Changes in Tendon Elongation and Muscle Atrophy Over Time After Achilles Tendon Rupture Repair: A Prospective Cohort Study on the Effects of Early Functional Mobilization.

BACKGROUND: Early functional mobilization (EFM) may improve patient outcome after Achilles tendon rupture (ATR). However, whether EFM affects patient outcome via changes in tendon elongation, thickening, or calf muscle atrophy is unknown. PURPOSE: To analyze differences in tendon and muscle morphology recovery over time between groups treated with EFM or standard treatment after ATR repair. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: This prospective cohort study included 86 patients (20 women) with ATR repair who had a mean (SD) age of 39.3 (8.2) years and were part of a lar

Mechanisms of Neuronal Alternative Splicing and Strategies for Therapeutic Interventions.

Many cellular and physiological processes are coordinated by regulatory networks that produce a remarkable complexity of transcript isoforms … e resource for therapeutic development, as illuminated by recent successful interventions of spinal muscular atrophy. Here, we discuss the latest progress in the study of the emerging complexity of alternative splicing mechanisms in neurons, and how these findings inform new therapies to correct and control splicing defects.

Exercise with blood flow restriction: an effective alternative for the non-pharmaceutical treatment for muscle wasting.

Significant muscle wasting is generally experienced by ill and bed rest patients and older people. Muscle wasting leads to significant decrements in muscle strength, cardiorespiratory, and functional capacity, which increase mortality rates. As a consequence, different interventions have been tested to minimize muscle wasting. In this regard, blood flow restriction (BFR) has been used as a novel therapeutic approach to mitigate the burden associated with muscle waste conditions. Evidence has shown that BFR per se can counteract muscle wasting during immobilization or bed rest. Moreover, BFR ha

Developing a systematic search strategy related to people with disability: A brief report testing the utility of proposed disability search terms in a search about opioid use.

BACKGROUND: The varied use of the term "disability" in the scientific literature makes it challenging to conduct systematic reviews of healt … articles captured utilizing the general search terms. The highest amount of overlap was for spinal muscular atrophy at 33.3%, with the overall median proportion of overlap being 13.4% (mean = 15.7%; SD = 11.7%). CONCLUSIONS: With a systematic search for articles about disability associated with opioid use as an example, condition-specific search terms capture a …

Ventilator-induced diaphragm dysfunction in critical illness.

Mechanical ventilation (MV) is life-saving for patients with acute respiratory failure but also causes difficult liberation of patients from ventilator due to rapid decrease of diaphragm muscle endurance and strength, which is termed ventilator-induced diaphragmatic damage (VIDD). Numerous studies have revealed that VIDD could increase extubation failure, ICU stay, ICU mortality, and healthcare expenditures. However, the mechanisms of VIDD, potentially involving a multistep process including muscle atrophy, oxidative loads, structural damage, and muscle fiber remodeling, are not fully elucidat

Respiratory and Peripheral Muscle Assessment in the Intensive Care Unit.

Atrophy and weakness of the respiratory and peripheral muscles is a common problem in the intensive care unit (ICU). It is difficult to diagnose, particularly in the early stages of critical disease. Consequently, many cases are detected only in advanced stages, for example, when difficulties in mechanical ventilation weaning are encountered. The aim of this review is to describe the main tools that are currently available for evaluation of peripheral and respiratory muscles in the ICU. Techniques of varying complexity and specificity are discussed, and particular emphasis is placed on those w

Electromyostimulation to fight atrophy and to build muscle: facts and numbers.

In recent years, electrical myostimulation (EMS) is becoming more and more popular to increase muscle function and muscle weight. Especially it is applied in healthy individual after injury to rebuild muscle mass and in severely atrophic patients who are not able or willing to perform conventional exercise training programs. Studies in experimental models as well as in human subjects confirmed that EMS can increase muscle mass by around 1% and improve muscle function by around 10-15% after 5-6 weeks of treatment. Despite a severe increase in circulating creatine kinase during the first session

Muscle Mass, Quality, and Composition Changes During Atrophy and Sarcopenia.

Skeletal muscle mass (SMM) and muscle strengh reach their peak in 20s to 40s of age in human life and then decrease with advancing age. The decrease rate of muscle strength or power was twice to four times as large as that of the SMM. Thus, the normalized muscle force (muscle strength divided by SMM) also decreases in aging. It depends on the number of factors in skeletal muscle tissues and neuromuscular system. In human study, SMM cannot be measured directly without dissection so that all of the methodologies are indirect methods to assess SMM, even computing tomography or magnetic resonance

Effect of resistance training set volume on upper body muscle hypertrophy: are more sets really better than less?

BACKGROUND: Resistance training (RT) for upper body muscle hypertrophy (UBMH) typically entails high volumes of sets per muscle group per training session. The majority of RT regimens does not discriminate between upper and lower body muscle groups, while these groups may respond differently to RT set volumes in terms of maximum skeletal muscle mass gain. Recent studies have examined the effect of different set volumes on the extent of UBMH to formulate optimal RT regimens and to make RT programmes more time-efficient. OBJECTIVE: To analyse the effect of the number of RT sets on the extent of

Emerging roles of nuclear phosphatase SCP4 in CKD-associated muscle wasting.

Cachexia with wasting of muscle protein is a serious complication of chronic kidney disease (CKD). Muscle protein phosphorylation is a potential therapeutic target. Liu et al. reported that small C-terminal domain phosphatase (SCP) 4 was increased in muscles of patients and mice with CKD. Importantly, knockdown of SCP4 significantly ameliorated muscle wasting in CKD mice. Inhibition of SCP4 may represent a novel therapeutic intervention for muscle wasting in patients with CKD.

Comorbidities and systemic effects of chronic obstructive pulmonary disease.

Although primarily a lung disease, chronic obstructive pulmonary disease (COPD) is now recognized to have extrapulmonary effects on distal organs, the so-called systemic effects and comorbidities of COPD. Skeletal muscle dysfunction, nutritional abnormalities including weight loss, cardiovascular complications, metabolic complications, and osteoporosis, among others, are all well-recognized associations in COPD. These extrapulmonary effects add to the burden of mortality and morbidity in COPD and therefore should be actively looked for, assessed, and treated.
