Use of a primary care online consultation system, by whom, when and why: evaluation of a pilot observational study in 36 general practices in South West England.

OBJECTIVES: Evaluation of a pilot study of an online consultation system in primary care. We describe who used the system, when and why, and the National Health Service costs associated with its use. DESIGN: 15-month observational study. SETTING: Primary care practices in South West England. RESULTS: 36 General practices covering 396 828 patients took part in the pilot. The online consultation website was viewed 35 981 times over the pilot period (mean 9.11 visits per 1000 patients per month). 7472 patients went on to complete an 'e-consultation' (mean 2.00 online consultations per 1000 pat

Variations in corticosteroid/anesthetic injections for painful shoulder conditions: comparisons among orthopaedic surgeons, rheumatologists, and physical medicine and primary-care physicians.

BACKGROUND: Variations in corticosteroid/anesthetic doses for injecting shoulder conditions were examined among orthopaedic surgeons, rheumatologists, and primary-care sports medicine (PCSMs) and physical medicine and rehabilitation (PMRs) physicians to provide data needed for documenting inter-group differences for establishing uniform injection guidelines. METHODS: 264 surveys, sent to these physicians in our tri-state area of the western United States, addressed corticosteroid/anesthetic doses and types used for subacromial impingement, degenerative glenohumeral and acromioclavicular arthr

Influence of insurance status and vaccine cost on physicians' administration of pneumococcal conjugate vaccine.

OBJECTIVE: In 2000, heptavalent pneumococcal conjugate vaccine (PCV7) was recommended for children younger than 2 years, but its high cost relative to other universally recommended childhood immunizations and variability in insurance coverage for the vaccine raised concerns. We investigated the influence of PCV7 cost and insurance coverage on physician recommendation of PCV7 to their patients and administration of PCV7 in their practices. METHODS: We conducted a mail survey from April to July 2001 of a random sample of 833 pediatricians and 788 family physicians in 24 states with different va

Adherence to the 5A and 5R smoking cessation counselling models among Turkish family physicians: a multicenter cross-sectional study.

BACKGROUND: The 5A and 5R counselling models are widely recommended frameworks for smoking cessation; however, their implementation in primary care remains inconsistent. OBJECTIVE: This multicenter cross-sectional study assessed the use of these models among 203 family physicians in Istanbul and examined the factors associated with counselling performance. METHOD: A structured questionnaire was used to evaluate demographics, smoking-related behaviors, knowledge of cessation methods, and the frequency of applying each step of the 5A and 5R models (10-item dichotomous responses with excellent

Opinion and Knowledge of Pulmonologists, Primary Care Physicians, and Nurses About Inhaled Therapy in COPD. Inhal-Epoc Survey.

PURPOSE: Despite the efforts made to improve inhalation technique by both patients and healthcare professionals, significant deficiencies were found in critical aspects of inhaler use and device selection. The aim of this study is to know the current opinion and knowledge of pulmonologists, primary care physicians, and nurses about inhaled therapy in chronic obstructive pulmonary disease (COPD). METHODS: Survey conducted between April-July 2023 by a committee of experts formed by 2 pulmonologists, 2 primary care physicians, and one nurse specialist in inhaled therapy. It consisted of 27 quest

Obstetrician-gynecologists and contraception: practice and opinions about the use of IUDs in nulliparous women, adolescents and other patient populations.

OBJECTIVES: Use of intrauterine devices (IUDs) by US women is low despite their suitability for most women of reproductive age and in a variety of clinical contexts. This study examined obstetrician-gynecologists' practices and opinions about the use of IUDs in adolescents, nulliparous women and other patient groups, as well as for emergency contraception. DESIGN: A survey questionnaire was sent to a computer-generated sample of 3000 fellows who were reflective of the American College of Obstetricians and Gynecologists (College) membership. Determinants of physician discussion regarding tobacco and alcohol abuse.

The U.S. Preventive Services Task Force recommends that clinicians screen adults for tobacco and alcohol abuse and provide appropriate interventions. This study employed direct observation and interactional analysis of medical visits to investigate factors associated with physician discussion of tobacco and alcohol use with patients. New adult patients were randomly assigned to primary care at a university medical center. Videotapes of the visits were analyzed using the Davis observation code. Regression equations related discussions of substance use (alcohol and other substances), smoking, an

General practitioners' knowledge of post-traumatic stress disorder: a controlled study.

BACKGROUND: Post-traumatic stress disorder (PTSD) is common, is associated with substantial morbidity, and is often not recognised in primary care. AIM: To explore whether general practitioners (GPs) have significant gaps in their knowledge of PTSD. DESIGN OF STUDY: A controlled study. SETTING: Primary care in two Scottish regions. METHOD: A validated postal questionnaire consisting of clinical vignettes for PTSD, acute stress reaction, and depression was used to gather the data. Understanding type 2 diabetes mellitus screening practices among primary care physicians: a qualitative chart-stimulated recall study.

BACKGROUND: Early diagnosis and treatment of prediabetes and type 2 diabetes mellitus (T2DM) can prevent future health problems, yet many individuals with these conditions are undiagnosed. This could be due, in part, to primary care physicians' (PCP) screening practices, about which little is known. The objectives of this study were to identify factors that influence PCPs' decisions to screen patients for T2DM and to characterize their interpretation and communication of screening test results to patients. METHODS: We conducted semi-structured chart-stimulated recall interviews with 20 Univer

Primary care in Durham County: who gives care to whom?

The purpose of this study is to describe quantitatively the primary health care system of a defined population. The ambulatory care services of private practices and institutions in Durham County, North Carolina were sampled four times during 1975-1976 to determine the relative contributions to primary care made by specified types of practice and sources of care. All the institutions and 96 per cent of practicing physicians participated. Utilization of primary care services was analyzed by race, sex, age and health insurance status. One striking finding is the predominant role in the delivery

The data and the reality: urine cultures and emergency medicine physicians.

BACKGROUND AND OBJECTIVES: The IDSA has published guidelines for the treatment of urinary tract infections, recommending limiting treatment to symptomatic patients and pregnant females. Our objective is to elucidate current practice patterns among emergency physicians (EPs) in treating positive urine cultures in various clinical situations. METHODS: This study employed a cross-sectional design utilizing a questionnaire addressing nine common scenarios encountered by EPs in the follow-up of a positive urine culture. The questionnaire was conducted using RedCap and distributed via email to curr

Clinical significance of monitoring early symptom change to predict outcome.

Even with efforts to develop medication algorithms for the treatment of psychiatric illnesses, there is no single authoritative method that can be used to incorporate multiple factors in the treatment decision process. For this reason, physicians are faced with the often daunting task of sifting through the numerous treatment options for psychiatric illness to develop an approach that will prove the most successful for their patients. Investigating patient patterns of response, particularly during the acute phase of treatment, and bearing them in mind when developing treatment protocols may as
