Evaluating oritavancin for Gram-positive infections: a systematic review of on-label and off-label use.

BACKGROUND: Oritavancin is a long-acting lipoglycopeptide antibiotic approved for acute bacterial skin and skin structure infections (ABSSSI) with potential applicability across difficult-to-treat Gram-positive infections such as osteomyelitis, bacteraemia and endocarditis. This systematic review evaluates the efficacy, safety and clinical use of oritavancin across on-label and off-label indications. METHODS: MEDLINE and CENTRAL were searched (2011-2024) for randomized trials, retrospective cohorts, case series and real-world evidence reporting clinical outcomes following oritavancin administ…

Rethinking Bacterial Osteolysis: Translational Evidence From a Porcine Model and Fracture-Related Infections.

Bone and joint infections (BJIs) are debilitating conditions that reduce quality of life. Key features include interosseous bacteria, biofilm formation, suppurative inflammation, and osteolysis. Traditionally, bacteria-associated osteolysis has been attributed to RANKL-mediated osteoclast activation, based largely on in vitro and murine studies, but emerging evidence challenges this view. In this translational study, a porcine implant-associated osteomyelitis (IAO) model was combined with clinical fracture-related infection (FRI) data to investigate mechanisms of bone loss. In the IAO model, R…

Intrapetrous Internal Carotid Artery Pseudoaneurysm Secondary to Malignant Otitis Externa: A Case Report.

Malignant otitis externa (MOE) can rarely progress to skull base osteomyelitis (SBO) with catastrophic vascular complications. We describe a 68-year-old man with uncontrolled diabetes (HbA1c 9%) who presented with left otalgia, purulent otorrhea, and hearing loss. Initial workup showed CRP 23 mg/L, leukocytes 8,000/µL, and ear pus culture positive for Pseudomonas aeruginosa. Computed tomography (CT) and magnetic resonance imaging (MRI) demonstrated MOE with temporal bone osteomyelitis. Despite prolonged intravenous antibiotics (imipenem, colistin) and topical ofloxacin, symptoms persisted. Thr…

A not too Fragile Bacteroides.

We present a rare case of Bacteroides fragilis vertebral osteomyelitis, in a 20-year-old female with no underlying medical conditions. She presented with a three-week history of lower back pain, myalgias, rigors, and night sweats. The patient was febrile and tachycardic, with borderline low blood pressures. Her initial labs showed elevated inflammatory markers with no leukocytosis. MRI of the thoracic and lumbar spine demonstrated T12/L1 discitis/osteomyelitis with phlegmonous changes and concerns for epidural abscess, causing mild to moderate spinal stenosis. Bacteroides fragilis grew in an a…

Antibiotic-loaded bone substitutes therapy in the management of the moderate to severe diabetic foot infection: A meta-analysis.

In recent years, local antibiotic-loaded bone substitutes (ALBS) have been used increasingly in the treatment of diabetic foot infection (DFI). The meta-analysis aimed to analyse the efficacy of ALBS on patients with moderate to severe DFI (with or without osteomyelitis). With an appropriate search strategy, 7 studies were selected for analysis (2 RCTs and 5 cohort studies). The result showed that the application of ALBS effectively reduced the length of hospital stay (WMD -5.55; 95% CI: -9.85 to -1.26; P = 0.01), the recurrence rates (RR 0.33; 95% CI: 0.15 to 0.69; P = 0.003) and the mortalit…

Intravenous versus Oral Step-Down for the Treatment of Staphylococcus aureus Bacteremia in a Pediatric Population.

Limited data are available regarding optimal antimicrobial therapy for Staphylococcus aureus bacteremia (SAB) in pediatric patients. The pur … p-down were less likely to be immunocompromised and more likely to have community-acquired SAB from osteomyelitis or skin and soft tissue infection (SSTI). More patients in the IV therapy group had a 30-day readmission (10 (25.6%) vs. 3 (5.3%), p = 0.006). Mortality was low (5 (5%)) and not statistically different between groups. Length of stay was greater in p …

Trends in diabetes-related foot disease hospitalizations and amputations in Australia, 2010 to 2019.

AIM: To determine trends in the incidence of hospitalizations and amputations for diabetes-related foot disease (DFD) in Australia. METHODS … 10 and 2019 (APC: 5.1% (95% CI: 3.5, 6.8)), including increases for ulceration (13.3% (2.9, 24.7)), osteomyelitis (5.6% (2.7, 8.7)), peripheral arterial disease (7.7% (3.7, 11.9)), and neuropathy (8.7% (5.5, 12.0)). In people with type 2 diabetes, DFD hospitalizations changed from 18.6 to 24.8 per 1,000 person-years between 2010 and 2019 (APC: 4.5% (3.6, 5.4); …

The most important predisposing factor for diabetic foot ul … pidly worsened over the next 5 days to develop bilateral cellulitis of the feet and right great toe osteomyelitis. His biochemistry showed uncontrolled diabetes (HbA1c: 9.9) and radiology confirmed right great toe osteomyelitis. Postradiation damage to the pelvic girdle in cervical cancer patients: is intensity-modulated radiation therapy safer than conventional radiation?

OBJECTIVES: Intensity-modulated radiation therapy (IMRT) is frequently utilized in the treatment of cervical cancer. Our study compared instances of pelvic fractures, osteonecrosis, and osteomyelitis posttreatment with conventional radiation therapy (RT) versus IMRT in patients with cervical carcinomas. METHODS: Eighty-three patients primarily treated with IMRT were case matched with 83 historical control subjects treated with conventional RT. Pretreatment and posttreatment computed tomography scans were reviewed. Logistic regression analysis was utilized to examine the effects of treatment t…

Management of osteoarticular infections caused by Staphylococcus aureus is similar to that of other etiologies: analysis of 199 staphylococcal bone and joint infections.

BACKGROUND: Acute hematogenous osteomyelitis, septic arthritis, and their combination are considered to warrant especially aggressive treatment if caused by Staphylococcus aureus. METHODS: Our prospective treatment trial of children aged 3 months to 15 years included 199 cases of S. aureus osteomyelitis, septic arthritis, or their combination. These cases were compared with 66 cases caused by other agents, mainly Haemophilus influenzae type b, Streptococcus pneumoniae, or Streptococcus pyogenes. According to protocol, the treatment was initiated intravenously only for 2 to 4 days and complete…
