Myelodysplastic neoplasms are inhibited by Brefeldin A through endoplasmic reticulum stress and autophagy.

Myelodysplastic neoplasms (MDS) are a clonal malignant bone marrow disease with high heterogeneity and a high risk of transformation into acute myeloid leukemia (AML). However, the progression of MDS varies significantly, and the available therapeutic drugs are limited. Brefeldin A (BFA), a natural antibiotic, has not been systematically elucidated for its anti-MDS activity and underlying molecular mechanisms. In this study, SKM-1 and MDS-L cell lines and patient primary bone marrow samples were adopted. Multiple experimental approaches, including CCK-8 assay, colony formation assay, flow cyto

From Sanger to Oxford Nanopore MinION Technology: The Impact of Third-Generation Sequencing on Genetic Hematological Diagnosis.

BACKGROUND: Sanger sequencing remains the gold standard for characterizing genetic variants in short DNA fragments (<700 bp). However, the increasing demand for short TATs and high sensitivities in variant detection, particularly in oncohematology, is driving the need for more efficient methods. Next-generation sequencing (NGS) has improved sensitivity and allows for the simultaneous analysis of multiple genes, but it is still costly and time-consuming. Consequently, Sanger sequencing continues to be widely used. In this study, we have compared Sanger sequencing with Oxford Nanopore technology

Genetic Variants in Pediatric Myeloproliferative Neoplasms Revealed by Next Generation Sequencing.

BACKGROUND: Myeloproliferative neoplasms (MPNs) are clonal disorders of hematopoietic stem cells that include BCR::ABL1-negative MPNs such as essential thrombocythemia (ET) and primary myelofibrosis (PMF). MPNs are rare in children, and knowledge of the genetics and biology of pediatric MPNs is very limited. Here, we report genetic variants in pediatric MPNs revealed by next generation sequencing (NGS). METHODS: The study included nine pediatric patients (eight with ET and one with PMF) consecutively diagnosed between January 2000 and June 2023. NGS was performed on an Ion S5 XL Sequencer wit

PNH clones prevalence study in ph-negative myeloproliferative neoplasms: a multicenter Italian study.

The prevalence of paroxysmal nocturnal hemoglobinuria (PNH) clones is little investigated in myeloproliferative neoplasms (MPN) patients. The aim of this multicenter study was to evaluate the prevalence of PNH clones (glycosyl-phosphatidyl-inositol lacking) in 119 Ph- negative MPN patients having anemia, LDH elevation, asthenia and history of thrombosis. All the participating centers performed the standardized diagnostic test by using a single lyophilized template for granulocytes, monocytes, and erythrocytes. Next generation sequencing (NGS) was performed in 2 PNH-positive MPN cases and 13 PN

Management and Long-Term Monitoring of a Young Patient with Pheochromocytoma and RET Mutation: A Case Report.

BACKGROUND: Pheochromocytoma is a rare catecholamine-secreting tumor that can present with severe hypertensive episodes and other symptoms due to excessive catecholamine release. Approximately 30% of pheochromocytomas are associated with hereditary syndromes, including multiple endocrine neoplasia type 2A (MEN2A), an autosomal dominant disorder caused by mutations in the RET proto-oncogene. MEN2A is characterized by the presence of medullary thyroid carcinoma, pheochromocytoma, and primary hyperparathyroidism. CASE PRESENTATION: We report the case of a 19-year-old female who presented with ph

Peritoneal Hydatid Cyst Mimicking Peritoneal Seeding; A Case Report.

BACKGROUND & OBJECTIVE: This study presents a rare case of hydatid cysts involving the liver, spleen, and peritoneum, in which clinical features and radiologic findings initially raised strong suspicion for abdominal malignancy with peritoneal seeding. CASE PRESENTATION: A 64-year-old man presented with vague epigastric pain, weight loss, and iron deficiency anemia. Abdominal ultrasonography revealed multiple splenic lesions suggestive of hydatid cysts. Hydatid serology was negative. Chest CT was unremarkable. Abdominal and pelvic CT showed splenomegaly with cystic lesions, including a calcif

Breast implant-associated squamous cell carcinoma in a male patient: A case report and review of the medical literature.

BACKGROUND: Primary squamous cell carcinoma (SCC) of the breast is a rare tumor type. The diagnosis of this tumor type is more frequently made only after microscopy evaluation. Breast implant-associated SCC is rarer with medical literature review indicating only 18 cases reported in female individuals. CASE PRESENTATION: We reported an unusual case that a man found a 3-cm sized mass on his left breast at first, who had a implant surgery 18 years previously to reconstruct the deformed left breast, as related to the Poland's syndrome. More than 1 year after the mass was detected, the size of th

Prevalence of pathogenic variants in cancer-predisposing genes in second cancer after childhood solid cancers.

BACKGROUND: Second malignant neoplasms (SMNs) are one of the most severe late complications after pediatric cancer treatment. However, the effect of genetic variation on SMNs remains unclear. In this study, we revealed germline genetic factors that contribute to the development of SMNs after treatment of pediatric solid tumors. METHODS: We performed whole-exome sequencing in 14 pediatric patients with SMNs, including three brain tumors. RESULTS: Our analysis revealed that five of 14 (35.7%) patients had pathogenic germline variants in cancer-predisposing genes (CPGs), which was significantly

Intraventricular mass lesions: Still a question of surgical approach?

BACKGROUND: Intraventricular mass lesions represent a small subgroup of intracranial neoplasms with various entities. The anatomy of the ventricular system is complex. Hence multiple surgical approaches are used. We sought to present a single center algorithm regarding surgical approach for intraventricular mass lesion. METHODS: We retrospectively reviewed the data of all patients operated in our institution due to a primary intraventricular tumor between 2011 and 2014. Descriptive statistics were used to evaluate clinical presentation, imaging, histology, surgical approach, complications, an

Future therapies for the myeloproliferative neoplasms.

Ever since their description as "myeloproliferative syndromes" by William Dameshek in 1951, the myeloproliferative neoplasms (MPNs) have been managed by the selective use of rather mundane, nonspecific therapies that rely on either antiplatelet effects or myelosuppression. The year 2005 ushered in a new era of drug development and discovery for the MPNs after the description of the JAK2 V617F mutation and the role this constitutively active tyrosine kinase has in MPN pathogenesis. Subsequently, multiple pharmacologic agents have begun (or are about to begin) testing for the inhibition of JAK2

CXCR3 expression in human central nervous system diseases.

The CXCR3 chemokine receptor, expressed on activated T lymphocytes, is seen within the central nervous system (CNS) in inflammatory conditions where a T-cell response is prominent. However, the distribution of CXCR3 in parenchymal CNS cells is unknown. Using a monoclonal antibody against CXCR3 and post-mortem tissue of patients with and without CNS pathology, we have determined its expression pattern. CXCR3 was found in subpopulations of cells morphologically consistent with astrocytes, particularly reactive astrocytes, and in cerebellar Purkinje cells. It was also detected in arterial endothe

Prevalence of Second Primary Malignancies in Patients With Well-Differentiated Neuroendocrine Tumors.

OBJECTIVE: Neuroendocrine tumors (NETs) constitute a diverse group of tumors. NETs are often diagnosed late, due to nonspecific symptoms. Second Primary Malignancies (SPMs) have been reported in up to 25% of NETs and their incidence has been described as negative predictor of overall survival. We aimed to assess the prevalence of SPMs in patients with NETs treated at a specialized center. METHODS: We conducted a retrospective analysis of patients with metastatic well-differentiated gastro-entero-pancreatic neuroendocrine tumors and lung carcinoids treated with Somatostatin Analogs between 201

Hospital Use, Morbidity, and Cancer Risk by Age Group in Neurofibromatosis Type 1: A Nationwide Retrospective Cohort Study.

Neurofibromatosis type 1 (NF1) affects multiple organs progressively, leading to a shortened life expectancy. This study aimed to identify the complications that are prominent in each age group by comparing 28,082 patients with NF1 and 84,246 matched controls. Benign and malignant neoplasms along with neurological complications were the primary contributors to hospital use across most age groups. Respiratory and circulatory disorders were a common cause of hospital use in children and older adults, respectively. Except for soft tissue and central nervous system-originated malignancies, which a

Differentiating Cerebral Toxoplasmosis and Tumor Recurrence by Thallium-201 Single-Photon Emission Computerized Tomography in a 28-Year-Old Female with Astrocytoma.

Cerebral toxoplasmosis is an opportunistic infection that, by itself, is difficult to differentiate from cerebral neoplasms by conventional neuroimaging. It rarely occurs concurrently in patients with a primary brain tumor but when it does, it makes diagnosis and management more difficult. This is a case of a 28-year-old female, diagnosed with a right frontal pleomorphic xanthoastrocytoma with several recurrences, treated with surgery, radiation, and chemotherapy. Three years from diagnosis, the patient was readmitted for generalized body weakness, fever, and a decrease in sensorium. A repeat

Second primary cancers in patients with acute lymphoblastic, chronic lymphocytic and hairy cell leukaemia.

Improvement of survival in lymphocytic leukaemia has been accompanied by the occurrence of second primary cancer (SPCs). Based on Swedish Family Cancer Database, we applied bi-directional analyses in which relative risks (RRs) were calculated for any SPCs in patients with chronic lymphocytic leukaemia (CLL), acute lymphoblastic leukaemia (ALL) and hairy cell leukaemia (HCL) and the risks of these leukaemias as SPCs. After CLL, RRs were significant for 20 SPCs, and high for skin squamous cell cancer (24·58 for in situ and 7·63 for invasive), Merkel cell carcinoma (14·36), Hodgkin lymphoma (7·16

[Intracranial cystic lesions].

CLINICAL PROBLEM: Intracerebral cysts are common findings in imaging of the neurocranium and are not always clinically significant. The pathological spectrum of intracerebral cysts is, however, very broad and in addition to incidental findings includes developmental disorders, malformation tumors, primary and secondary neoplasms and infectious etiologies, such as cerebral abscess formation, cysticercosis or residuals after congenital cytomegalovirus infections. Intracerebral cystic defects may be caused by inflammatory central nervous system (CNS) diseases, such as multiple sclerosis as well a

From the radiologic pathology archives: gastrointestinal lymphoma: radiologic and pathologic findings.

Gastrointestinal (GI) lymphoma encompasses a heterogeneous group of neoplasms that have a common lymphoid origin but variable pathologic and imaging features. Extranodal marginal zone B-cell lymphoma (ENMZL) and diffuse large B-cell lymphoma (DLBCL) are the most common. ENMZL usually occurs in the stomach, where it is associated with chronic infection by Helicobacter pylori, and is typically a superficial spreading lesion that causes mucosal nodularity or ulceration and mild wall thickening. DLBCL may arise de novo or from transformation of ENMZL or other low-grade lymphomas. This form of lymp

Epigenetic dysregulation of epstein-barr virus latency and development of autoimmune disease.

Epstein-Barr virus (EBV) is ahumanherpesvirus thatpersists in the memory B-cells of the majority of the world population in a latent form. Primary EBV infection is asymptomatic or causes a self-limiting disease, infectious mononucleosis. Virus latency is associated with a wide variety of neoplasms whereof some occur in immune suppressed individuals. Virus production does not occur in strict latency. The expression of latent viral oncoproteins and nontranslated RNAs is under epigenetic control via DNA methylation and histone modifications that results either in a complete silencing of the EBV g
