Ocular complications of disseminated intravascular coagulation (DIC) in abruptio placentae.

A 30 year old white woman presented at term with vaginal bleeding, dizziness and blurred vision due to bilateral serous retinal detachments. Extensive areas of choroidal occlusion with focal leakage of dye from the posterior retinal pigment epithelium and choroid were demonstrated by fluorescein angiography. With the diagnosis of abruptio placentae, a dead fetus and disseminated intravascular coagulation (DIC) the patient underwent removal of the uterine contents by cesarean section. DIC and retinal detachments resolved with improvement of vision, DIC, frequently observed in obstetric complica

Neurosyphilis and ocular syphilis in patients with concurrent human immunodeficiency virus infection.

Epidemiologic studies have demonstrated a strong association between human immunodeficiency virus (HIV) and syphilitic infection. Recent reports have suggested that concurrent HIV and luetic infection may lead to an accelerated and more fulminant course of syphilis. Specifically, neurosyphilis is encountered much earlier in such patients. We report two cases in which both neurosyphilis and ocular syphilis were present in HIV sero-positive patients. A review of the literature reveals that 11 of 13 (85%) HIV-infected patients with ophthalmic syphilis also had neurosyphilis. In patients who prese

Subfoveal neovascularization in the ocular histoplasmosis syndrome. A natural history study.

The visual outcome of 74 eyes with the ocular histoplasmosis syndrome and active subfoveal subretinal neovascular membranes was studied retrospectively. Follow-up time ranged from 12 to 109 months (median, 36.5 months). Ten eyes (14%) retained visual acuity of 20/40 or better. Fifty-seven eyes (77%) suffered visual loss of 20/100 or worse and 36 eyes (49%) suffered visual loss of 20/400 or worse. Factors significantly associated with retaining vision of 20/40 or better were: age less than 30 years (P = 0.008); smaller membrane size (P = 0.0002); and absence of visual loss secondary to ocular h

Optics of the eye with air or silicone oil.

Replacement of intraocular contents with gas or silicone oil changes the optical characteristics of the eye. This affects the surgeon's view of the fundus and the patient's vision. Gullstrand's schematic eye was used to calculate the refractive properties of eyes with air or silicone oil in the vitreous cavity or anterior chamber in phakia, aphakia, and pseudophakia. The article gives a general solution of the optical characteristics of the eye with any substance filling the eye or the vitreous cavity. Criteria are shown for selection of appropriate corneal contact lenses for visualization of

Refractive changes from use of silicone oil in vitreous surgery.

When silicone oil fills the vitreous cavity of the phakic eye or the entire aphakic eye, the refractive correction can be expected to change by 5-9 diopters and sometimes as much as 14 diopters. Aphakic eyes become less hyperopic when filled with silicone oil, whereas phakic eyes become more hyperopic when the vitreous cavity is filled with silicone oil. Previously emmetropic aphakic eyes that are filled with silicone oil have only 4-6 diopters of hyperopia, whereas the same eyes have 10-12 diopters of hyperopia before insertion or after removal of silicone oil. Phakic eyes develop 5-7 diopter

Applications of a cannulated extrusion needle during vitreoretinal microsurgery.

A specially designed cannulated extrusion needle facilitates the internal drainage of subretinal fluid during vitreoretinal microsurgery. Case histories demonstrate the use of this instrument in the management of complex retinal detachments including eyes with preretinal and subretinal hemorrhage, proliferative vitreoretinopathy with open peripheral retinal breaks, retinal detachment with giant retinal tear, and combined traction-rhegmatogenous detachments due to diabetic retinopathy. This method of internal drainage appears to have specific advantages over standard techniques by allowing the

Nanophthalmic uveal effusion.

A nanophthalmic patient with hypermetropia, shortened anterior-posterior axial length, and thickened choroid presented with a choroidal detachment and nonrhegmatogenous retinal detachment (NRRD). He underwent partial thickness, 5 x 7 mm, sclerectomies and 1-2 mm central sclerostomies 9.5 mm posterior to the limbus, specifically avoiding vortex veins. Complete resolution of the retinal and choroidal detachments occurred in spite of postoperative ultrasonograms demonstrating residual choroidal-scleral thickening. The effectiveness of this technique in our patient, in light of recent studies demo

Chorioretinal lesions in chronic granulomatous disease of childhood. Clinicopathologic correlations.

The authors examined an eye obtained post-mortem from a patient with chronic granulomatous disease of childhood and clinically apparent chorioretinal scars. Histologic examination disclosed numerous chorioretinal scars with associated retinal pigment epithelial changes and glial proliferation. Special stains for bacteria and fungi were negative. Additional findings were scattered pigment-containing macrophages found in the patient's spleen, liver, lymph nodes, bone marrow, lungs, kidney, thymus, choroid, and retina. The significance of the pigmented macrophages is unknown, however they may rep

Factors related to spontaneous visual recovery in the ocular histoplasmosis syndrome.

In an effort to better describe the phenomenon of spontaneous visual recovery in the ocular histoplasmosis syndrome (OHS), 700 patient records were reviewed. Spontaneous visual recovery was defined as a two or more line improvement in Snellen acuity after a minimum of 12 months of stable visual reduction to 20/70 or less, secondary to central disciform scarring. Ten cases of spontaneous visual recovery were found. Eight patients had recovery of vision only when visual acuity was reduced to 20/80 or less in the fellow eye. One patient experienced spontaneous recovery without visual loss in the

Cryptococcal chorioretinitis and endophthalmitis.

The diagnosis of endogenous ophthalmic fungal infection may be difficult. A patient initially presented with hydrocephalus and later developed a progressive bilateral intraocular inflammation unresponsive to corticosteroids or antituberculous therapy. Further evaluation with vitreous biopsy study and lumbar puncture led to the diagnosis of cryptococcal chorioretinitis, endophthalmitis, and meningitis. Initial treatment with intravenous amphotericin failed to control the intraocular infections. Vitrectomy and intravitreal injections of amphotericin combined with the systemic intravenous amphote

Antiphospholipid antibodies associated with retinal vascular disease.

Two patients with retinal vascular disease also had antiphospholipid antibodies, defined by the presence of the circulating lupus anticoagulant (LAC), anticardiolipin antibodies, or both. One had retinal arterial occlusions and the other had a nonischemic central retinal vein occlusion. The association of thrombotic vascular disease, including both retinal and cerebral vessels, with the presence of these antibodies should be investigated in otherwise normal individuals or in patients presenting with a lupuslike syndrome and retinal vascular occlusive disease. Cytomegalovirus infection in a healthy adult associated with recurrent branch retinal artery occlusion.

The authors report a 16-year-old healthy male who developed branch retinal artery occlusions sequentially in both eyes. A few vitreous cells and retinal vasculitis were associated findings. A complete work-up of the patient for an etiology for the occlusions failed to show any possible embolic source. Spinal fluid studies showed increased protein content and lymphocytic pleocytosis. Serologic studies showed a tenfold rise in the cytomegalovirus (CMV) titer. Other viral titers did not rise. CMV could not be cultured from the patient's body. The case represents a possible cause of the so-called

Clinicopathologic features in two cases of multiple myeloma.

Multiple myeloma is a serious and fatal condition with various ocular manifestations. In two of these cases, pathologic findings included opaque ciliary body cysts, a ciliochoroidal effusion, retinal microaneurysms and hemorrhages, and detachment of both the sensory retina and the retinal pigment epithelium. In one case, a recanalized thrombus within a retrolaminar vein and direct continuity of a superonasal retinal detachment with a large pars plana cyst was observed. The proteinaceous material in the anterior chamber, ciliary body cysts, choroid and ciliary body, subretinal, and subretinal p

Internal limiting membrane wrinkling after argon and krypton laser photocoagulation of choroidal neovascularization.

A retrospective study was performed to determine the prevalence and risk factors for internal limiting membrane (ILM) wrinkling following successful argon and krypton laser photocoagulation of choroidal neovascular membranes. The study included 139 eyes with presumed ocular histoplasmosis syndrome (POHS) and age-related macular degeneration (AMD). Argon treated eyes with POHS had a significantly higher prevalence of ILM wrinkling (30/63, 47.6%) than those with AMD (6/40, 15%, P = 0.0007). Krypton treated eyes also demonstrated a higher prevalence of ILM wrinkling in eyes with POHS (3/12, 25%)

Viscosity of subretinal fluid and its clinical correlations.

The viscosity of subretinal fluid is an important factor in transmitting force created by subretinal fluid motion to the retina. The variation of viscosity of subretinal fluid can explain variations in the effect of subretinal fluid motion on the retina. Pseudoplastic variation of subretinal fluid viscosity was confirmed using a cone-plate microviscometer. To explain the variability in viscosity between subretinal fluid specimens, 14 clinical variables associated with each of 46 specimens were examined. The strongest statistical relationship was between aphakia and lower viscosity.

Gravity-dependent distribution of retinal pigment epithelial cells dispersed into the vitreous cavity.

Release of viable retinal pigment epithelial (RPE) cells into vitreous cavity and subsequent attachment to the retina may be the first steps in the occurrence of macular pucker and proliferative vitreoretinopathy (PVR) complicating rhegmatogenous retinal detachment. Gravity and post-operative position of the patient together may influence where the cells settle, and, thereby, the location of subsequent membrane formation. To study the effect of gravity on the location of RPE cell attachment, 3H-thymidine-labelled RPE cells were injected into 12 enucleated pig eyes after vitrectomy performed to

Vitrectomy for excision of intraocular larva (hypoderma species).

A 2-year-old girl presented with a 2-month history of anterior and posterior uveitis associated with the presence of an intravitreal larva. A Hypoderma species larva was successfully removed using a pars plana vitrectomy technique. A combined traction/rhegmatogenous retinal detachment and epiretinal membrane formation occurred later, requiring a second operation, but a good visual result was obtained.

Retinal vasculitis in polyarteritis nodosa.

Although uncommon, a wide variety of ocular manifestations can be seen in polyarteritis nodosa. These occur as a result of the arteritis or secondary to the associated renal induced hypertension. A case of biopsy documented polyarteritis nodosa is reported in which the patient presented with bilateral iritis, vitritis, and a retinal vasculitis involving both the retinal arteries and veins, a feature not described previously. Patients with this potentially fatal disorder may initially present with ocular involvement; thus ophthalmologists should be familiar with the clinical features of this di

Neuroretinitis in presumed visceral larva migrans.

Neuroretinitis is a rate manifestation of toxocariasis that can occur as a late sequelae to the active disseminated phase of the disease visceral larval migrans (VLM). This case report describes a 14-year-old boy who developed VLM and concurrent neuroretinitis. During the acute illness, the patient's serum ELISA titer to Toxocara canis rose to greater than 256 and a lymph node biopsy revealed necrotizing lymphadenitis with eosinophilia. The constitutional symptoms of VLM may be mild and easily overlooked in children. Toxocariasis is an important diagnostic consideration in persons with acute n
