Ameloblastic Carcinoma: A Case Report with Emphasis on Relevant Diagnostic Aspects.

Ameloblastic carcinoma (AC) is a highly aggressive malignant odontogenic epithelial tumor. It may occur in a pre-existing ameloblastoma, odontogenic cyst, or de novo. The condition is typically painful, highly invasive, and multifocally metastasizing in nature, with the lungs being the most common secondary tumor site. The condition is most often encountered in adult men during the sixth decade, with a propensity towards the posterior mandible. Radiographic examinations typically exhibit ill-defined and aggressive lesions with massive bone loss. Histopathological examination shows classic cyto

Iatrogenic Nerve Injuries in Head and Neck Surgeries: A Systematic Review of Mechanisms, Outcomes, and Prevention Strategies.

Iatrogenic nerve injury in head and neck surgery remains a substantial yet potentially preventable source of morbidity across endocrine, otolaryngologic, and related procedures. This systematic review synthesized evidence to identify the nerves most frequently affected, delineate operative mechanisms of injury, describe the clinical course, and evaluate preventive strategies. Traction on visually intact nerves emerged as the predominant mechanism, with additional contributions from thermal injury, compression, ischemia, and entrapment by suture or clip. The recurrent laryngeal, facial, trigemi

Combined Expression of Cyclin B1 and Cytoplasmic HMGB1 Is Associated With Delayed Cervical Lymph Node Metastasis in Early-Stage Tongue Squamous Cell Carcinoma.

OBJECTIVE: Delayed cervical lymph node metastasis is strongly associated with poor prognosis in early-stage tongue squamous cell carcinoma, but useful biomarkers remain limited. This study evaluated cyclin B1, high-mobility group box protein 1 and 2, and proliferating cell nuclear antigen for stratifying the risk of delayed cervical lymph node metastasis. METHODS: Eighty-two patients with Stage I/II tongue squamous cell carcinoma who underwent partial glossectomy without elective neck dissection were included. Cyclin B1 and the cytoplasmic expression of the other three markers were assessed i…

Epidemiology, Clinical Presentation, and Diagnosis of Medullary Thyroid Carcinoma.

Medullary thyroid carcinoma (MTC) is a rare neuroendocrine tumor originating from thyroid C cells that produces mainly calcitonin (Ctn) and is used as a tumor marker. MTC can occur either sporadically (75%) or in a hereditary variant (multiple endocrine neoplasia type 2, MEN2) due to germline mutations in the RET proto-oncogene. The discovery of MTC in a patient has several diagnostic implications involving a specific strategy, preoperative evaluation of the tumor marker Ctn and the extent of the disease, classification of MTC as sporadic or hereditary using germline RET testing, screening for

Personalized risk prediction of lateral lymph node metastasis in male papillary thyroid cancer: a sex-specific model leveraging central lymph node ratio.

AIMS: Notable sex-based disparities exist in papillary thyroid carcinoma (PTC), necessitating male-specific models for predicting lateral lymph node metastasis (LLNM) to guide treatment. METHODS: We developed and validated two nomograms, based on central lymph node ratio (LNR) and preoperative ultrasound (US), respectively, using data from 433 male PTC patients who underwent total thyroidectomy (TT) with central and lateral neck lymph node dissection. Multivariable analysis identified key risk factors. Model performance was evaluated using ROC curves, calibration curves, and decision curve an

A new twist in the evolution of chameleons uncovers an extremely specialized optic nerve morphology.

Chameleons are unique among lizards for their exceptional suite of anatomical modifications which has allowed them to adapt to and diversify in arboreal environments. Specializations shared by members of this lineage include a trunk with a reduced number of presacral vertebrae, a body that can be mediolaterally compressed or expanded, reduced flexibility in the trunk and neck, grasping hands and feet, a prehensile and non-autotomizing tail, highly developed and independently movable eyes, and a ballistic tongue. Here we use contrast enhanced computed tomography to describe the arrangement of o

Application of Indocyanine Green Fluorescence Imaging Technology in Laparoscopic Duodenum-Preserving Pancreatic Head Resection.

This protocol aims to demonstrate the integration of indocyanine green (ICG) fluorescence imaging into laparoscopic duodenum-preserving pancreatic head resection (LDPPHR) to enable real-time visualization of the biliary tree and minimize the risk of common bile duct (CBD) injury. Adult patients with benign or low-grade malignant tumors of the pancreatic head are selected. Preoperatively, 5 mg of ICG is administered intravenously to achieve optimal hepatic uptake and biliary excretion. During LDPPHR, a near-infrared camera system is employed to detect fluorescence and delineate the CBD and its

Diagnostic and Therapeutic Strategies for Intraoperative and Postoperative Hypocalcemia in Pediatric Thyroidectomy Patients: A Single-Center Experience.

BACKGROUND: The primary goals of surgical management in thyroid disorders are the treatment of malignancy, goiter, and thyrotoxicosis, while adhering to the principle of 'primum non nocere'. Hypocalcemia is among the most common complications, primarily resulting from inadvertent injury to the parathyroid glands. METHODS: A retrospective analysis of 2015-2023 records of Polish pediatric patients who underwent thyroid surgery. RESULTS: In our cohort of 217 children, 98 underwent hemithyroidectomy (HT) and 119 total thyroidectomy (TT). Postoperatively, 32-55 % exhibited decreased total calcium

A rare presentation of Horner's syndrome following cervical epidural steroid injection.

Horner's syndrome is a clinically significant condition caused by disruption of the oculosympathetic nerve pathway, typically presenting with ptosis, miosis, and facial anhidrosis. It can result from serious, life-threatening conditions such as trauma, carotid artery dissection, or malignancy; however, it may also arise from iatrogenic reasons, including stellate ganglion blocks, by other surgical interventions of the head and neck. While exceedingly rare, Horner's syndrome has been reported to occur after a cervical epidural steroid injection. Given the various potential etiologies of Horner'

Carotid Artery Tortuosity With a Vertebral Artery Entering the Transverse Foramen at an Atypical Level: A Cadaveric Case Underlining the Need for Thorough Anatomical Assessment Before a Stellate Ganglion Block.

A stellate ganglion block (SGB) is a therapeutic procedure that transiently interrupts the cervical sympathetic trunk using a local anesthetic, applied not only for pain management but also for conditions such as facial nerve palsy, sudden sensorineural hearing loss, hyperhidrosis, post-traumatic stress disorder, and certain cardiac arrhythmias. The widespread adoption of ultrasound guidance has improved both the safety and the precision of SGB. During cadaveric dissection at Teikyo University School of Medicine in 2024, a rare coexistence of right common carotid artery (RCCA) tortuosity and a

Iatrogenic intracranial venous hypertension treated with intracranial venous stenting: illustrative case.

BACKGROUND: Venous sinus injury resulting in thrombosis is a possible complication of skull base surgery and neck dissection. Although usually asymptomatic, sinus thrombosis can obstruct the dominant cranial venous outflow pathways, leading to a cycle of increased intracranial pressure secondary to venous congestion, which further compresses the remaining sinuses in a positive feedback loop. This can present with symptoms resembling idiopathic intracranial hypertension. OBSERVATIONS: A patient underwent a left mastoidectomy for chronic mastoiditis, complicated by injury of the ipsilateral dom…
