid colon cancer in May 2017 and with warm AIHA in October 2017. She received prednisolone for her warm AIHA treatment, which relieved her anemia symptoms. In January 2020, she had a warm AIHA relapse
