(Cost-)Effectiveness of personalised multimodal physiotherapy compared to surgery in patients with painful cervical radiculopathy. Protocol for a randomised non-inferiority trial (The MOVE-IT study).

BACKGROUND: Painful cervical radiculopathy can lead to substantial and long-lasting limitations in activities and participation. Surgery is considered when conservative treatment fails to deliver relevant improvements or when neurological signs are severe. However, personalised multimodal physiotherapy may offer non-inferior outcomes to surgery, with possibly fewer adverse events and lower costs. Further research is needed to assess the (cost-)effectiveness of personalised multimodal physiotherapy compared to surgery. METHODS: This randomised non-inferiority study compares personalised multim

Environmental and occupational factors in the aetiopathogenesis of systemic sclerosis: insights from a large multinational patient survey.

OBJECTIVE: Systemic sclerosis (SSc) is a rare and heterogeneous disease. SSc pathobiology is complex and it occurs in a genetically susceptible host. Our aim was to describe potential environmental and occupational exposures relevant to SSc. METHOD: An online survey was developed in English to collect detailed information about relevant current/past environmental and occupational exposures. Individuals with SSc were invited to participate through international patient associations and social media. Participation was voluntary and no identifiable information was collected. We used descriptive

Philippine Clinical Practice Guidelines for Periodic Health Examination: Screening for Musculoskeletal Disorders.

BACKGROUND AND OBJECTIVE: Musculoskeletal diseases (MSDs) are acutely or chronically painful conditions that cause limitations in mobility, dexterity, and participation in society. They are a major contributor to years lived with disability worldwide and their burden is higher in countries with a lower sociodemographic index, like the Philippines. The goal of this clinical practice guideline (CPG) is to provide recommendations to primary care providers on screening for MSDs among asymptomatic, apparently healthy children and adults. METHODS: Following the Grading of Recommendations Assessment

Oral health related quality of life and dental caries of adults in Ebolowa, Cameroon: a cross sectional study.

BACKGROUND: The participation of patient in this task can be done with the help of certain indicators such as the oral health related quality of life (OHRQOL). This indicator helps to measure the extent to which oral health conditions impact on a patient's behavior and social functioning. Oral health-related quality of life (OHRQOL) serves as a measure to assess the impact of oral health on the overall well-being of individuals. Good oral health improves quality of life, necessary to improve an individual productivity in life. Therefore the aim of this study was to evaluate the effect of denta

[Cardiogenetics in Piacenza: integrated care and telemedicine for precision medicine].

BACKGROUND: Cardiogenetics aims to identify the genetic causes of inherited cardiac diseases, with significant implications for early diagnosis, clinical management, and prevention of sudden cardiac death in both patients and their relatives. In Italy, however, access to structured cardiogenetic services remains uneven and is often limited to tertiary care centers, partly due to economic sustainability constraints. METHODS: We developed a second-level outpatient model that integrates telemedicine to facilitate access to genetic testing. The pathway includes pre-test genetic counseling perform

Acceptability and efficacy of a smartphone intervention informed by interpersonal psychotherapy for perinatal mental health: A randomized controlled trial.

BACKGROUND: Perinatal depression is a global health concern linked to adverse maternal and neonatal outcomes. Although evidence-based psychological interventions, such as interpersonal psychotherapy (IPT), are effective, women lack access to in-person perinatal care. Smartphone-based interventions may offer scalable support; however, evidence from randomized controlled trials (RCTs) evaluating IPT-informed smartphone interventions remains limited. This study assessed the acceptability and efficacy of an IPT-informed smartphone intervention for perinatal mental well-being. METHODS: Participant

A qualitative study of the discrepancy between patient expectations and assessment practices in 5q-adult spinal muscular atrophy in France.

BACKGROUND: Despite advancements in spinal muscular atrophy (SMA) management, evaluating outcomes in adults remains difficult. Standard motor scales often show floor/ceiling effects and fail to capture patient-relevant constructs. This creates a disconnect between clinical measurements and the lived experience of adults with SMA, particularly in regulatory contexts requiring demonstrable benefit. This qualitative study explored the evaluation priorities, expectations, and preferences of adults with SMA and the corresponding assessment challenges faced by healthcare practitioners (HCPs). Breathe, Eat, Talk: Three Essential Ingredients to Quality-of-Life Outcomes in Movement Disorders.

BACKGROUND: Breathing, eating, and talking (BET) impairments are common yet frequently underrecognized features of movement disorders. Deficits in respiration, swallowing, voice, and speech may emerge early in the course of a disease, adversely affecting safety, participation, and quality of life. Routine neurological examinations may not adequately capture their impact. OBJECTIVE: To characterize the clinical importance of BET dysfunction across several movement disorders by comparing neurological and speech-language pathology (SLP) approaches to assessment and recommendations for improved e

Early Treatment of Post-Stroke Spasticity with Botulinum Toxin Type A: Myth or Reality?

Post-stroke spasticity may interfere with function and rehabilitation and contribute to pain, contracture, care burden, and disability. Botulinum toxin type A is an established focal treatment for post-stroke spasticity, but it is often introduced in the chronic phase, when persistent involuntary muscle overactivity may coexist with secondary musculoskeletal complications. This opinion paper critically examines whether early treatment with botulinum toxin should be considered a myth or an emerging clinical reality. Current evidence from meta-analyses, randomized trials, observational cohorts,

Utility of Exogenous Ketones and Modified Atkins Diet in a Child With GSD VII (Tarui Disease).

Glycogen storage disease VII (GSD VII), also known as Tarui disease, is caused by a deficiency of the skeletal muscle enzyme, phosphofructokinase (PFK). Classical GSD VII is characterised by reduced exercise capacity and exercise-induced myopathic symptoms, exacerbated with carbohydrate intake. No recommended medical or dietary therapy currently exists to improve exercise symptomology in GSD VII. An 11-year-old female was recommended four trial dietary therapies for 8 weeks each: (1) usual diet, (2) 150 mg/kg exogenous ketones pre-exercise, (3) modified Atkins diet (MAD), (4) MAD and pre-exerc

Impact of hearing aid usage on the quality of life of the older adult in KwaZulu-Natal.

BACKGROUND: Despite the benefits of hearing aids for patients experiencing presbycusis, their adoption and consistent use in this population remain low. OBJECTIVES: This study explores older adults' experiences with hearing aid use to better understand its perceived impact on the quality of life (QoL) and factors contributing to inconsistent use. METHOD: Adopting a phenomenological approach, semi-structured, in-person interviews were conducted with eight current hearing aid users, aged 67-84 years, presenting with varying degrees of hearing loss. Data were analysed using inductive thematic a

What's in a name? reframing advanced Parkinson's disease as a multidimensional state.

"What's in a name?" is more than a literary question for advanced Parkinson's disease (APD). Although APD is widely used in clinical practice, what constitutes "advanced" remains poorly defined and inconsistently applied. Existing definitions have often anchored APD to motor complications, treatment complexity, or eligibility for device-aided therapies (DATs). These approaches are useful for referral and treatment planning, but they risk mistaking a treatment phenotype for the whole disease state. They also fail to capture the broader burden of progression, including non-motor symptoms, cognit
