Hyperacute iris neovascularization following cataract surgery.

RATIONALE: We describe a case of acute neovascularization of the iris after uneventful cataract surgery. PATIENT CONCERNS: A 78-year-old man visited our clinic for cataract surgery and glaucoma management. DIAGNOSES: The patient underwent bilateral laser iridotomy 4 years ago. On ocular examination, the best-corrected visual acuity was no light perception in the right eye and 20/100 in the left eye. We observed pseudophakic bullous keratopathy in the right eye and cataracts and hemicentral retinal vein occlusion (CRVO) in the left eye. INTERVENTIONS: The patient underwent cataract surgery i

Therapeutic Changes in Bilateral Choroidal Metastasis from Non-Small Cell Lung Cancer with Response to Afatinib: A Case Report.

A 49-year-old male presented with a three-day onset of blurred vision and metamorphopsia in the left eye. Fundoscopy and OCT images identified multiple subretinal masses with associating serous retinal detachment in both eyes. Bilateral choroidal metastasis was suspected, and further evaluation revealed a mass in the lower left lobe of the lung with multiple metastatic loci. Ultimately, the patient was diagnosed with Stage IVB (T2aN2M1c) adenocarcinoma of the lung. Genetic analysis revealed an exon 19 deletion mutation in the epidermal growth factor receptor (EGFR) gene. The patient was starte

Post-radioiodine De Novo Onset Graves' Ophthalmopathy: Case Reports and a Review of the Literature.

New-onset Graves' ophthalmopathy (GO) following radioiodine treatment (RAI) and worsening of existing GO are well-described in the endocrinology literature. These phenomena are recognized by ophthalmologists, yet poorly documented in the ophthalmology literature. Two male patients, aged 43 and 62 years, respectively, with Graves' disease without GO, received RAI. Four months later, one patient developed acute GO with unilateral reduction in visual acuity, conjunctival chemosis, lagophthalmos, bilateral severely restricted ocular motility, and lid retraction. High-dose intravenous steroids, fol

A selective deficit in the appreciation and recognition of brightness: brightness agnosia?

We report a patient with extensive brain damage in the right hemisphere who demonstrated a severe impairment in the appreciation of brightness. Acuity, contrast sensitivity as well as luminance discrimination were normal, suggesting her brightness impairment is not a mere consequence of low-level sensory impairments. The patient was not able to indicate the darker or the lighter of two grey squares, even though she was able to see that they differed. In addition, she could not indicate whether the lights in a room were switched on or off, nor was she able to differentiate between normal greysc

Renal supportive therapy for pediatric acute kidney injury in the setting of multiorgan dysfunction syndrome/sepsis.

In the setting of sepsis and multiorgan dysfunction syndrome, the development of acute kidney injury can be an ominous event, particularly in the pediatric patient. In this setting, rapid initiation of renal supportive therapy is likely to positively impact on mortality rates. Therapeutic initiation and choice of dialytic modality are dependent on physician beliefs, as well as patient and organizational characteristics. Patient-specific factors including adequacy of nutrition provision, acuity of acute kidney injury, degree of uremia, and severity of fluid overload all must be taken into accou

[Complex visual hallucinations in partially sighted elderly: Charles Bonnet syndrome].

In two patients, women aged 85 and 80 years, who suffered from visual hallucinations involving living creatures, cars and buildings, Charles Bonnet syndrome was diagnosed. Both were worried that these signs were an indication of a severe psychiatric condition. In diagnosing the Charles Bonnet syndrome the following criteria can be used: presence of complex visual hallucinations, retention of insight, absence of hallucinations in other sensory modalities and absence of delusions. Hallucinations in dementia, delirium, migraine, epilepsy, or in extreme states of exhaustion or sensory deprivation,
